This video provides you with a step-by-step process for calculating parenteral nutrition.
You will learn how to the estimate nutritional needs for a patient, determine the grams and volume for each macronutrient, and how to write a prescription.
Remember, parenteral nutrition is intravenous nutrition!
There are several types of parenteral nutrition like PPN, TPN, 2-in-1 bags, 3-in-1 bags, and pre-mixed formulations.
This video just gives you the basics of figuring out the composition using macronutrients.
In future videos, I will talk about the different types, how this influences the composition, and how to determine the micronutrient composition. So subscribe and stay tuned!

When discussing grams of carbohydrate per day, I say “1,400 minutes” on the audio, but the screen says “1,440”

Thank you so much! To answer your question: Sometimes there’s a method, but not always. The one instance that comes to mind where selection is meaningful is when the patient requires a volume restriction (i.e ESRD or CHF). In this case, you’d want to choose the higher percentage options since they’re more concentrated. I could also see this being the case if the patient has very high calorie/protein needs, but I’ve never run into that issue before. If the patient has no fluid restriction and has relatively normal energy/protein needs, any combination of the percentages will work just fine. Most facilities will only carry 1 or 2 of each, so clinicians usually don’t have many to choose from.

thank you for this helpful and easy video illustration however, what if the bag used 3 in 1 .. how to start the feeding and increase gradually if all the solution is all in one bag ?

Hello! Happy to hear you liked the video. This example is for a 3-in-1 bag. In most instances, there is no reason to start slow and increase gradually with a continuous, 24 hour infusion. You can just begin and end at the goal infusion rate.

Hi Jisselle! Thank you for watching my video. For the 474 mL of free water added to the bag, I was taking the estimated fluid need (2,308 mL) and subtracting the amount of fluid provided by the ingredients (1,906 mL). 2,308 - 1,906 = 474. That 474 mL is just extra water I’d want added to the bag to help meet the patient’s baseline water requirement. It may not be the perfect amount for them, but it’s usually a decent place to start. What if the prescription was 1,200 kcal but the available is 1,900 kcal to be transfused for 36 hrs. Hmay I know how we can only gice 1200 kcal

Can you explain the rationale of using mL/kcal (25-30 mL/kg) for fluid calculations in EN prescription but higher fluid of 35 mL/kg in PN calculation?

Great question 👍🏻 The decision wasn’t made because it was EN or PN. I went on the higher end for the PN patient because he recently vomited (fluid loss) and was with inadequate intake for a prolonged period (~1 week). You could do 1 mL/kcal for PN, or you could go higher or lower with EN - It all depends on the case you’re presented with. Admittedly, these cases don’t provide very good explanations for choosing energy/protein/fluid needs. I will have to make specific videos for that in the future!
