This particular video is intended as a demonstration of a physical exam that may be useful in evaluating a patient with low back pain.
It is not intended as a complete instructional video and should not be considered a source of complete physical examination instruction. It is also intended not as a perfect example of a physical exam that would be performed for a patient in clinical practice, but is designed to optimize function and efficiency for a OSCE testing setting.
Instead, it should be treated as a supplement to independent learning using primary Osteopathic Physical Examination instructional resources. Clinical skills are best learned and developed with support from faculty in the context of a complete Osteopathic Medical School Curriculum.
Osteopathic Clinical Skills is a channel dedicated to discussing and exploring Osteopathic Clinical Skills concepts for medical students, residents, and clinicians and presenting them in an easy to understand manner.

Erythema friction rub: Redness/pallor indicates vasomotor changes that may be secondary dysfunction.

Hi! Skin drag (the test with the light touch dragging on the skin) is one of the steps involved in tissue texture assessment as part of an Osteopathic Structural exam. It is intended to assess texture of the skin as well as changes in skin friction created by varying moisture levels, small levels of tissue edema, etc. These small changes in texture and moisture in specific skin areas can be a sign of underlying somatic dysfunction (which can cause a local imbalance of sympathetic autonomic tone affecting the microcirculation to the overlying skin). Essentially it’s one more clue in addition to the rest of the temperature, light palpation, deep palpation that points towards underlying somatic dysfunction (which can be treated with osteopathic manipulation when appropriate). The Erythema Friction rub (the other test with more pressure and rubbing along the skin) provokes a red reflex response in the skin and gives you additional information about how the circulation at various levels is functioning, which is another clue pointing you towards the presence of somatic dysfunction.
