For optician training or ABO NOCE study, here is a big-picture overview of what a prism is and what a prism does. This video lesson shows you how you can use the predictability of prism to alter the position of an object as it is perceived by our brain. I was hired as an optician at an early age and really only new there was a job that sounded very interesting and offered an opportunity to challenge myself and grow as a person wishing to enter the field of optics. It has taken much time and learning to even be comfortable to explain to a patient the complexities of an Rx, and I really had to take a moment to thank you for the time and methodology you have taken to teach your own personal experiences. I started off as a optometric tech after never being in optics ever. The place I work is very busy. They found me to be hard working and want to promote me to salesperson but I’ve only been in optics for 3 months and all this information is so much to take in at once. Really appreciate these videos because me knowing what they need will make me and my customer very happy and that’s what I want. Getting prism glasses in a few weeks. You explained this SO well. I went to a specialist and when she put the prisms on my eyes I Cried. I no longer saw double. I’m 55 and this has been going on since my 20’s but now it’s getting worse. Driving is miserable… I close one eye to stop the double vision. As a Dental Hygienist, I’m all good seeing close up. Wish I could use Surgical Loupes. Again , this was very informative. Great Lecture.

I am 70! I have worn glasses since 3rd grade. I’ve worn contacts for years but because of dry eye I’ve been wearing glasses. I’m very nearsighted. And have a high astigmatism in my left eye. Around 40 I started wearing progressives with no problem. As I age I have problems with my vision now. My eye exam was last week and the optometrist suggested prism lenses. I have never heard of prisms. I picked up my new glasses yesterday 🎉I put these on a miracle! I can see clearer then I ever have in a very long time. It’s truly incredible

I realize your videos are geared toward opticians, but as a consumer they are incredibly helpful. This video especially! I'm amazed by how complicated optics can be but yet there is legal, allowable margins of error! I've discovered I'm quite sensitive to things being just slightly off, needing to know how to explain what I require in terms opticians understand.

Thank you - I'm a client, not an optician. I have been spending a lot [too much] time on my iPhone. Long story short I have a bad astigmatism. I was headed to the grocery store and all of a sudden I was seeing double cars [turned around and came home]. Then I went to my eye Dr [I recently moved to FL], and saw the younger Dr, not the older one. He seemed pleased with a 25/20 and said we're done. Too bad he didn't ask me if I was satisfied! Well, when I picked up my glasses, I was in fact not totally satisfied so I decided a second opinion was necessary. I talked with the optician at Costco and she said the word prism. Anyhow, I had her do a basic exam and picked up my new glasses. My desire is to have the best/clearest vision for driving. I can see fine up close if I remove my glasses. Anyhow, I am waiting to hear back just what she/Dr. was able to correct my vision to. Now, I need to go back to the older Dr #1 and ask if he knows how to work with prisms. What a learning experience, and your video helped a lot. The younger Dr #1 had no prism correction in my prescription. He must need a class or three in understanding prisms. Gotta love it when the client knows more than the Dr, thanks to your video.

Thanks, nice clear explanation. I'm not an optician but I studied physics so I know the basics of optics. I have homonymous hemianopia since I suffered a stroke three years ago. I've hunted around for treatment options and found out that prisms can be fitted which shift an area of the visual field frome the affected side to the good side. I'm currently trying to find an optician/optometrist who can do this for me, so far the ones I've contacted have been no help. Handy to see this from an optican's viewpoint where the brain is essentially a black box which they don't really deal with. Hemianopia is a neurological condition, my eyes are fine except for (-12) myopia and moderate astigmatism.

