Sometimes a cbc can't return fast enough as we don't have a lab on site or someone to read it on site. This is most often seen in rural communities

Ikr. I never expected this level of in depth teaching. I think in my country, people will get bothered if doctor visit took long, and wouldn't even want to hear what the doctor would be teaching. So, it is nice to see that maybe it could get better. This video gives me hope.

@@jroark101 So let me get this straight, you graduated as a nurse practitioner and you need a KZfaq video to go over all the differential diagnoses for a patient that presents with SIRS criteria (tachy, febrile, and hypotensive) who has an elevated white cell count, no rebound or guarding, and a negative UA (besides the trace protein)? If that’s the case, I can see why my colleagues continually say they believe we are currently watching the downfall of medicine.

Awe, my hear went out to this poor little girl for being so sick. Her parents were smart to bring her in right away to see you. Glad you did the CBC. I didn't know you could read the test right away in your office. Amazing technology today. I understand protecting the rights of the parents decision on options, but I just want to know if she is OK now. Thank you for helping this family with their very sick child.

My sons doc was amazing. He used to take his bunny toy with him. She would first check out bunny too show him it wouldn't hurt and of course make sure bunny was OK. She always spoke to him as her patient no matter that he was only 2.

My mom was a nurse. In the lab she did blood counts under a microscope. Near the end of her nursing career she learned to use a new machine that did the blood counts. The simplicity of use of that modern machine reminded me of my mother's work and how much medicine has changed since the early '70's.

That machine was probably called a hemocytometer. I never used one, but did use current machines.

*Dr - as a medical student I want to point out that observing for Kernig and Brudzinski signs (stiff neck meningitis signs) is being phased out of medical practice as these are considered signs which appear too late in the illness.* I appreciate I'm coming from a UK perspective but I urge you to take this information on board and look into it for the safety of your patients.

This comment will probably be deleted but he's the reason a child was horribly sick with tetanus and almost died because he recommended against a tetanus shot. First case of tetanus in Oregon in decades. His medical license was suspended because of this. He does not care about modern medical practice.

I hope the poor little girl feels better soon. I wish GP surgeries in the U.K. could do blood tests in house with same day results. However, it is probably expensive and financially I am completely reliant on our good old NHS.

This is not entirely true, the NHS can do same day return if specified as urgent by the GP to a local hospital. I was sent to A&E for this exact reason because they could turn around the blood tests within a couple hours. Ultimately, the GP will have to decide whether or not the test is truly urgent in terms of stressing the system versus letting the patient be uncomfortable but safe. The NHS is blamed for a lot of things, but urgent triage is generally one of its good points (if we ignore the always free at point of care treatment).

When necessary they will do a same day CBC, it goes by need. Like in this case. If you were to show up at urgent care with these same symptoms, adult version of severity, they would absolutely do a same day CBC.

I have had the weirdest medical stuff. This year so far I had the ambulance take me in and I sat for 17 hours and started a melt down. My sweetie wasn't with me . I then waited 2 more hours of CT and they found a gulfball sized deep abcess 8n my throat. They drained it and it was traumatizing after 21 hours there I finally went home.. The last time was an infection in my Bowel in the large intestine. These things happen so fast. Those are amazing parents.

Great doctor. These patients are very fortunate. Like with everything else, not all doctors are good! If your doctor does not talk to you like this, explaining things and teaching you, find a new doctor!

My if my doctor made me look to my chest the morning I came in very sick, I would have been sent to the hospital. Instead he sent me home on antibiotics for a sinus infection. My mama rushed me to the hospital later that day because I became very delirious and spiked a dangerously high fever. Had she not acted the second she did, I would have been six feet under. I had a very severe case of bacterial meningitis. That’s a week of my life that is totally blank to me. When I came to, I was very confused but I was coherent and made a full recovery. To this day, I still get a panic attack when I have a sinus infection.

My kiddo had similar symptoms and is ASD. Ended up in the ICU with DKA and a diagnosis of T1D 🥺 Very scary. Hope this kiddo feels better soon.

When I saw the title and thumbnail I immediately thought T1d I was diagnosed age 11 but it unfortunately want picked up at the first dr visit so I was a lot sicker by the time they realised. Definitely showed similar signs although for me they were more gradual and drinking and peeing all the time were the main ones!

When my daughter was 3 years old she had a temperature of 103.9 vomiting and couldn't keep anything down. So, I made a cup I'd green tea and put a little bit of honey in it. Told her to sip it slowly. She was so dehydrated before the tea. And, the tea was the only thing she didn't vomit up. Once she began to tinkle in the potty, I knew she would be okay. Temperature went back to normal and the vomiting stopped.

Yu can buy organic dried ginger and steep it in the tea before serving, great for settling stomach and has other health benefits as well. I use about a half teaspoon, For a small child I would use 3 little pieces. Dried ginger is diced and dried.

I wish you could teach my doctors office how to get CBC results in that amount of time. It always takes the outside lab 1-2 weeks, sometimes months before I get them

I’m curious what options were presented to the parents based on those results? Ex. ER for further evaluation to find bacterial source of infection? I understand letting family have privacy n their decision but what was presented to them as options?

Middle abdomen pain around belly button is something to watch. If it's appendix it can switch to low right side then get worse from there.

lol i had a pediatrician who thought I had after symptoms of an flu infection and kept on telling me cuz my blood scores weren't that high 😂 my 10kg weight loss, the pain and bruises were normal to her😂 turns out I have a light to mild crohns disease 😂, luckily have another one now who really listened and ordered an colonoscopy and gastroscopy to make this diagnosis and he immediately did a liver biopsy too to see if it was connected cuz my enzymes were high too but "luckily" it was just medication and i stopped with it and these enzymes are almost normal again

Sometimes a cats heart murmur is amplified or only noticeable when they’re dehydrated. Is it the same for children?

Doctor help us out, my 1yr10m old toddler, had convulsion due to high fever twise first was on 15 march 22 and second 21 August 22, how can we prevent this situation, his Dr prescribed him a carbamazepine 100g wich im not comfortable with the medication..

I've had Zofran when I was sick back in June. Couldnt keep fluid or food down. Kept vomiting with little food in my system making it hard and even impossible to eat. It does work effectively however I think it may have been a nasty bug and it was bad on my end. Zofran was helpful

It was invented for cancer patients, because previous drugs sometimes didn't work that well to prevent that most feared of chemo side effects. It's also used a lot for surgical patients, and is very safe to use during pregnancy. It also does not cause drowsiness the way some of the older drugs do.

@@GoldRangerFanGirl35 it relaxes me and makes me sleepy also, but not to the point I would pass out completely. It works amazingly for anxiety also, not just preventing vomiting. It always helps with my pms nausea.

wbc count can be eleveted in children with vomiting episodes due to cortisol secretion. it does not necessarily mean bacterial infection. Crp can help also

Why dont ALL medical dr's or facilities run these quick CBC's? It could save parents from having to bring children back and save children from having to stay sick and possibly get even sicker. You are a fabulous Dr!
