This video caught my eye since I had femoral spiral compound fracture when I was 3 years old just by jumping off running board of my Dad's pickup. I am now 72 so LONG time ago. My treatment involved casting and 6 weeks with my legs suspended in the air in hospital for 6 weeks. By the time cast was removed had apparently quite a bit of muscle atrophy and took awhile to walk normally again. Recovered quite well but would not want to be immobilized like that now! Love your informative videos Dr. Webb!

Wow, that is amazing--walking immediately after surgery! 100 years ago and this would have likely been a crippling injury or death. 50 years ago and it was weeks and weeks of recovery. Nowadays, you are walking within 24 hours of the accident and sent home the next day. Thank you science.

It's in part due to us learning more about the very damaging effects of atrophy and trying to reduce immobilisation as much as possible- the patients cannot use their leg as usual right after surgery, but they're strongly advised to move it anyways just so they don't end up having to do 2 years of physiotherapy after :) But science really did come a long way!

Thank you for posting this informative video, it was very helpfull in understanding trauma surgery. 1,5 years ago I broke my femur bone from falling down a roof, it was only about 3 meters and I was 30 years old, but I fell very hard. Long gammanail surgery happened and it took me 6 months untill they removed the pin, I was unable to walk normally when the pin was inside my leg and was able to walk without sticks 3 months after surgery, but with a wobble. After another 3 months they remove my long gamma nail and I was able to walk again after a few days, without a wobble and like normally. The pain was something I can't describe, very thankful for surgery and people that work as surgeon, your work is really amazing piece of art and helped me. Also back in the days someone would die from femur fracture, nowadays surgeons can do so much its amazing.

Watching this video and when I saw the x-ray that made my legs hurt-ouch! I remember seeing the competition when Tony Hawk performed the 900 for the first time in competition and broke his ankle. If memory serves me correct I believe three screws and a plate were put in to stabilize everything while healing. Hopefully Tony heals up with no problems.

Great video!! 🔥 When I was 3 years old I had titanium rods placed in both of my tibias and both of my femurs. Crazy to see the growth and how the rods got “smaller” as I grew. When I was 14 I fell and dislocated my knee and popped the rod out of the femur but not through the skin. They quickly put me under and took that one out 😂

A broken femur is a serious injury that requires immediate medical care. Let me fist say that pain was unbearable, even after they gave me morphine it still was hurting. I had fallen off a ladder at work in 2005 I had a Broken femur. They put about 8 screws and a plate inside of my left leg, after that recovery I was in so much pain, I would come home from work and just sit in my easy chair for an hour or so. Then about 2011 I had total knee replacement by Dr Crovetti in Henderson NV, what an amazing Dr and staff, I feel much better now, thanks Dr Crovetti for all you did.

Not a PA but worked in an ortho practice for a few years before starting med school. Going to depend on the provider but essentially seeing post-ops/follow-ups, doing joint injections, first assisting in every surgery, rounding in the mornings if you’re doing in-patient cases like joints/spine. A lot would let their PAs see new patients and manage them within their scope of care, but some wouldn’t and wanted to see every new patient. I think a lot of it just comes down to how much they trust you. Of all the subspecialties in ortho I think hand/wrist PAs got to do the most and theres no rounding since its all outpatient cases.

This was interesting. In '71 I had a (car accident,) left comminuted femur, broken into 4x2" pieces, and a compound left tib and fib. 2" femur shortening thru surgeon error. 4 months in hospital and non-weight bearing full plaster for a further 8 months. 5 ops. Then 15 years later a right femur shortening by 2" to line me up. Sadly my leg was joined up 60* out. So a de-rotational osteotomy was performed. Now, 51 years later, my left knee clonking is my only issue. Still have the 10" steel plate with 8 screws on right femur.

The emergence medical splinting for the is basically a bicycle seat on a telescoping pole. The seat sits in the patients groin while the far end straps to the foot/ankle. The the pole is elongated to, if I remember right, 5 pounds of force stretching the leg out so the ends of the break to not over lap and can return to a more natural alignment and reduce pain.
@nocalsteve

It’s called a Sager splint, and one of the main benefits of applying traction to a femur fracture is that it stretches out the thigh muscles which will cramp when the leg is shortened. Traction provides a great deal of pain relief.

Thanks for this, I just got my nail put in last month. Then a second surgery as the rotation of my leg was off and the bottom screw needed to be adjusted… so that sucked. I’m 41 year old male and all I did was slip on ice while trying to help push a car slipping on the ice on the road. So let that be a lesson to everyone. This can happen to not just Tony hawk. Too bad for me, there’s large chunk of bone that the Psoas muscle on the left side attaches to that has completely broken off. Doctors here keep saying I’m a young man and it will heal. With no additional surgery for that one piece, I’m not sure how that will happen, but hey, I’m just kinda glad I’m not the only younger guy that this happens to.

