You guys are missing one mechanic of the game. If the patient cannot or does not follow the rule of their symptom for any reason, another player can call out 'Psychiatrist!' at the player who messed up (or fibbed on purpose) and then they have to switch seats. This makes it so that the Psychiatrist clearly knows when something has gone awry, that can help keep hints less confusing.
