William J. Evans, PhD is an Adjunct Professor of Medicine at the Duke University Medical Center and Human Nutrition in the Department of Nutritional Sciences at the University of California, Berkeley. Dr. Evans is the author or co-author of more than 300 publications in scientific journals, has more than 75,000 citations and was the first to describe sarcopenia. He is the co-inventor of a non-invasive and accurate measurement of muscle mass which is strongly related to health outcomes in older people. He is a founding member of the Society for Sarcopenia, Cachexia, and Wasting Disorders.
In this episode we discuss:
• What’s more important, muscle size or strength?
• The difference between lean body mass and skeletal muscle.
• What you should focus on to maintain health as you age.
• Why much of the research about muscles up until now has been wrong.

I’ll continue having my coconut oil cooked 4 eggs daily. I feel good when consuming them & did not hear any compelling reasons why saturated fat is evil. Not sure why everyone is so hard-lined either for or against saturated fat when it seems the truth is that it’s context dependent and differs from individual to individual.

I really appreciate your work. I am a Physical Therapist. I only work with the elderly. I call myself a geriophile😂. My work is always on function and balance…which all stems back to muscle and strength. Unfortunately at the assisted living facilities I work at they get very little to no protein so it’s like spinning my wheels. I can’t tell you how often I want to fill a 3cc syringe with nandrolone D and bring my patients chicken every day just to see what would happen. Of course I never would. But why aren’t we using nandrolone? (I’m not a fan of testosterone because the elderly aromatize heavily so of course it would back fire). I know I know I know there are side effects and possible major health impacts. Anyways thanks for all you do. I just get sad and frustrated because I want my patients to not fall and get stronger. Keep it up and I’ll keep plugging away.

Nathan Toney - gheeze...yet another excellent reason to do everything possible (diet/exercise/lifestyle) to work on STAYING as FAR AWAY as possible from assisted living facilities. I.e., the dearth of quality protein (or ANY) in their diet🤯 ...that just 'seals the deal' for guaranteed decline into advaned sarcopenia😟 AND...in answer to your question about WHY... well, obviously its good old American Capitalism and protecting the bottom $$$ line at its best (worst😪)

Saturated fat is bad and causes inflammation? I was surprised that there was no pushback on that point.

I came to the KZfaq channel to see other people’s response. It kinda seems elevated blood glucose is bad. Glucose levels elevate quicker and more intensely in a body with maxed out energy (fat cell) storage. Excess energy consumption (fat or carbs) maxes out those stores, reducing one’s tolerance to glucose further reducing glucose tolerance. If you’re intolerant to carbs because your energy stores are full, it doesn’t mean that fat will raise your blood sugar. But if you’re constantly eating in a calorie surplus, your energy cells (fat cells) will fill giving you less tolerance when you eat carbs. Thoughts? I think there’s a value to reducing your energy to protein ratio, so that you’re not always running around with a full tank.

Yes it’s confusing, running on fat or carbs for energy, full carnivore or more ketovore, 40 percent fat 10 percent carbs 50 percent protein or 33 fat 33 carbs and 33 protein, what is the best ratio for someone not wanting to lose weight just maintain or add a little weight and build a bit muscle

He lost me when he got to SFA's....those epi-studies he said were confounded by proteins and fats, were also confounded for carbs...and rodent studies with rodent chows are nothing I'd rely on for any sort of argument. So, no exogenous SFA's do NOT cause diabetes.

Yup! Me too! Such a generalized blanket statement on saturated fats - and yah, no pushback! (pick your battles, I guess?)🤷 Aside from that one faux pas, IMO, this was an excellent/informative discussion - ...especially the creatinine urine biomarker for muscle comp. Would LOVE to learn more about that!

Thank you for this interview. I love the new exciting research on delineating muscle from "lean body mass". I remain skeptical when someone says they know saturated fat is bad but I've probably drunk the modern keto/carni Kool-Aid. In his attempt to expand on that, the subjects (might've been rats) were eating 60+% carbs. He didn't say that. I did the math after he said 20% fat and 20% protein. I'm just a guy that has been self-hacking my diet since 2015. If you're eating a diet of 60% or more calories from carbs, it is not going to be coming from broccoli, asparagus, and brussels sprouts. My question: Is it possible that saturated fats become dangerous in the context of a garbage diet?
@barbarafairbanks4578
@barbarafairbanks4578 Жыл бұрын
Dan Engell - EXACTLY!👍 ...and that's why it is never a good idea (especially from a PhD in this field of study) to make a generalized blanket statement, such as this 'I think saturated fats are bad' (at least he qualified the statement with 'I think', and not as a cold hard fact. And that's because many of these studies on food intake & outcomes are not controlled enough to make a hypothetical conclusion so cut&dry like that - and the ones that can be strictly controlled are rodent studies and are going to have a whacky outcome because 1) the study went to extremes to prove a hypothesis, i.e., (rats fed ONLY fat) and, 2) the majority of rat studies are done with genetically non-variant lab rats rather than 'wild rats'. (Lab rats are ubiquitous, cheaper & more convenient to use). Rats that are exact genetic copies of one another are always going to yield skewed outcomes.

