I don't care what terminology you use. My butt still hurts and the pain makes its way to my ankles. I do all the exercises and work out 3-4 times weekly. It hurts when I continually walk around a table or stand (part of my work). I have been to PT. The irritation has been going on for 4+ years. Doctors get hung up on terms. What I seek is results.

I understand that you are suffering and that it's really frustrating. I'm in the same boat. However terminology matters because diagnosis matters ...and if you don't get the right diagnosis then you can't get the right treatment. And you can't get the right diagnosis if your doctor or physical therapist doesn't understand the difference between one condition versus another; and they have to know and use the correct and precise terminology to do that.

I started pain in my butt very sharp .2ys on vibrating down mostly left side pins and needles . This came on after the AV covid never been well since . I know my body well as i am a ballet dancer . I do most of these exercises . Some days better than others ....??? I wish my life could get back to normal doctors usless. Love your platform it really helps .thankyou.

Mine came from smashing my ass on a ramp while skating. My life is ruined. It's been 3 years and my toes are numb and I can barely walk. Makes me just want to die.

Sorry for you 2. I feel your pain. This kind of pain is crazy intense and people who have never dealt with it will never understand. Please though don't wish for death and I hope you are joking about that. Maybe check for herniated discs as well. I ruptured 3 from SI up. It's been 20yrs now and I still get crazy tightness at times that cause this insane pain. Myofascial release helps me more than anything and you can do at home. Some good videos on here about what to do. Best wishes to you 2

You forgot that tight muscles or lack of mobility in the myofascial parts are able to reduce the blood supply of the nerves (vasa privata). Not a Kompression of the nerves themselves but of the blood vessels that are doing the 'catering service' to the glia cells, so that the nutrition of the nerves is reduced. In this case you also can have tingling or pain at the backside of the leg. Pain at the side of the leg is rather a myofascial problem.

Update. After years of pain, they did a CT guided injection into the SI joint. The anaesthesia killled the pain completely for up to 3 hours post injection. The steroid has helped somewhat but it was mainly a diagnostic tool. It all indicates to the fact that we’re barking up the right tree. I’m trying not to get my hopes up but this is excellent progress after years of ambiguity and negative testing. I believe I have chronic inflammatory sacroiliitis. I’ve done my research and it’s common not to show up in CT and MRI scans, of which I’ve had several. I’m preying that they will burn the nerve off so I can get my life back. I’m fully functional, full strength and range of motion, it’s just the pain and the consequences of movement that is so horrendous and debilitating. If the would go away, I’d be fine. Sometimes things are hard to pin down but it doesn’t mean there isn’t something legitimately wrong . FIGHT for your treatment. Fight for yourself. I wish the very best for anyone reading this who is struggling with this stuff.

I am 19 and run track and thought I had hamstring tendinopthy, 5 months after injury I get two MRIs and they found nothing I still have pain in the upper buttocks hamstring area and it travels from private area to mid hamstring. MRIs found no sciatic problem

I believe i have piriformis syndrome. I tried a deep release with a soft ball and then tried to do some exercises afterwards and i couldn't do the exercises because of the pain.

Apart from pain in the glutes there are other neurological symptoms. While sitting pain hugely grows and gas incontinence due to sudden loss of muscles control appears (it is not mentioned by anyone but it is worse than any pain).

It sounds like you have nerve damage. Ideal would be an MRI read by a competent person. A good physiotherapist could perform tests to confirm which nerve has been damaged, and perhaps offer manipulation treatment to relieve it. Deep tissue or Thai massage might help. If you could do plenty of modified lunges and squats, that might help.

I give up. 25yrs of pain in the butt. Spent perhaps $2.5m and still no relief. Just now i am trying to live with it but this is very difficult.

I ruptured L5 and L6, I have shooting pain in my butt and down my left leg to my ankle. My Neurologist tells me its not Piriformis but I have 4 different Physicals Therapist who says it is.

Wet and cold weather really works mine up! I get terrible thoracic and butt pain on left of body, neck pain, and lower back, often upper back.

I agree and studied this deeply, although the piriformid may be tight and those stretch exercises help. I believe the glut medius was the most affected area and some stenosis.

