in loss of eyes question (at 38:16) probability that a person will loose eyes is P = 10/100000 and N = 4000 therefore, mean = (10/100000)*4000 = 0.4

We use that formula if Lembda is not given in the question. As here it is given in the question but, indirectly. That is why the value of lembda is considered 0.0001
