Documents to upload 1. Employment form (handwritten or typed) 2. Qualification form(handwritten or typed? 3. Transcript (Clinical and theoretical breakdown)...should be notarized 4. Curriculum/Syllabus...should be notarized and reflect hours 5. Certificate of good standing 6. Coloured copy of passport...should be notarized 7.Gap letters...if necessary Additional Documents 1.Affidavit.. should be notarized 2. Marriage certificate ...should be notarized

Yes do that . Write a letter to explain why u didn’t commence work immediately you completed rotation.

Dear, In my country, lebanon -Educational cerificate can be attested from the ministry of education, the transcript and the syllabus as well _good standing also can be attested from the ministry of health as they issue the health license Is this what you mean by notarized? Thank you

Hi Bendy , Yes you can however if you are changing to your husband’s name then affidavit is the best .

I was honestly having a hard time before coming across this video but please my concern is,are we required to upload all the required documents ourselves or is it to be sent officially to Ireland board websites by the schools or organizations because I am quite confuse as to either collect the documents and upload or request that the school send to them directly. When we open any professional experience line item it shows the details filled however not the document that we indented to upload alongside it Is it feature of the website that it will not show the document or is it like the document is not uploaded ?