I had that deep pain in the butt and other horrible, painful symptoms. So, I could no longer run for exercise. Walking was painful. I put on weight. I was in denial. The belt was long before on its last belt hole. I had trouble admitting to myself that I needed to start buying "extra-large" underwear. After I realized that the light clothes was causing me horrible extra pain in the hip and down the leg, and I started wearing larger size clothes, and now that deep pain in the butt has become quite rare.

@@mimi-dq1tx Loose fitting clothes...the larger point is that the pain in the butt is apparently a tendon strain, because is starts and stops hurting quickly. It doesn't have the weird symptoms of a nerve problem. It doesn't get better with I take a nutritional supplement called "Nerve Regen Formula." And, it doesn't hurt like a muscle strain, where it gets more acute for about 40+ hours and then gradually becomes less severe for about ten days. I can deliberately cause that pain by straining against the blockage of the internal rotation of my left leg. I can deliberately not cause that pain. See second reply.

Any part of range of motion that is not used for weeks or months accumulates a bad molecule that causes pain and stiffness. Apparently, too much of that lack of motion cause a part of the range of motion to become 'frozen' due to points of adhesion. My remedy plan is to do seven or more repetitions of moving through that range of motion. At the apparent end of the range of motion, pause, and move a little farther. Don't strain. Do those repetitions three or more times a day every day.

Orthopedic surgeons are looking for an obvious mechanical defect that has a proven surgical solution. Preferably a diagnosis that is written by a radiologist in an MRI report. Surgeons are looking to do surgery that they have practised and are good at or want to do research on. Surgeons aren't really interested in solving your pain in general, they are interested in if your pain is related to a surgery they are proficient in providing a good outcome to, or a research project they are doing. Took me a while to figure that one out, but helped me be less offended by what seemed like lack of care. Once I figured it out it's radiologists that diagnose, I got lots of imaging with detailed reports. Then sought out surgeons that are good at doing surgery on that diagnosis. Get yourself good 3D MRIs that show the nerves, vascular, soft tissue and bone. There are better options than surgery if it's small and something your body can heal on its own with a little help. If you have already done physio, look into biologics.

It has been with me since the day of my accident I damaged something deep inside the glute/piriformis area scar tissue is so deep that no treatments have helped there is only one solution that is piriformis release surgery I have been messed around and told lies for so long now they cant back track.

What can be the reason for repeated spasms in that area? I get 10+ a day and it is not related to posture, it just comes out of the blue, even when I've not moved. I'm really fed up.

I think this explains what my pain is. I know I need to exercise, but... old/lazy/weak/tired blah blah blah. Thank you for this. It explains a lot and I will try to spend more time walking. I've gone from thinking it was sciatica to piriformis syndrome and this sounds like the end of the line, the actual reason for my pain. I've been in tears a lot over the past months the pain is so intense/radiating to and fromthe top/bottom of my butt but never down the leg. Actually the top usually has be say it's the hip, and further below I was guessing the end of the piriformis. Hard to move in bed, that's when it hurts the most. Right side only. Only started a year ago which is why it's probably related to my now mostly sedentary lifestyle (loss of loved one).Taking some supplements and now sit with a cushion under the outside edge of that side of the butt (right) when sitting watching TV and while driving. Q: Could me getting a hernia in the right front side be cause for back pain? I haven't had hernia diagnosed as yet, but if I hold that area in when walking around, it seems to relieve butt pain for a while. I've had the pain yes in the butt cheek and underneath and down the leg AFTER a lot if research on my end I figured out its inflammation. So I researched what to do for inflammation I watch my sugar intake & sodium I drink a shot of fresh tumeric & ginger every day and eat a lot of pineapple It subsided a lot and for some reason it's back I know I need to cut back more on the sweets

Actually numbers 1 and 2 are arguments that it IS piriformis syndrome. It only becomes sciatic nerve pain when the legs are affected. So, if your legs aren't affected, it can't possibly be sciatica. If the pain never leaves the buttock, a good way to test for piriformis to do the piriformis muscle stretches. It will be tender and stiff. I have arthritis in my low lumbar spine and I find that this is probably why I suffer with piriformis syndrome. Exercise does resolve it and if I did those exercises more frequently, I would probably stay under control. I also happen to have arthritis in that same hip. It's hard for me to know if the arthritis caused the piriformis syndrome or the piriformis syndrome caused the arthritis, or maybe they both ran tandem from the very start. I think the title of this video is misleading because a pain in the butt truly can be piriformis syndrome and also is not likely to be sciatica. In fact, I was told at one time that if the pain does not go past the knee, it may not be sciatica either. Of note, my job required prolonged sitting for roughly 45 years. I'm absolutely convinced that has a lot to do with why I have arthritis and piriformis syndrome.

