I asked my friend who was a Tropical Disease specialist at the Mayo Clinic at the time and he said without a test it would be impossible to know exactly.

Maybe a severe case of malaria? Indicated by the swelling of the deceased's spleen. Just a guess.

he begged his family to let him take Koldenim to a hospital on the next flight out, but Kodenim himself refused.

"White man's burden" circa 2021. These people may have been under his observation (something of a problem, but one I'm sure he recognizes), but there is no way they were under his "watch."

Different culture = different rules. 😉 Not everyone lives the same way as us in the West,we're just so used to it. In these Pacific peoples relationships and honouring others has highest currency and not individualism. Everything a person does affects others if not the whole village.
