Forsure. It's tough not getting in and nobody likes to talk about it, could make you feel disappointed and lost. But so many people have to reapply, I'm sure it's reassuring to see someone living their dream even if the path wasn't drawn out how they imagined

Wait...what about a dad with a guitar room says he didn't get things handed to him? He's super young...yea he had a massive amount of help that most of us dont/didn't get. I didn't start med school til I was 36. The majority of kids there had a LOT handed to them. Not saying he won't be a good physician, just saying he probably got all the best MCAT courses...he was able to apply to 25 schools lol check the price tag on that alone

@JakeGoodmanMD I need some advice from a psych standpoint. I have Cushings Disease due to a pituitary tumor. Due to that, I have pretty severe anxiety and some depression. My neuroendocrinologist for SOME reason believes my hypercortisolism is functional. My labs disagree, ACTH is 93.8, 78, very High as is cortisol up to 1,000x normal esp late at night. What does one do when a physician refuses to DX or treat? She won’t send me for IPSS, insists it’s functional/psychosomatic. I’ve asked psuedo Cushings patients what their ACTH levels were like - 24-30! And cortisol largely normal. It can’t be correct. I’m on year 17 of cyclical Cushings. I don’t want to die or leave my son behind. Psych says it’s Cushings Disease too! I’m at a loss.

I really enjoyed this interview! I love how Dr. Jake without saying it directly said, “I want to work to live, NOT live to work.” His personality screams he loves and cares about others but also cares about his well-being. Clearly he’s excited with his career and the role he plays in society without jeopardizing his mental health, family and social life. Dr. Jake’s attitude towards balancing work and life was very eye opening to me and hopefully to others watching this video.

Thanks so much for your videos! Since you're doing videos of students and other healthcare workers, you should interview someone in EMS (EMT or paramedic)! I've been working in the field during my 2 gap years and it has opened my eyes so much to pre-hospital medicine, which I think a lot of people in more "traditional" healthcare professions are minimally aware of. It's a really unique and interesting field that I wish people knew more about!

I follow Jake on KZfaq and he just has the same lovely feel to him in this interview. I wish he had been a psychiatrist in my care team over the last 30 years or even into my future. I think I'd have had better outcomes with his wisdom and support.

I am a medical student in Kenya ( I’m in my 4th year heading to 5th year) and I take my pathology supplementary paper Tomorrow and I love how he said “ being mentally is not your fault” I have struggled with mental illness during my years in medical school and I used to blame myself for being “lazy” and not as “disciplined” as my colleague.. but I’m learning to remind myself that mental illness is just like physical illness it’s not any less severe. I’ve learnt to learn it the hard way after consistently invalidating my struggles. If you are in the same space you got this! We all do! Thanks for the interview!

U got it sis! I am rooting for you as a Kenyan Psychiatric Nurse far away from home. I went to nursing school wanting to be a midwife but learning about how the mind works and seeing how people from all walks of life are affected with mental issues I switched and and would keep pushing to be the best I can be in Psych.

Cannabis hyperemesis and the treatment was something I also found quite interesting. The treatment in the ED is generally capsaicin applied to the chest and abdomen, OR hot showers. Cannabis HE in some people (mainly long term users of cannabis) causes the TRPV1 receptors to inactivate. It's believed that the capsaicin or hot water essentially stimulates the nocicepters cutaneously, and thus results in reactivation ofTRPV1 firing which restores proper nervous system singaling. TRPV1 is found in a few places, but the two key areas here are gastric enteric tissue, and cutaneous tissue.
