Dear Dr 1)What you got in our hospitals r passive cases seeking the treatment for the symptoms after confirmatip. 2)If a case is reported all the contacts are followed up and monitored for the symptoms/ signes; if suspected with symptoms they tested for confir mation and treatment. 3) when a case is repoted without any contact histroy and from the infected area we should assume that there is a community just started. 5). Based on the above suspicion of community spread , we shoud start screening the entire community of that suspected area to rule out the community spread. The causes found during such survey shall be treated and this process should continue till the last case in the community is eliminated. 6) This community surveillance is called active surveillance which is followed by our public health department to bring down all outbreak of communicable diseases. We have eliminated Small pox, plague, cholera, VPDs, etc 6) So far we we have seen only the tip of the iceberg as per the openion of many epidemiologists world over. 7) So social distancing, active community surveillance, case cofirmation, treating the identified cases, tracing the contacts- for any infection and treatmrnt till the lost case is eliminated, effective IEC and community participation may the holistic solution to the current problem.
