Second outcome is the truest, cause aizen always starts a fight with kyoga suigetsu and then plays with his enemies, to then surprise them. Ichigo and urahara won because deep inside aizen wanted to loose and because he didn't put ichigo in his kyoga suigetsu to make it so that ichigo can make him evolve to the next level.

Imo, Madara would win, simply because Kyoga Suigetsu operates the same way basic Genjutsu works, they both manipulate the 5 senses, and it was shown in early Naruto that a 3 tomoe Sharingan is easily able to break through those Genjutsu, and even more advanced Genjutsu in the form of Tsukuyomi. So there, let’s see if people will be reasonable here and not drop a ton of salt on my opinion for once.

Aizen is a higher dimensional being than Madara and being on a higher dimension means that all kinds of attacks and hax the lower dimension have will be meaningless.Aizen fused with his kyoka suigetsu which means that his ability doesn't have weakness anymore.He also affected the almighty(the power to alter the future) with his kyoka suigetsu and survived attack that can erase existence.He got so strong that even people with great reiatsu got erased just by being close to him. Madara is planetary at best while Aizen is universal because he kept up with Yhwach.Yamamoto, Kenpachi, Gremmy, Ichigo, Aizen, Yhwach, Ichibei, Jugram, Gerard, they all solo Naruto+Boruto verse no diff.
