This Doctor REACTS video is looking at a classic episode of House MD. House has been admitted to a psychiatric hospital to address his Vicodin dependency and of course, this uncovers some much deeper issues. This is a two parter so I'm dividing this into two videos. Over the two, we'll learn about a variety of mental illnesses, about some medications like SSRIs, and some principles of psychotherapy.

I always did hate that scene with them forcing Freedommaster to face the lack of super strength head-on. Even without any education on the subject it seemed obvious that yeah sure you need him to eventually come to the realization that he isn't a super hero. But surely the trick is in helping HIM come to the realization, not forcing the subject?

Surely, the emphasis should be on why he values being a superhero so much? Does it make him feel special, protected, invulnerable? Why do you need to feel special/invulnerable when you're a person with self-worth & value? Then he might feel less reliant on the superhero delusion & recognise he's valid regardless of his uniqueness. I'd be worried he's got low self esteem or had been sexually abused.

@@skullsaintdead yeah you could basically just tell him being a super hero is great but until you get the alter ego down its too dangerous to be open about it. By the time he's built up a life perhaps he'd no longer need the fantasy of super heroics.

@@Babidi111 right but that's just reinforcing the delusion. confirming he's right by being a professional that agrees with him. you need to understand it and then unravel it, not put him into a high-stakes position where his delusion could get him killed. the whole reason he's in a psych ward is that he CAN'T take care of himself/needs around-the-clock supervision.

Yeah, contradicting a delusion head-on is always a bad idea and has the only effect of worsening the therapeutic relationship

As a psychiatric nurse that’s had an almost compulsive interest in psychiatry for nearly three decades, it feels so good to see a KZfaqr like you advocating for acting on behalf of mentally ill people instead of challenging them.

This episode broke and healed my heart at the same time. You hit the nail on the head, the entire series of House is him trying to rescue himself through others. sad and beautiful all at once. I'm glad we have new young blood coming through training who are open to learning and professional development. it matters, thank you!

This is actually my favourite pair of House episodes - it's a little overblown and often resorts to cliché, but the underlying messaging is quite strong, I feel. Also the cast of guest stars is amazing, Andre Braugher is incredibly talented.

I used to work in the restaurant industry and one of the biggest cultural points of the industry in the US is that suffering is necessary to achieve great things. I think there is this idea that depression and pain are necessary for certain fields of work to be successful and it wasn't until I took a break from restaurants, and went to therapy, that I was able to recognize that it's bullshit. I am not my pain and my pain is not a measure of my success. House is a bit of a dick but I really connect with his character in that moment.

There's also a big difference between simply hard work and as you say suffering. We can live with being sore here and there, the occasional bad customer moments, bad colleague moments, bad boss moments, but if it's nothing but misery, it's simply not worth it and nobody will hold up for that forever.

I think it's general idea about life not just certain fields of work. I see it all the time where people say things like you shouldn't pursue your passions or your passions are just a hobby, your job can't be enjoyable it's miserable and that's how it is. The most insane version of this I heard was when talking about the schooling system, people literally said that school should not be engaging or fun that it's a job and learning isn't meant to be enjoyable etc. It's so insane to me that people say this stuff.

@@ganimol9691 depending on the country I agree (I say depending because I'm not familiar with every countries work expectations) however certain industries are worse than others. One of the ways we struggle in making improvements in any direction is by saying "yeah but so is every industry". The service sector, especially the restaurant industry is known for being worse than most industries. By saying it's a general cultural issue, it's actually very dismissive of the genuine toxic culture of the restaurant industry. The restaurant industry ranks around the 13th in successful suicides and 2nd in suicidal ideation. We also have one of the highest rates of substance abuse out of all industries. (These numbers are reflective of the restaurant industry compared to other jobs in the US) as well as rates 50-70% of people reporting sexual harassment. I get you weren't trying to be dismissive but there's a lot of horrible things that go into restaurants that's more than everyone else.

Them dragging the guy away because he was "too much" and sedating him; sadly accurate. I've seen it, my sister's seen it, it's very well known practice in American psychs; some people are sedated because they don't get family visitation but have insurance, so the faculty try's to squeeze out as much money as possible. One on one Therapy, and or counseling: Inaccurate; Too many people per worker, the only one on one is psych medicinal, 10mins on average using the chart, quick blurb about you're meds, quick blurb about your mood, med switch or med up. I'm sorry, the American inpatient system is fucked and absolutely broken and I just can't contain myself.

Yes, the episode is fundamentally unrealistic in it's depiction of psych wards. What actually happens: you are very likely to be drugged to the point of hardly knowing what's going on, there will be nothing to do for almost the whole day, all doctors will be like asshole doctor, good news is you only see them for 5 minutes a day, at some point you will be so broken you will start complying while not wanting to, then you will be released in the worst state of your life. Stay away from psychiatry.

