i suggest going to where you bought the sim card and ask them to help you, they should be able to

How do you get to the screen to enter the PUK code. My phone is stock on the "enter PIN" number screen and I don't know how to get to the screen to enter the PUK code.

If you have forgotten the pin you used, keep entering the pin code until it pass the maximum time, then the phone will be locked aksing you to enter the PUK code