I sit with a ball under my butt with a heating pad, usually I had a shot on my butt on the right side It helped, but now the pain is excruciating on the left. Tried tens unit and it didn't really work

Started down my left leg with sciatica pain. Over time became worse and developed weakness in both legs, deep butt.pain , lower back pain and pain down both legs. Now in constant pain, and live on pain relief medication

If they are prescription pain meds, you’re doing your body a world of hurt and it will be very difficult to come off of opioids. Your calf needs stretching, the back of your knee, your quads, and hamstrings all need to be stretched in addition to the piriformis muscle. Try doing them several times a day.

guyz, I have officially cured my Sciatic nerve pain FOR GOOD, it was totally an accident, not intentional, i had this pain for about 4 weeks, I watched a lot of vids, and i did exercises, it helped for a while, but it keeps coming back a few hours later, and nothing seems to cure it! At the same time, i was going to the dentist for some teeth work, after fixing like 4 teeth, i had pain all day, so the dentist told me to take some antibiotic shot in the butt to ease the pain, anyways, i went to the pharmacy, and the dude there gave me the shot on the same side of the nerve pain, i immediately felt the shot like a small tough ball that is stuck in my butt, it lasted for 15 minutes, then it went away, immediately after that, the nerve pain disappeared, i could bend, stand, do whatever, there was no pain at all! yes ! just like magic, i don't know if these 2 are related, but definitely right after the shot i was cured, and now, a week later, i haven't felt a thing! maybe some real doctor can explain this, hopefully there is a scientific explanation to what happened.

Maybe what happened to u was similiar thing that people have after acupuncture. Needles can unlock nerves, blood vessels etc. Still u got lucky that person giving u shot sniped right spot😅

I have a spinal cord inury at L2/L3. Trust me for 16 years ive dealt with Nerve compression as im addicted to working out. I have tried to strengthen certian muscles however nerve damage is just there and Ive had to accept this. Sitting is the worst thing I can do so I tend to walk more. Salt baths greatly help and I mean I OD on salts and use more than recommended. 😅. Thanks for this video.

I am a 65 year old female and was first diagnosed with myofascial syndrome until the pain had spread on both sides equally, back of neck downward to scapula to under and sides of my arms and my buttocks where you show this In your video. Now I have been diagnosed with Fibromyalgia since 2020. I am so afraid to stretch because it can make my symptoms worse the next day. I am wondering if PT would help me to ease in to stretching exercises to show me how to do it correctly? Thank you sir for your wonderful video!

Yes, but I had spinal stenosis. As a result I had to have spinal fusion. Vertebrae worn down, pinching nerves. Now nerve pain gone, but have extreme tight hip flexor and piriformis.

My pain is mostly in 1 and 8 but when I stand up after sitting it also hurts in 2, 3, 4, and 7 😩 Laying on my back or stomach is literally the ONLY relief. Going to try these workouts to see if any of it helps. Thanks for sharing 😊

Depending on the small amount of information I have I would advice you to let someone do the straight leg raise test on you and see if that is painfull as well. There might be more going on because of the 'laying on my back or stomach is the only relief'. If positive, seeing a doctor might be good if negative try these exercises.

I've been to a chiropractor first for some treatments. They have a stretching table. The pain moved from my back and left leg to the right buttocks and down the leg. Went to 1 PT guy but he didn't have an opening for 2 weeks. I went to a different PT. Both use stretching and massage. I can drive with little or no pain. The pain hit when I get out of the car.

I don't understand how training and thus extra contractions help muscles with knots in it. Shouldn't solution of the painful muscle knots and irritation of the nerve be the first thing before one starts the exercises? You state first lower everything that triggers the pain. I have done that. Then I started exercising but had to lower the intensity and later on stop altogether because it aggrevated the pain. It is a merry go round because when I start the exercises I have more pain the following day. What to do to get rid of the pain?