If you studied physics then you could probably sort out the problem on your own. You could experiment with Fresnel press-on prisms which are pretty inexpensive and easy to work with. Email me through the OpticianWorks website and I'll get you the how-to guide. John

@@LaramyKOptical Thanks, that's a great idea, I'm pretty handy with fiddly work (used to build pcs and replace phone screens for people before the stroke.) The loss of vision and hand-eye coordination since the stroke has limited my abilities for a while but I did manage to build myself a new pc last month so I reckon I'll be up to sticking some prisms to my glasses without wrecking them ☺

Hey! There is one point you did not point out, that is how to decide if BI should be on left or right eye. Or BD on left eye or BU on right eye should be applied. I found the answer, if BI is in the wrong eye, when you look at a pair of speakers playing music, it will appear the sound is not coming out of the speakers but somewhere next to it. Hence correct prism is perfected when sound and sight overlap is achieved and it is something I had to work out myself as a patient and ask the optometrist to make changes to the prescription until I had the BI perfect.

Sounds like you need a good eye doctor. I noticed regular optometrists are constantly afraid of giving patients prisms. A well trained doctor specialized in related issues such as strabismus will prescribe the correct prisms with appropriate positions without a question.

I'm having a hard time reading prism in a lensometer if there are two directions. For example if it's 3 BO and 1.5 base up. The base out is fairly easy to read, but the 1.5 I just don't see or get. Can you help?

I am an optician and I have been working in a finishing lab for 4 years now and I have never had to work with prisms because my optometrists do not prescribe it but in case in the future and it is brought to my lab, is there like a calculation, steps or markings I need to consider first before fitting it for my patient. Do you put in a seg height on a single vision lens that is of low rx and has 1 prism up and 1 down? Does changing the position of the lens affect the ground in prism on the lens?

Might help you to watch some of the other videos... like creating prism using SV stock lenses. SV lenses don't have segs. This reads like it might be best to leave it up to someone familiar with finishing eyeglasses. John

Im a little confused , is the base or the top of the prism controls the position of the object ,cus from what I know the prism is described by it base!

You are correct in that prescribed prism is always described by the direction of the base (i.e. base in, base up, etc.). Just know that when viewing objects through that prism, objects will appear to be shifted in the opposite direction of the base (or toward the apex).

I do have a question though - if a patient is prescribed prism for both distance and near then are both the distance and near prisms worked into their varifocal lens? Or is it just the distance prism in it? Thanks again

I'm not aware of any kind of prism that is distance or near specific. I'm not aware of any kind of prism that is assigned to the distance or near area of a lens design. Now, just because I'm not aware of it doesn't mean it doesn't exist - but - I've never come across anything like it. Prism used in near vision SV sure. Prism used in very high plus, low-vision readers yes. I suppose I've never really thought about it but reading prism at the PRP on a progressive is just below the distance circle. I've always assumed it is present in the distance area not in the corridor or near.

@@LaramyKOptical According the the gentleman who made them, The lens was actually constructed using two single vision lenses, each with different prism amounts, cut in half and combined to form one lens. Lots of hand edging!!!

Do you need a correction if you have a prism prescription of 0.5? How is vision distorted with a minimal prism diagnosis?

We see with our brains not with our eyes. If 0.5D of prism helps you focus then you may need 0.5D of prism. But --- yes, that is a rather low amount. Your practitioner should be able to tell you the how and why they are suggesting it. They should be able to demonstrate it in the phoropter for you. You could try a press-on and see if it works before spending a lot on a complete pair that may or may not work. John

Glasses with prism. You need to be sitting behind a phoropter or trial frame with someone who knows which way to spin the lenses. You are looking at a letter and when the two fuse you got it. John

Can prisms help a really bad gait? I understand they can for people with vision problems but as far as I know things are clear to me and I believe objects are in the right spot. I am in tons of pain and have seen tons of drs and PTS but I continue to walk like a duck and off balance. You just get sick of spending money on things.........I guess my question is can prisms help people who dont have any known visual issues?

I have a question. If we are to make eyeglasses with a prism to address convergence insufficiency. Which PD do we use? Distance PD or Near PD? Eyeglasses will be used only for up-close.