Fun Fact about the second time, it was racial profiling mixed with just not understanding or knowing Autism; The did all this shit to throw me in; Lie about my mother's testament, when they told me it was happening, I was allowed to say voluntary, they switched that, and lied about me being looked at or seen by Psych at the hospital; guess what? They didn't send the resident psych at the partnered hospital ANY PAPERS or INFO, A Psych they were forcing to do inpatient work when he was a Outpatient doctor (Something he said with more than a little bit of anger). When I told him I had a psych, therapist and a treatment plan, he went fuck it and just had me released in the minimum amount of time (5 days) and refused to treat me.

@@DOOM8976 Yeah, I know that shit. I was locked up for alledged suicidality and schizoaffective disorder. I wasn't suicidal and the psychos lied to me about the diagnosis until I had access to the court documents, which they obviously tried to keep from me. The release papers don't even mention the schizoaffective disorder. The level of lies was insane. The sectretary lied to me about not recieving my request for documentation, while writing down notes about the phonecall on that very request. That's just plain dumb. Those psychos drive people insane by locking them up, abusing them and forcing them to take psychoactive substances and then blame ther victims suffering on a diagnosis they pulled out of the place where the sun don't shine.

Not all systems are like this. Or at least they weren’t. I worked in a number of hospitals. Regular therapy DID happen, groups and activities went most of the day unless there was an emergency and - with the exception of one specialized hospital, forced medication was rare.

I really wish I had encountered psychiatrists with half your empathy, during all my years as a teenager in outpatient and residential care for PTSD in the 90s. Instead I encountered straightjackets and bribed judges. Thank you for your videos. You give me faith in your profession. Maybe I'll try to get help now.

Racism put me in a psych ward as a child. That place was more terrifying than any other situation I've been in. And I've been in some pretty crazy situations.

I agree about that last "touching" scene being really cheesy and unrealistic, but I think aside from that it's probably very helpful for "Freedom Master" to be able to help that woman in a way that doesn't rely on his perceived superhuman abilities. As a testament to the character of...the character, when under the belief that he had super powers, he didn't want to use them for personal gain or to hurt others, he wanted to help people. Maybe him realizing that he can still get that good feeling from helping someone without having to rely on "super strength," or, "flight," or things like that is what he needs.

You REALLY need to react to the next scenes involving House and Holt. They are beautifully written and shot and really dig deep. I cry every time I watch them because they feel cathartic somehow for me, mostly from the neurotic, negative and pessimistic thought patterns. Mind you, unlike House, I only ever think like this about myself and my life, my thoughts about other people are hopeful and happy. Watching the following scenes made me really want to find a psychologist or councilor who thinks like Holt and has really useful insight, like a Detective putting it all together. Sadly I haven't had any luck finding any therapists or councilors who could help me because their suggestions on how to change didn't work for me. More than anything I feel my brain needs to be rewired.

Find a psychiatrist. They're the most logical and therefore the most helpful. Councilors stereotype people too much and don't see things clearly.

This was such an interesting video! I'd love to see your take on "fleabag", in season 2 there's even an episode where the main character goes to therapy but I think the show would generally be interesting to analyse :) I always love your commentary and insight

Yes absolutely agree about the medication. People would not think of saying to a person with diabetes, can’t you just make more insulin. Nor should they say to a person with anxiety, can’t you just stop worrying!

I love how Dr. Elliot was shaking his head while watching Dr. Asswipe talk down to Freedom Master. I have no formal medical training, and even I could tell the guy was just on a power trip. He didn't give a damn if he was helping the patient. He just gets off on causing misery. He needs to have his medical license revoked and find another job. Wal-Mart is always hiring, though he'd be a shitty door greeter, too.

So happy to see this episode! Just happened to comment about this a week or so ago. As someone who has been struggling with opiate addiction and comorbid psychiatric disorders this episode was always really interesting to me. It's interesting hearing your thoughts on "Capt Holt" here as well, despite Houses many, many ridiculous inaccuracies I thought the way they portrayed the process of psychiatry was amazingly well done and his responses to everything were really perfect. The only criticsm I had was kinda how close they got in the end, it seemed a little inappropriate. Also screw that one doctor that tried to "break" Freedom Masters delusion, I hated that so much. Anyways, love your commentary as always, you deserve so many more views, thanks for covering this!

