I love the idea of collecting key learnings. Do you typically prefer to store these key learnings in a simple shared spreadsheet or do you use some else like a wiki or knowledge base tool?

@Tom Chi: You talked about separating designing for adaptability and designing for scalability. What if you are a lean team that cannot afford to separate it like that?
