It depends on if you can afford it or not, but I would say that it is worth it. It is better than every other 359 Trinket, so you'll only replace it when you get 2 372 ilvl Trinkets from the Heroic Raids.

Yeah, he is definitely a step up in difficulty over the other bosses and can be a huge pain - Are you doing it on 10-man or 25-man? We did on 25-man this week and used a 3rd Tank to help corral the adds to keep the fire off of the raid and it helped a ton. I'm not sure if that's worth doing in 10-man though since there's less adds and it might not be worth dropping a DPS. The main thing with the fire is just positioning, the fireballs always get launched at the adds so the adds need to be away from the group, ideally on the outer ring, and the Tank has to move them quickly before the fireball lands and the ground-fire spawns so that they don't regain energy and get more damage / movement stacks. We used Hunter Misdirects to help get them out, and also asked people to not use AoE / Earth Elemental / Army of the Dead etc. so that the adds went straight to the Tank - we also tried to let the adds die near the edge of the ring in P1 instead of the in middle of the room so that they didn't spawn right under the boss, which can lead to fire in the raid stack. And then we just rotate the boss as the adds get kited around so that they don't get hit with the Boss' Breath and so that the fire doesn't catch up with the Raid. It also helped a lot to use a Druid tank with the Infected Claws slow Talent (or whatever it's called), otherwise assigning a Frost DK with the Chillblains Talent or a Hunter Trap rotation for slows helps a ton to allow the Tank to kite without getting smacked too hard.
