Diagonal bars are required in addition to the straight bars if the opening size is 450mm to 900mm, in case of circular openings and the slab thickness is 150 mm or more at the top and bottom face of the slab.

Yes. Both top and bottom diagonal bars are required.

Mam i can't understand how you calculated 250mm2 steel in opening...Did you multiply length& dia of steel?

250mm^2 is the area of main reinforcement in the slab, as per the SP-34 code we need to consider one-half of the main reinforcement(slab) around the openings. 250mm^2 area was taken for example.