I often think about how people have this idea that their mental illness is the key to the work that they do or create. In truth, most creatives don't make most of their work when they are in a depressive episode. Most work is created in reflection. Probably when they are feeling a bit better or are on the mend. I know I can't think clearly or physically function at times when I am in the middle of the episode. I am more likely to be cranky, totally out of it, and barely hygienic. It's unlikely, that I am going to touch my paints let alone brush my teeth unless I'm manic. The fundamentals of what makes you up as a person, your opinions, your approaches to problem-solving, and your perspective don't go away just because you are taking medication or feeling healthier. Depression doesn't make you a creative person. If you are a creative person you just are a creative person whether you are feeling depressed or not. Because I know so many people that put off taking their medications when they have a big project at work they need to finish and they think that their medications are making them slow & then all that happens is that they experience mood swings, headaches and don't even finish their work because they are withdrawing from their medications on top of the symptoms of their mental illness. Just take your medications or talk to your doctor about the brain fog and in my experience they will often give me a different dose.

This reminds me of the episode 'Good Damage' from Bojack Horseman where this issue is discussed through a slightly different angle as the title suggests, and I think it's handled pretty well.

Other good Psychological/Psychiatric episodes to look at are: - Season 7 Episode 4, Massage Therapy. - Season 2 Episode 9, Deception - Season 2 Episode 10, Failure to Communicate - Season 1 Episode 14, Control - Season 6 Episode 20, The Choice. (That last one is… not truly psychiatric but does touch on certain outdated practices)

I'm just looking at your little plants in the background and they make me very happy - especially the pot with legs... I just potted my own little succulents to bring inside, but they need a few weeks outside with some sun to become well established. Seeing you react to the way they treated Freedom Master, you might "enjoy" watching a couple Dr. Phil clips. (And "Good Damage from Bojack Horseman but that's just because it's a good show altogether) 9:49 - I can actually answer this rhetorical question: as someone who happens to live in a state that to this day stigmatises psychologists and psychiatrists as doctors for crazy people, the issue is that hypertension is something that can be measured. It's not exactly visible, like a broken leg, but it's close. Mental illness, unless it directly shows on your physical appearance, is not tangible in their minds, not real. There is this prevalent belief that depressed people do it for attention, that they are lazy and weak because 'we totally did it without anyone's help, this generation is full of soft snowflakes'. Mental illness is not an illness in their eyes, it's an excuse unless you're completely incapable of functioning on your own - say people with severe autism. Psychologists are worthless, pills are for crazy people and you're just making everything up - and even though I know better I am still hesitant to look for help which I probably need because having this reinforced in most of my life makes me wonder that maybe I am making it all up. My biggest fear is finding out I am prefectly okay; my second biggest is being ridiculed by a bad practicioner or the people in my life I can't cut off.

If your biggest fear is that you’re actually perfectly okay, that is probably a sign that there is something significant weighing on you that is worth taking the time to address. Even if it turns out you don’t have a diagnosable something, you can hopefully find strategies to help deal with what’s bothering you and keep it from worsening. Everyone has mental health, just like they have physical health, and there should be no shame in taking steps to become healthier. You don’t have to be currently in crisis to seek help. I say this from the perspective of someone who, nine years ago, sat in a chair in my GP’s office saying that worrying about stuff was my superpower but I didn’t think I had an anxiety disorder, while sweating, shaking, and barely able to speak from anxiety. Don’t be like me and live with brain weasels for so long that you just think they’re part of your personality. The sooner you get support, the easier it is to recover.

This episode in my mind should have been the end of the show. It gave us enough closure on house and the season finale before closed off on the rest of the cast. It's sad that all this progress house makes in confronting his misery and addiction is just thrown away later.

I agree, they could have done the relatively same story with some tweaks to show he’s at least trying to keep his new coping styles. Like if they really wanted to keep him driving into Cuddy’s house they could have had him try to call Holt, only for him to be out.

I think it's good they continued as addiction and mental illness is never linear in recovery. Some people get better and some people go back to their old ways. It's accurate to real life!

@@stephaniec2892 Not saying it's inaccurate. I perfectly understand that it's an ongoing process. Not a one and done cure like for other illnesses. But there is also the fact it is fiction and a show. Ending the story with House leaving the hospital would not mean "He is all fine and dandy for the rest of his days", but that he is making an effort now, that he is willing to try and change.

My favorite 2-part episode of House. Unfortunately the rest of the season(s) afterward spiral down to where he reverts back to his normal self when this should have been the series finale since it ends on such a high note. Excellent show, in my "head-cannon" this is the finale and going to keep it that way.