Dr. Webb, my background is in engineering and woodworking, but the woodworking side of me makes me think about how I’d repair a table leg or post… and that would be with pins (wooden or metal) and epoxy. When I watch surgery videos I often think how similar to woodworking orthopedic repairs are… except your tools and fasteners are 20 times more expensive. 😬 So my question is; is epoxy or glue ever used in orthopedic surgery? Or would that impair the healing process of bone? Thank you!

I’m curious if the medics used a Sager on him? I imagine this presented in the field as a classic mid-shaft femur which is a basic green light for a traction splint, but without knowing about the femur head fx as well, which would be a contradiction for the Sager.

Hello Dr! Great video! What happens if the femor bone is inflammed and irritated with a very slight chip but not fracture. Can it heal itself ?

I had the same midshaft transverse fracture from a MVA and had a retrograde nail (I also had bad ankle and Lisfranc so that's why they went retrograde)... anyways, my nail was locked and the fracture was slightly distracted. Didn't heal. Bone stimulators. Didn't heal. Dynamized the nail. Didn't heal. Finally after a year we did a exchange nail that went on to heal. I always am bummed when people recover so well from these fractures, unfortunately I wasn't one of them.

Speaking from experience - don't break your femur. However, if you do Mr Kunster's intramedullary rod is the road to go down.

I broke my femur about 4.5 weeks ago. I actually just slipped on ice and my femur snapped. I did find out that my Vitamin D level was very low in my blood.

What is the pain level for this? My 12 year old had Ehlers Danlos and has to have a 3rd surgery, removing her plates and screws and going in with a rod due to non union for over a year. She’s currently in her second spica cast until rod surgery in 2 weeks

I fractured my femur as a child. Barely any impact I just tripped while walking. no one could figure out why it had fractured until we found out I had a genetic condition wearing my bones and ligaments.

Would love to have your thoughts about my xrays and ct images of my fractures. Had a HET and fractured my acetabulem left, my left SI APC like, my OS Sacrum left and a shattered OS Pubis. My left femur was classified as the most severe comminuted fractures on 2 stages, proximal and distal. The lose fragment on the lateral side is 12 cm. Many lose bone fragment in the femur. The distal fracture was an open fracture with a Morell Lavalee leasion of 15 by 12 cm My Tibea plateau was a very strong comminuted fracture with again a lot of lose bone fragments. My distal Tibia was a strong comminuted fracture Right Antebrachi, Radius was comminuted and has a 12 cm plate. The right Ulna was fixed with K wire Left Antebrachi. The Radius was comminuted and the Ulna was very strong comminuted and open and non union with a gap of 14 mm. After 12 months I got a spongiosa. Also my L5 Tranverse was fractured and I had a SAB I got 6 long surgeries in 9 days. Stayed a month in the hospital and 4 months in rehab center. I recently was tested by an Orthopedic surgeon for the insurance company. He expected to see a man walking with a roller, since the healing process with these kind of injuries takes normally 3-4 years, the accident happened in March 2021. The trauma surgeons delivered a top notch job he said and you rehabbed very hard. My ACL left though is very weak which makes it hard to gain muscle and strength.

My femur break was in a few more pieces. I have a muscle disease so I’m not walking yet, if ever again. Going on 7 months sitting in a chair. I have a $10k titanium rod also. Supposedly I would’ve been up in a day if I didn’t have the muscle disease. I did it just falling on to my knees from standing. I’ve done it a dozen times without anything but a sore knee. It’s a pure headache.

I would have used an anterograde approach and fix the neck fracture with the screw system that integrates it to the nail. I feel leaving that small space of femur in between fractures is just asking for trouble

I was out at a Firebase in Afghanistan when the locals brought in a guy who had a compound fracture of his femur after getting hit by a truck on his motorcycle. SF Medic stabilized him, gave him morphine, Called for a US Medevac back to Kandahar. Saved the guys life. Even if an Afghan hospital set the leg he was likely to die from infection

Hi, I'm 50 this yr.. I fell from height at work and had facture at 6 places.. both side hip, both thigh, both knee.. 6 implants. It's going 2 yrs in 3 months, all are healing well even removed the implants in my knee.. BUT, both my thigh (femur) were not healing. Did total 5 surgeries on both my femur.. first subsequent surgery was due to implants slippage, second was bone graft to help healing (failed), third was putting the fractured bones nearer to help healing ( cause tremendous pain when walking, failed), fourth was to switch out old implants for so called bigger implants to help healing, (cause even more pain at more places, again failed).. I did physical therapy thru out the yrs but always disappointed with the x-ray results.. Does anyone knows if my femur bones will fully heal even after such long time?? Or anything at all that might help my recovery.. Thank!

Question. Is there other options other than having a nail hammered into your bone? Can you just keep it in place for set period of time n it will heal itself or does the human body move too much in their sleep to be able for that to be possible
