one tiny doubt sir. i think at 13:29 sir means "no need to give OXIMES in this condition" (condraindicated i believe) in case of carbamate pois,. rx. thanks sir for the explanation.

Sir today i got a case of monochrotophos poisoning the patient was a 16 y.o female who ingested about 200ml of substance , she had pinpoint pupils, sweating excessively , and muscle fasciculations but her lungs were clear sir . She was given atropine loading dose by 108 staff , i gave her a stomach wash and 3 2mg doses of atropine with 15 min gap and Pralidoxime 1g loading dose and referred her to higher centre. Is this atypical presentation without increased bronchial secretions commonly seen Sir?

Give a dose of 0.5mg Atropine initially..If it's not a Op poisoning or mild poisoning..there will be dilatation of pupils and increase in heart rate..It helps to rule in or rule out OP poisoning in Emergency room when the history is lacking..(Later to be confirmed with acetylcholinesterase levels..a 50% decrease in lower normal limit is significant)..

As sir added..New guidelines give importance to Increase in heart rate and clearance of tracheobronchial tree..as a response parameter to atropine..
