Trigger Point Release for Piriformis Muscle Not Working?

If you're dealing with back, butt, or sciatica pain- chances are you feel all sorts of tender spots. Much of the world believes these to be "muscle knots" or "trigger points" and because of this, many people try releasing them by getting deep tissue massage, foam rolling, using tennis balls and all sorts of other gadgets. But, if you're like many of the people I work with, the trigger points and tightness just keeps coming back. Why?
Because evidence suggests that trigger points are NOT of the muscle- but rather due to a sensitive nervous system or even referred pain.
Sensitivity of the peripheral nervous system (as a result of inflammation) or central nervous system (central sensitization) can cause one to feel "trigger points" and tender points in the area of the body supplied by that level of the spine.
L5 results in glute, hamstring and lower leg tightness.
S1 issues result in calf "trigger points" and "plantar fasciitis."
L4 issues refer tightness and "trigger points" to the psoas and hip region.
This results in much confusion- along with various pain points and diagnoses, as doctors and people chase the symptoms, rather than treating the cause (a sensitive nervous system or structure referring pain).

Brilliant, thank you Charlie for explaining this so beautiful. I wish all PT's would learn about the body mind connection and nervous system dysregulation - stuck in fight and flight (my story anyway).

I have heard this called "Tension Myositis Syndrome" or "Mind body Syndrome" or Perceived Danger Pain. The brain is misinterpreting the signals in the body as dangerous and creates neural pathways of pain without any damage in the body causing the pain. It is all based on fear.

Hey Laura- if the central nervous system is driving symptoms- then yes, there are many terms for this… central sensitization, TMS, MBS, PDP and so on. If the PNS is driving symptoms this is proposed to be due to neurogenic inflammation. Also, one can experience somatic referred pain away from the origin of pain (didn’t discuss this much) 👍

SO, a heightened Sympathetic Nervous System can get TRIGGERED, STUCK or BECOME DOMINANT and create these multitude of aches and pains? PTSD, Anxiety, Victims/Survivors of Trauma and or Violence, etc. being the cause and baseline factor that takes over the CNS and keeps us STUCK in our pain???

Hey Dr Charlie, does what you describe here, could that include SEVERE MUSCLE SPASMS, that feel like the muscle is being torn from the bone? Muscles with trigger points that beg to be worked on with pressure or Dry Needling and the only way to loosen the muscle?

What do you do if you have these issues but your back is fused from scoliosis surgery. From T2 to l1 with hardware?? My orthopedic surgeon is of little help besides just telling me I have a deformity. Any suggestions?

Hey Leah! good question... we work with many people who have spinal fusions as a result of different issues (including scoliosis). so many things we can teach you... from better understanding the symptoms you're experience and pain you feel... to how to optimize the healing environment physically in order to move painfree... to how to systematically and scientifically identify motion that is safe and works (reduces symptoms). Basically, we would teach you a process so that you can be in control and learn what you need to do to heal yourself. Looks like you set up a time to chat with us too! thanks for watching my content. Please be sure to submit your case review so that I can personally review it and we can develop a plan. Thanks Leah! Dr. Charlie

And what about the theory of Oxalate crystal accumulation in tissues??? Thanks!
@SpiritHawk
@SpiritHawk 3 ай бұрын
So many people on the carnivore diet are experiencing healing from joint and muscle pain that I can't discount oxalates as a cause. Also, it's well known in the medical literature as a cause of poisoning the system and ending up as things like gout, kidney stones, skin issues, eye problems, tinnitus, etc -the list of symptoms and damage from oxalates is long.
