This is a demonstration of the methods for catheter care according to Prometrics guidelines for the nurse aide test. My name is Dr. Lee. I hope you enjoy the video. If you have any questions about catheter care please leave the question in the comment section below.

Knock, introduce, and explain 2. Gather supplies (3 towels, 4 wash cloths, water basin, soap, privacy blanket) 3. Wash hands 3. Place 1 towel on table as barrier 4. Use paper towel to turn on and off water, and fill the basin. 5. Test water 6. Put on gloves 7. Ask patient to test water 7. Place barrier under patient 8. Lift privacy blanket to hips and sheet down to knees 9. 1 Wash and 2 rinse pinched catheter 10. 3 wash and 4 rinse entire vagina 5. Dry everything using big towel 6. Cover patient 7. Remove barrier under patient 8. Gather dirty linen and wash basin in sink 9. Take off glove and wash hands! :)

Hi Dr. Lee, I never leave comments on any videos but I have the urge to come say “Thank you” I had my skills test yesterday in Dallas and I passed, it’s been a year since I took the cna course. Hello! I watched all types of videos for this skill and all of them had the patient moving and sliding everywhere and I was concerned with how much the patient could handle. Seeing you simply lifting the patient up put me at ease because I was PRAYING that lifting would be an okay thing to do for this skill

Hello Catherine, I am glad to help. Keep in mind, if the mannikin is to heavy to lift, Prometric states it is acceptable to ask the Evaluator for help lifting or holding the mannikin on the side. In the real world we ask for help with patients when needed.

Thank you, Doctor Lee! I`ve been watching your videos and they helped me a lot. I passed skills from the first time. In your videos you reminded about the key points all the time and stroke as a person who does know that stuff and much more. Strangely, but if your videos were 100% perfect and you did not admit to forgetting some little stuff I would find that standard unattainable. My anxiety stopped to kick in and I could learn.

QUESTION: @texascnaskills i noticed you gave her the call light while wearing dirty gloves. I know that the call light is also dirty but it feels like that is wrong. AM I WRONG?

Hello Eliza, Prometric will not count it against you for giving the call light to the patient with the gloves on. It is also acceptable to put up all of your supplies, remove your gloves, wash your hands, and then give the patient the call light. Prometric will grade the student on weather or not they gave the call light to the patient before the skill is done.

Hello, The Prometric evaluator will not penalize the candidate for touching the call light with gloves. Great question.

Hello Izy, I pray you passed the test. Students are not required to place a linen protector under the patient at the end of the skill. You can give the patient the call light with your gloves on.

Hello again Dr Lee! I am taking my skills exam at the end of the month so I am watching all of your videos continuously. You make things so much clearer than in my skills class. One question if you start with washing the catheter before moving onto the peri area do you need to dry the catheter? In skills class they told us to do so, but it seems silly since the catheter gets wet again as you clean the peri area. I noticed for your video you only dried both areas afterwards.

Good day Dr Lee, where do you advise we pour the water, in the toilet or wash hand sink, our teacher said anything from neck down goes to the toilet. I'm confused. Thanks Ma

Hello Eky, The only thing that MUST go into the toilet is our waste products (BM and urine). Bath water is acceptable to put in the sink even if you are doing peri care. Remember during the state exam we wont be cleaning BM or urine from the mannikin. Some patients even take a wash off or partial bath at the sink and not over a toilet. (LOL)

Hello Lovelyne, Prometric allows the student to empty the basin in the sink. We are washing the body. It is no different from a patient washing up at the sink while sitting in a chair.

Hello Raquel, Prometric does not tell the candidate how to check the water. We must check the water and have the patient check the water. Therefore, the applicants can test the water with their fingers, wrist, elbow, or a gloved hand. There are no guidelines on how to check the water.

@@raquelserranogarcia Hello Raquel, I think your instructor likes the wrist. It really doesn't matter where you test the water. Think about giving a toddler a bath in a tub. Do you test the tub water with your fingers or wrist before placing the baby in the tub? Take care

My instructor had taught us to wipe one side then the other then the labia. If the groin is included when washing the side is the sufficient or does the groin need to be done separately?

Thank you for your wonderful videos. I have a question. Which is first to wash, labia area or a catheter? Although you cleaned up the catheter first, the order was labia area first on the checklist of Prometric.

Hello Megumi, We are doing the catheter care skill, so I started with the catheter. Prometric does not tell us where to start at with this skill. If you go back and review the modified bath skill, you will notice that Prometric SPECIFICALLY tells us to start with the eyes. Therefore, we can't get marked off for starting with the labia or the catheter because a starting place is not specific for this skill. I hope this makes sense.

