Hey doc - I'm reading the ADA guidelines on antibiotic stewardship in regards to dental infection and for penicillin allergic patients with a history of anaphylaxis, angioedema, or hives with the cillin antibiotics, they recommend either azithromycin or clindaymcin. Seems that the main issue with azithromycin is the possibility of resistance while with clindamycin it is c.difficile. What do you typically give for these patients?

My dentist hates me ,,how do I know we'll I had three abscesses and he insisted I didn't have an infection even though I also had a fever then he pulled them and I felt every bit of it

Today removed my lower wisdom teeth... and local anesthesia was not working. 1 bottle and above was used to nome teeth and it was vety painfull. I can't express

I just had #12 pulled and only the area around the gum and my nose was numb. When my dentist began to actually pull the tooth I began having the most excruciating pain I’ve ever had. She stopped and began giving me more injections to no avail. I told her to just pull it out because I was in so much pain. I did have a few glasses of orange juice about an hour and a half before the surgery. Would that cause the numbing agents not to work. Now I am terrified of going back to the dentist and I’m looking for answers.

Mike - sorry to hear this. Some teeth can be very difficult to get numb, however the orange juice wouldn't factor in. Rest assured this certainly isn't the norm and there are many approaches to these types of situations that can get patients through the procedure comfortably. All the best!

I have to get a filling in my front tooth, I got 4 shots of novacaine because they all didn’t work, still felt it after the 4th shot... he temporarily sealed it up and we set up another appointment. The next day my face was not back to normal and I experienced pain in my nose for over a month. This has never happened to me, I have gotten cavities filled in the past. How can I reduce the pain during my appointment if I can, and is there anything I can do on my end to help this appointment go smoother?

Had the same situation myself, except needed SIX injections before I didn’t feel anything anymore. Came to this now because the same tooth is a problem yet again… which means either six more injections and a redo of the filling, OR getting it pulled…. Neither of which are inviting.

I have this problem too. After for shots of anesthesia, I still feel everything, so my dentist just started using this pressure gun thing that puts the anesthesia directly into the tooth/jaw.

Thank you for the video! I have a few questions. Is extreme tooth decay and breakage considered a type of infection? If you see your teeth seeming to turn gray and clear, and becoming very painful, and developing holes, is there any way to combat that besides a root canal and crown or pulling the tooth snd installing an implant? Is it okay to take this homeopathic medicine I found (Hypericum perforatum 30c) advertised fo help block nerve pain? I’m asking to see if it could be thing for me to take before dental appointment where I’m expecting a lot of pain. I have severe problems getting numb and when I do it is only partial numbness and wears off within 2 hours or less., and I have TMD, sideways-displacement on both sides of my jaw. In your option is it best to go to a specialist for a root canal? I do have a TMD specialist but she doesn’t do dentistry herself. Do you think they would have more numbing options that might be more successful? Thanks you ♥️

Kiana, if possible prior to your next appointment try taking 600mg of ibuprofen 45 minutes before assuming that you are able to take this medication safely and haven't been advised not to by your doctor. Ibuprofen is an anti-inflammatory medication which will often help to facilitate the anesthetic kicking in. That's just a thought of something you could try. Often when teeth are badly broken down, they do not anesthetize well. This is why regular dental visits to manage minor decay is often more comfortable for people than only going when a tooth is aching or causing trouble with large holes or decay. All the best!

What about taking a strong pain pill before you go in? I got #2 extracted and felt like a horse kicked my face during extraction and after.

That is a good approach as well and NSAIDS for example can be effective in helping anesthetic work in situations where teeth are very sore. All the best!

was deeply traumatized, the last time i removed two wisdom teeth on two different occasions. They were the laying down deep inside type. The local anesthetic didn't work on both occasions so i ended up feeling every single probe durimg the whole removing process. At one point the tooth got stuck half removed cause the tooth was out but the nerves or some sort of "thread" was still connected to the gum.. it was the worst 30 mins of my life, i was screaming and crying. Today, i have another wisdom tooth that stopped growing cause my jaw is too narrow. Honestly thinking about getting a GA instead of local....

Same thing happened to me, but with a root canal. Anesthesia didn't work, even after 4 shots. Hurt like hell.

Same here. 2 days ago I was set to get my bottom 2nd molar removed. The local anesthetic failed, he kept injecting more anesthesia until I finally screamed from the pain. He asked me to come back later for a GA. The inside of my cheek is bruised up. I’m so traumatized by this event.

We were taught not for IAN blocks, due to the risk of neuropathy/parasthesia. For uppers it’s fine.

if we have infection or inflammation in area that we will be injection with anesthesia in this case the anesthesia can not worked if we alter the site of injection technique is the procedure will done and is the inflam area will anesthesia..

Often times for a lower tooth that has a lot of infection around it, an IAN block supplemented with PDL injections at the line angles of the tooth will more than suffice to get the tooth comfortable enough to remove. I would use the same approach on the upper (do a block, supplemented by PDL injections). All the best!

Dr. i was supposed to have a root canal awhile ago but we didnt continue becuz the anesthetics wont work on my tooth. How do i let this anesthics work? Does antibiotic help ? The doctor just put something on my cavity before putting a temporary filling and he said that it will kill the nerves. Will the anesthetics work the next time ? Im so scared that i might experience the kind of pain i had awhile ago upon the start of the rct.

That is certainly one approach. Sometimes formocresol is used in a tooth like that which may be what was done. I can't say I use this approach myself. The tooth was likely very inflamed which makes it difficult for the anesthetic to work. Typically, just opening it is enough to relieve lots of the discomfort. If you are able to take a NSAID drug like ibuprofen you could take 600mg to 800mg 1 hour before your next appointment (assuming you have no contraindications for doing so). This will help the onset of the anesthetic. I wish you the best!

@@rbkylebacabis6684 That's not a drug I'm real familiar with, but it's similar to ibuprofen in terms of the class of drug (NSAID). It should have some positive effect. All the best!

They say I don't have infections yet I I can never get numbed I just have to go along with the agonising pain for every extraction 😓

Maxine - this isn’t normal and there are options for you to have teeth removed comfortably. Nitrous oxide or IV sedation would be an option and frankly, it’s rare that local anesthetic doesn’t work for people so you should be numb. All the best!

When would it be best to put the patient on antibiotics and call them back for an extraction, or do you prefer getting the route of the problem head on?

The standard of care is to remove the tooth that day to address the infection. There are many ways to combat / overcome infection to achieve great anesthesia which I discuss in other videos including, but not limited to: 1. Pre-op NSAIDS 2. Nitrous 3. Buffered Local 4. Block anesthesia 5. Supplemental injections (PDL, IO, etc.) If you send a patient home with antibiotics that the infection does not respond to...you have just made indirectly their condition worse and in some cases, this could be a big problem. The video explains this well, but I will try to sum it up. Sorry first of all to hear about your experience. Second, if a tooth requires a root canal the nerve is often either dead, or very inflamed and unhealthy. When there is much inflammation, the anesthetic does not always convert and cross into the nerve to block the pain signals that your tooth is sending. Anesthetic gets activated by the body (it doesn't come in the active form when it's inside the glass carpules we use while injecting) so if the environment isn't right to buffer and convert it, it won't exert its full effect. I am lucky I got good gums and teeth not lost one tooth I know ppl who hasn't got any or younger ppl losing there tooth and have drilled in there gums. Stop surgery fatty stuff that will effect your gums and teeth I do know about teeth I have a dentist I do see