I have a terrible back, 5 disks in L2 thru S1. 4 laminectomies, laminotomies. I fell. Since then, 2 months ago, the pain is in my right iliac crest. (I'm an RN). Nobody knows or wants to know what it is. all say it's my back because my back is bad. I know my back pain, all on the left, this is on the right. NOT referred, not sciatic, Very localized deep in my right iliac crest. I fell on tile flat on my butt from sitting height. I'm being offered spinal cord stimulators and injections in my back, my SI joint but not one dr, of the two i've seen, have even touched my body. Have even looked at where I say I hurt. They look at my MRI and said "back." It's a new injury. Why can't I get help? Dont' inject me until what you can tell me where it is and what it is. I have CRPS of both legs so it's complicated but I can not just let drs be injecting my back where ever they want as they guess. The hip dr refused my referral because on MRI he is sure it's my back. I could have a fracture or my PT thinks I torn my gluteal muscle. Two months and last week I decided I needed to just stop life and go to bed. it's getting so much worse I can't bear weight on my leg anymore. I've had to stop PT and chiro as it's getting worse but feel so disrespected by drs. yes, lots of back pain - this is different and new! Thanks for this video. It's the first video anyone has pointed to the actual iliac crest. I know sciatica, this is not it. It's a lump in my butt like right where you would give an injection in my butt. Upper right quadrant. What can be done for a torn gluteal muscle. How can it be diagnosed. Do you have a video or can you make one????? Or respond. I fell. and I'm about to end my life because it feels over as I can't walk. And I walk with CRPS from the hips down, you know I have a high pain tolerance. I'm about to sui. Dr.s dont get it. I feel so disrespected. any thoughts. My life is over right now. and nobody cares but me.

I've been put on prednisone 20mg/day just for my PCP to try to do SOMETHING so I can at least get a meal. I live alone. I have responsibilites even just to my cat! ugh. I'm a graphic designer who can't stand, walk or sit. help? just a guess based on location would be more than I'm getting. what does a torn gluteal muscle feel like and is there a test for it. If it's torn, I'm afraid over the 2 months, its gotten more torn. So, i'm in bed trying to diagnose myself and not get depressed.

my above-buttocks pain goes into lateral thighs and calves. Clam shells exercise with some resistance aggravates symptoms. MRI showed lumbar anatomical stenosis but with no nerve compression. Can I assume it is piriformis or gluteal muscles causing the problem then?

Now I have shooting pain in my right butt and down my right leg after a fall. This might be due to Bursitis in my hip due to favoring my left leg that was damaged in the fall. An MGI tells me I did damage to my L3 and L4 this time.

Its called nerve tension. Its a burning pain you get when you put tension throughout the nerve the more tension the worst it gets....ts caused by scaring on the siatic nerve from disc bludges. it will only go away once you have the surgery to remove the disc thats hitting the siatic nerve Once its removed the nerve can start the process to heal which is years depending on how much and how bad the scar tissue on the nerve is... it goes away n cones back but will eventually stay. The longer you put off surgery the worst your final recovery will be in the long run.. Been there done that...

I had my 1st literal pain in the arse from a recent job due to heavy lifting.. made me very immobile and took about 3 weeks to go, its still niggling but i can walk much better.. worst injury ive had.

Olbas Oil helps me! (Topical & be careful!) Also could have alignment issues in the sacrum. But working on these exercises strengthens weak muscles and really can make all the difference by changing the dynamics of the area.

I had it for 1.5 years and literally limped when I walked it hurt so much. I tried needing, acupuncture and chiropractic care. I went to the chiropractor about a year and a half into the situation because I had what seemed to be a pinched nerve in my neck. He put me in traction for my neck… The third time I was in traction I noticed that the pain in my buttock left. Traction fixed my issue after 1.5 years of horrible pain. I purchased an inversion table after that and use it from time to time as well. Traction was the KEY though. Something had compressed the nerve and when spinal decompression was used… it disappeared.