Hellos Dr lee, I have a few questions. If you’re going to perform your first skill out of the 3 tested do you have to actually wash your hands the first time even if the evaluator said you can assimilate? Also, shouldn’t you have a trash bag and dirty linen bag with you and if not are you allowed to set them on the table after using? Thank you you’re videos are very helpful!

Hello Natalie, You will have to actually wash your hands for your first skill. The evaluator will grade you on hand washing at that time. For the remaining skills, you ONLY need to tell the evaluator that you are washing your hands. You don't need a trash bag for the dirty linen with you. The site should have a linen container close by the bedside when you test. If you are using the bedside table, you must place a towel or something on the table before placing clean or dirty items on it. Prometric will NEVER count it wrong is you place dirty things on top of a towel that is on the bedside table. I hope I understood your question. Good luck with your exam.

Hello, Great question. Let's look at the instructions from the state. Pay attention to #3. The gloves MUST stay on until all supplies, including the dirty linen, are put up. So, please keep the gloves on if you'd like to pass this skill. Does the candidate: 1 Greet resident, address by name, and introduce self? (IC) 2 Provide explanations to resident about care before beginning and during care? (IC) 3 Wear gloves while providing catheter care, cleaning equipment, and handling soiled underpads, washcloths, and towels?

Hello, The normal pulse rate is between 60-100. The normal respiration rate is between 12-20.

Hello, For catheter care the state only requires the student to clean the catheter and front genitals ONLY.

Good day!! Thank you so much for making these videos. As an instructor they are a great tool. I do have a question. Our Manikins are small and one of my students is cleaning in this manner. 1 wash cloth folded in 4 she washes the right peri area and groin with one wipe, next side of cloth the other side of the peri area and groin, 3rd side the labia area, the 4th side wraps the catheter tubing and wipes it. She rinses in the same manner. Will this be acceptable??

Hello Christine, This is an interesting question. I don't know. The instruction doesn't require the student to use a different towel for the catheter and genital area. I will ask around to see if I can find an answer from other instructors.

A gown should never be used as privacy protection. Some women wear very sheer gowns, and their body parts are exposed through thin clothing. A bath towel should be used to dry the client.

Hello Annie, When testing through Prometric, it is not necessary to check the bed wheels or raise the bed.

Hi Dr. Lee, after catheter care, you did peri care. What about the rectal area? Does all prometric guidelines are the same in all state? I'm from NY. Thanks Dr. Lee.

Hello Albert, Prometric guidelines in New York is the same as we do in Texas. Please see the link below to New York Guidelines. www.prometric.com/sites/default/files/NAClinicalSkillsChecklist.pdf So, to answer your question, Prometric only required the candidate to clean the front perineal area with catheter care. See pages 5-6 when you click on the link above. Cleaning the rectum is NOT apart of this skill while testing.

Hello, Prometric does not grade against the student if the student touches the call light or curtain with gloves on. This is not a requirement in the skills checklist.

My advice would be to change gloves after washing. Charge gloves again before putting on underwear/ brief. I personally used one washcloth for each area just to make sure. For state exam. New rule says never touch faucet with bare hands. Paper towels turn off and on.

Hello Ventura, You don't have to replace the barrier under the patient with catheter care. The only skill that requires you to leave a barrier under the resident is PeriCare.

Hello Julie, You will actually wash your hand with running soap and water for the first skill. After the first skill you will just tell the evaluator that you will wash your hands at the appropriate time.

Hello Lily, Prometric does not tell us what to use as a barrier. We need to keep dirty supplies off the bedside table that the patient will eat on.

hi on cath cath once we are done and we are putting supplies away basin etc and then i removed gloves and wash hands -are we allowed to do all that and remove privacy blanket with no gloves on?

Hello Raquel, You must have on gloves when putting away the dirty towels and cleaning the basin. Look at number 3. Prometric's skills checklist states the following: Does the candidate: 1 Greet resident, address by name, and introduce self? (IC) 2 Provide explanations to resident about care before beginning and during care? (IC) 3 Wear gloves while providing catheter care, cleaning equipment, and handling soiled underpads, washcloths, and towels?

Hello, The steps you speak of ARE NOT tested during the state exam. Those are personal choices to make when providing catheter care. No where does the state require students to cross legs and arms and roll a patient to place a pad/chux under the patient.

You must Wash, Rinse, and Dry the Catheter. Most people dry the catheter immediately after washing and rinsing, BUT, Prometric will not count it against you if you dry the catheter later. The key is to wash-rinse-dry before saying I am finish with the skill. You must also Wash, Rinse, and Dry the front genital area.