Basic SV readers with required prism would still be made with a proper monocular NEAR PD. I guess convergence might be a factor but I honestly don't know. Refraction would/should be able to sort it out. I don't know the formula for doing the prism required for true low vision (very high + readers) if that is what you are asking about. John

That would be a question for an ophthalmologist and/or a contact lens specialist. Considering what prism is used for it would make sense but ask.

Worked with no. Familiar with yes. Do they have a place in the lens offering world? Yes. BUT that should be a 1 in 1000 scenario (maybe 1 in 10,000) not an everyone or a 1 in 10. I'm sure it works for some people, heck so do many placebos... But it becomes a self serving, self affirming cycle. I just fear the fancy machine, fancy talk and fancy brochures and high costs etc. create an unnecessary demand - perhaps preying on those a bit vulnerable to a bit of snake oil. Please don't get me wrong - if there is a legitimate need and the lens provides a solution then great. I see you watched What Is Prism so you may have a problem that neurolens might help. But you have billions with a b of folks wearing progressives with little or no issues. Maybe we should watch out for creating fixes for something that isn't broken? So if you have a problem and neurolens is supposed to fix it ask for a money back guarantee. Just my opinion. John

An object can't be off center. An object is just wherever the object is. Our perception of where the object is and what we process in our brain can make the object appear off center. We can use prism to trick the brain to perceiving it as being where it really is. Think of grasping for an object and in reality it is 3" to the left. You could use prism to trick the eye/brain into seeing the object 3" to the right so you could grasp it.

I really think my prism glasses were a bad idea and made my double vision MUCH worse. I shouldve just dealt with the slight double vision I had before, when my eyes were trying to focus at maybe TV length of 5' away. With the glasses, my eyes got used to looking at the image on the glasses lens only 1" away. Now things look double if they're only a foot away. The glasses were a terrible idea. The optometrist screwed me over with terrible advice. But it's too late now. The damage was already done. The glasses made my eyes cross in more to focus on the glasses, so I basically stare at my own nose now. I never shouldve gotten glasses. They made me worse. I only had a slight double vision before. Now it's really bad. The glasses made me focus on the glasses, so now I'm dependent on the glasses. I was better before the glasses. I shouldve just lived with my eyes how they were. I used to be able to see fine within a room, cuz my eyes were trying to focus at TV distance. Now I get double vision everywhere, even within a room, cuz my eyes are trying to focus at GLASSES distance. That was a terrible decision. I should've done my own research instead of trusting the optometrist. These prism glasses definitely made my eyes much worse. They really screwed up my life. I'm really upset that I didn't do my own research. 😢

We see with our brains not with our eyes. If you are struggling with daily life activities you may need to retrain your brain to see without the glasses. Or find another doctor, maybe one who does vision therapy and you should be able to return at least to where you were before you tried the pair with prism. And hopefully you understand I'm NOT a doctor, don't claim to be one and I'm not suggesting that prism is either "good" or "bad." John

I as a patient, you are told you will have a prism added because your vision field is off. (According to. Vision Field test, I would guess?) Then another eye doc says, no, we RARELY do prisms unless there has been a brain damage for various reasons. In short... you don't need the prism because your brain needs to train to know where things are positioned, not the other way around. As your brain tells the eyes what they are seeing. My question: Do I need or do I not need the prism added to my eyeglasses?

If you suffer from some form of double vision and prism allows you to see things normally then, yes you do want prism. Beyond that the use of prism would be the realm of a doctor - IMO preferably an ophthalmologist who is familiar with its use. Don't listen to some guy on KZfaq. John

My optician didnt believe me when I had my eyes tested & mesed up my prescription so Ive lived with it for another 3 years untill December but Im still waiting for an appoinment on 14th Feb. Im so miserable.

Sorry - these videos are for opticians not ophthalmologists or optometrists. Opticians are the people that make glasses. We don't have anything to do with medical conditions.