Mine happened a few hours after a Chiro adjustment, that included the hips, after a few hours of the adjustment felt the buttock pain and same pain radiating down down the back of both legs, wasn't sure is vagus nerve was involved. Not horribly painful but very noticeable. not sure if it will subside or if I need to go back to chiro. Never had sciatica issue before just lower back, and upper muscles in back from lifting or moving incorrectly.

I had something similar after a chiropractor adjustment. I went back, told them what I was experiencing and that it came after the adjustment and thankfully they did another adjustment and that took care of the pain.

My pain starts at a point in the buttocks where the sciatic nerve would be under the piriformis and then shoots down the leg all the way down to the lower calf. So whether I have actual piriformis syndrome or not I do not know. It's not just the right buttock for me. It's down the leg for sure and it's reacting more when I have to bend etc or after sitting.

This could also be a case of Sciatica from the spine due to a disc bulge. The lower back and buttock are in a way aligned with each other when doing certain movements. Have it checked out and see what a physiotherapist has to say.

My pelvis is crooked on the left, and I also have a left leg that is significantly too short. I've been walking around with it for years and now more and more problems arise, my pelvis is becoming more and more crooked, my leg is somehow becoming shorter and shorter, very hard tension in my entire thigh and the whole Leg and foot. and super hard tension in the buttocks with pain. My entire left leg is externally rotated and also my foot. The ankle is also extremely tense and tight. Are there any good ways to get this under control again with targeted exercises? so that my pelvis is completely straight again and my leg is no longer too short? and the tension disappears?

I've had 3 back surgeries in past 2 and a half years, lamenectomy, then fusion of L4 and L5 8 weeks ago S1, still in terrible pain I don't think this guy is for me , also been to PT 4 times after each session couldn't get out of bed next 2 day's, I've done all of this and Narcotics nothing work's I have piriformis syndrome

The usual tests for piriformis syndrome can give a false negative. The reason is that flexing the hip (leg forward) causes the apex of greater trochater to rotate posteriorly. The apex of the greater trochanter is the location of the insertion of the piriformis muscle. Thus, flexing the hip relaxes the muscle which prevents the pain associated with tensioning it by internaly rotating the femur. A better test is extend the leg. This extension rotates the apex of the greater trochanter anteriorly, putting slight tension on the piriformis muscle. This alone may cause pain, but if not internal ration of the fenur, with the hip in extension, will.

@@andreadalex8893 "which is the answer" - what is the question? "solution" solution to what, the diagnosis or the cause or the treatment? (I prefer full sentances so I don't have to write an answer to every possible interpretation of a question.) "what exercises can cause it" or perhaps you mean what causes it, not necessarily an exercise. Piriformis syndrome is caused by many things, from malformed piriformis to a mal positionied sciatic nerve, or weakness of any of the other hip muscles which cause s the pirifomis to be overworked and strained. "Which can solve it" it really depends on the cause from strengtheing the gluteus medius, to stretching (gently) the piriformis, strengthening it or even resting it - so the cause of piriformis realy dictates the treatment. The first thing to do is image the lumbar spine to ensure that other (and more common) causes of sciatica and or buttock pain are not present.

@Worldsbiggestloner A short post in a comment secion is not a replacement for a detailed history of symptoms, and physical exam. If you are over 55 then wear and tear arthritis could be the cause but it could be a disc Remember that when have an MRI it is laying down, but when you stand things get compressed downward, so the appearance on an MRi may be less than what you experience standing up. There's nothing wrong with getting a second opinion from another doc.

I experienced the buttock pain for about 3 days and then it progressed into very intense sciatic pain that prevented me from even being able to sleep for about 4 days. With stretching and a foam roller, the sciatic pain gradually subsided, but here I am now, about 6 weeks from the initial injury and the deep gluteal pain is still here. Although I have progressed to dynamic stretching and band exercises, my flexibility is still limited and my balance is very off. I cannot run, jump, or do anything explosive. Is this piriformis syndrome?

You say "first rule out Radicular syndrome"; but... patients look to therapy to keep them from having surgery. I go to a spine doctor and he says I need surgery; it is what he does. But as a patient I want to do the opposite of your instructions. I want to rule out all other reasons for my burning butt and leg first before going to sleep under surgery and that long recovery period.
