Please try to make your camera still because basically we have to zoom it and then watch it for a clear view and your camera was not still and that caused lack of focus Kindly work on that

circuit diagram first you show.According show in bread board.In bread board internal connectivities are not visible. what you changed from previus circuit diagram , that most explain.
