If you wipe beforehand on him, you have to fully wipe and not use anything like DI.

Switch your damage meter to track "Friendly Fire" and you will see when/who causes you to fail the achievement.

Hi, yes it's DBM. Although it can be a little slow at times, so better to watch for the debuff near your mini map