Great video and knowledge transfer but it is worth a small mention here that this older guy looks very under muscled, meaning he doesn't appear to have any superior muscles mass on his frame vs an average Joe, maybe thougut he might have more muscle since his research revolves around it.

I agree - in my opinion, he -especially as a PhD in his field - sort of misspoke with his statement that 'saturated fats are bad.' But...at least he somewhat indicated this is his opinion - I believe he prefaced that statement with, 'I think' ...but still - it did, imo, seem to be a bit out of place. Forgivable, nevertheless!😀 Because IMO, the majority of this discussion was spot-on! I learned alot😃 Muscle fiber type was left out of the discussion, much to my chagrin😒 (I did think he was headed there when he said they believe the muscle 'must be changing'' in older adults.) He did say that muscle cell is lost. I thought they might've mentioned, though, that type II (fast twitch fiber) is lost with age. Dr. Peter Attia just recently mentioned this in one of his interviews. Ironically, type II muscle fibers are much needed as we age. Type II are stronger & more powerful - this is the type of muscle fiber that would produce enough quick force to prevent going down for good (i.e helps to quickly recover balance) in a potential trip/fall situation. ....and it doesn't happen with cardio - fast twitch muscle fiber can best be maintained with strength training.

A few months ago, I experienced some palpitations or maybe heart burn or maybe acid reflux right after finishing 2 orders of iced coffee from my local mcdonalds, since it was my first time experiencing it i don't know which one it is lol. That made me decide to go back to the gym after for how many years because of fatherhood & having my first born sons which is 4 years old now because i gained a significant amount of fat, but i can still see that i retained some of my muscle. I use to smoke & drink but quit like close to 10-15 years now. Fast forward to now being active to the gym like 3x a week & having a treadmill at home & doing some cardio nothing crazy just 30-40mins walk + jog just to burn 250-300 calories. & recently experienced again the same sensations, thigh chest & can hear my heartbeat from my clavicles right after consuming a cup of coffee with milk on a empty stomach before eating a meal. Later that evening I Went to the hospital & requested some blood test & some heart attack test as well. Tests came out Kidneys OK Thyroid OK Electrolytes OK Liver OK Heart OK and NO enlargement All they noticed is a little bit of elevation on my creatinine. The doctor asked me. When was the last workout you had? Which is a day before my blood test. What supplements you take? Only multi vitamins, whey protein & some non caffeine preworkout. & explained to me why my creatinine was slightly elevated. & explained also that the palpitation/heart burn sensation happened long before i started talking whey protein & non stim pre workout together with high protein diet. The doctor gave me a prescription of anti acid & it worked pretty well. But also gave my body some rest & minimized my whey protein intake from 2 scoops per day ( 1 in the mornin 1 at late afternoon ) to 1 scoop. I still continue to walk on the treadmill & still lift weights. Will do another blood test soon for that creatinine thing. Planning to add creatine on my stack soon!

I love all this info expect highly concerned about using GLP-1 agonist drugs. I truly believe we need to educate on diet and exercise and lifestyle changes to create metabolically healthy individuals that are strong, insulin sensitive and highly functioning without the use of drugs. People are so accustom to just taking a pill to cure every I’ll with no regard to positive healthy habits. I have a friend that takes ozempic and her health is deteriorating by the day with exasperated gastrointestinal and psych issues, over all weakness and haunting weight loss. This is not a healthy route, in my opinion, to becoming a healthy person. It scares me that people think this is a good weight loss diabetes drug.

Sarcopenia obesity: I have been a sports massage therapist since 1987, I can't tell you how many 110lb females I have worked on who where basically obese, almost no muscle mass. But they " looked good in a dress". I am in no way picking on females, it was just vastly more prevalent in them than male clients. The key to longevity is changing from taste base eating consumption to purpose based eating consumption. To put it simply. If a human eats foods “that taste good” they are going to be eating less healthy foods and eating a greater variety of foods. If a human eats for nutrients only they will be eating healthier and less variety. Easier on the body.