I'm not going to lie I show symptoms of pure o before I realized I was constantly trying to backtrack things that have happened to me in my past to see what parts are relevant although some events that I have experienced seem relevant even though these events are very serious

I always wondered if the whole music box thing had any basis in reality, I'm both a mental health patient, and student of psychology by way of voluntary study and general fascination. I equally find the more I learn, the more I can reflect on myself, and examine things I do with my therapist om help progress and move forward as well as understand myself better. Really been enjoying your channel, been watching several videos tonight and subscribed. Look forward to more

Catatonia! I got my Catatonia video My understanding is it was treated with Benzos but very specifically Lorazepam? Edit: this actually prompted me to watch this episode again. Even though I've seen it dozens of times already and I gotta say his Psychiatrist really is spot on. When I think about the Psychiatrist that was able to help me they were able to talk to me in a similar manner, make me understand the same concepts. First time watching it looking at the Psychiatrist's perspective, not House's

so you mentioned the correct way to approach some like "capt holt" would be to talk to them to find how deep the delusion is. while not trying to colude it So would a good question to ask in "capt holt's" case be "were you born with these powers or did you acquire them later in life?"

I wouldn't validated the perception of having powers, but I'd ask something more open like "you say that you have these powers, tell me how you came to this realisation" and "'could anything else account for why this may have happened other than having special powers" or something along those lines. Probe to the strength of the belief without directly challenging it or reinforcing it.

I'm really interested in learning more about why benzos are used for catatonic states? As someone who abused benzos very heavily for years I would never have guessed that would be the class of drug used in treatment since I abused them specifically to get myself into a state of complete numbness emotionally and physically (in tandem with opiates to achieve the physical part). I know they're used to stop seizures in a hospital setting too so that's also why it seems like benzos would just make a catatonic state worse.

Hi! I'm not a psychiatrist, but I am a nursing student who just recently finished my mental health rotation. The way that the psychiatrist I talked to explained it was that there is too much activity in the brain for the person to be able to do anything outwardly, and the benzo's calm this activity which allows the person to be able to take outward stimulus again. I think it's fascinating! Apologies if this is not entirely correct but it is the best of my understanding, I hope this helps!

As much as house can be put into a relatable position, it is important to remember that this is house, and the authors have predestined him to be the most rational people in the room. When in reality, there are no sherlock holmes where the world revolves around giving them the "just so" situations.

So this couple had a baby, and the the baby was healthy, but he never cried. At first, this was a relief to the parents not to have to put up with a crying baby, but then when he didn't start babbling or speaking, they got worried. So the parents took him to doctors, speech pathologists, audiologists, therapists, everything trying to find out what was wrong, but ultimately, they realized it was hopeless, and decided they were just going to raise their mute baby. Then one day when the kid was 10, they were all sitting at the dinner table, and the kid said, "Can you please pass the peas?", and his parents were amazed! And they asked the kid, "Why are these the first words you spoke? Why have you never said anything before now?" The ten year old shrugged and said, "I never wanted anything before."

Have you any content about dissociative disorders? I have multiple dissociative disorders including dissociative seizures (nead/pnes). I've been on waiting list to see mental health services for 5 years now and getting nowhere

Hey, if you're taking suggestions, there's a new animated series called Arcane that deals with mental health through one of the main characters, a little girl called Powder. It's really different from most things you've reacted to but a really high quality show in its own category :) And if you were to watch it, I would recommend doing episodes 1-3 at once (a bit of a commitment I know), because they form a coherent arc when it comes to the mental health stuff.

Hard to watch these knowing I suffered for years on all classes of antidepressants and antipsychotics, yet one Psychedelic experience was all I needed. Now I looked back a decade later, wishing Psychologists and Psychiatrists would realise they haven't half the tools they need to help heal half their patients.

One thing that crushed me watching house was, it felt like the show writers just couldn't let the show progress. I know status quo beats all im show writing, but a wiser, healed House could be a great mentor for other people in the same position he was in once.

Unfortunately there is a stigma regarding taking meds for physical health too - we are told to try yoga for pain, just to relax if our heart rate is too high, and to shove kale up our arses for our mobility

In my opinion They force house to do that which resulted in 'freedom master' almost killing himself. They deliberately make a confrontation with the boy infront of house Allow house to see the state he was in They put house in a situation where his emotion overcome hia reasoning.

I'm certainly not a doctor, but i'm curious as to why you think the music box gift would not have worked in real life. Meaning, I think you had mentioned not putting all patients in the same box with regards to treatment or assuming why they are having the troubles they are having. So, even if it would be extremely unlikely to make a mute talk, is it not possible that that particular gift from someone she may have respected, even if she didn't speak to them before but could observe, could have impacted her enough to make her say thank you? I mean, i'm guessing that the complexities of why people suffer from such illnesses are more complex than humans can even begin to understand. I don't know, just thinking out loud. Glad I stumbled onto your channel!
