I wish somebody would please explain what treatment is needed for spinal stenosis not caused by a herniated disc, but caused because of ligamentum flavum and other causes. Seems nobody covers that issue.

I have very little nerve pain or back pain but my calf is very weak and I have pins and needles down back of leg and into left side of foot. I walk with a slight limp. MRI can’t be done for a while yet. Anyone with similar experience? I herniated l5-s1 years ago and assume it’s reinsured but the lack of any real pain is baffling. Anyone have ideas?

Have a question Doctor, I have sciatica and spinal stenosis , as soon as I had them I had trouble with both knees , hard clicking, pain and was diagnosed with arthritis. Do you think by me always leaning forward due to the stenosis when walking has anything to do with my knees being like that? Thank you Doc! Stay well!

Certainly, it's plausible that your altered posture due to spinal stenosis might have had some impact on your knees. However, there's a more direct explanation for your knee problems. Spinal stenosis involves the narrowing of the spinal canal, caused by thickening of the spinal ligament and enlargement of the joint walls, which are influenced by arthritis. This arthritis affecting both your back and knees could be the primary reason for your knee issues. It's essential to address the root cause and manage your spinal stenosis and arthritis symptoms with the guidance of a healthcare professional. Stay in touch with your primary care doctor (either medical or chiropractor) to help you manage these issues.

What about the other buldging discs causing multi level radiculopathy?

You've highlighted a critical point. Indeed, the relationship between bulging discs and radiculopathy (such as sciatica) is complex. Bulging discs, in themselves, typically don't cause radiculopathy. Most often, radiculopathy results from inflammation of the nerve root. When a bulging disc occurs, and the annulus (the outer part of the disc) remains intact, the nerve root usually doesn't get irritated by it. Every nerve root normally contacts the annulus without irritation. However, if the annulus tears, the nucleus (inner part) of the disc can be highly irritating to the nerve root, causing inflammation and radiculopathy. It's essential to note that the only way a bulging disc can cause radiculopathy (and therefore sciatica) is through compression. It's concerning when people attribute all their pain to a bulging disc, as many individuals have bulging discs on MRI scans without experiencing pain. Therefore, it's a classic example of a false positive, where it appears on the scan but might not be the true source of pain. Addressing the actual source of pain is crucial for effective treatment. Focusing on accurate diagnosis and understanding the specific cause of radiculopathy is essential to ensure appropriate and effective interventions.

Will you please speak about your thoughts on ultrasonic surgery for decompression versus a traditional laminectomy for lumbar stenosis? I would love to hear your thoughts, as that is my challenge at the moment- severe lumbar stenosis l4l5 with leg numbness/weakness and disc disease. Thank you.

Sure. The ultrasonic knife is a tool spine surgeons can employ to remove bone, especially in procedures like Laminectomy surgery, which involves removing par of the lamina bone and decompressing the ligamentum flavum which is typically thickened and compressing the spinal canal. While some surgeons prefer the ultrasound tool, most still rely on the traditional technique utilizing a diamond-coated bit on a high-speed pneumatic drill for bone removal. In the end, it boils down to the surgeon's skill and expertise. A skilled surgeon can achieve excellent results regardless of whether they use ultrasonic or traditional methods to remove the bone. Instead of focusing solely on the tool being used, it's crucial to pay close attention to the expertise and experience of the person wielding it. Their proficiency and understanding of the procedure will significantly impact the outcome. So, when considering surgical options, it's wise to prioritize the surgeon's approach, experience, and results over the specific tools they use.
