To answer the first madara one, its because kakashi lost the sharingan because madara stole his during the 4th shinobi world war and sasuke couldnt due to mangekyou sharingan overuse, it is stated that as long as a susano user hasnt gone blind from mangekyou overuse he can still use the susano. That being said if you loose your sight because of another reason you can still use your susano, even if you do not have eyes, madara hadnt lost his eyes due to mangekyou overuse and that is why he didnt lose the ability to use the susano. The second one of madara i dont think he used it because 1. He had the 9 tails in a susano and i think he thought that was just better 2. If hashirama's wood is the type of wood that is resistent or even immune to fire then thats a reason.
