Before my comment you have my sincere condolences... I did some research and a very important part of the autopsy, the toxicology report, is missing... It's mentioned in the report that no specimen was present to do so. A toxicology report shows the information about the levels of toxins present in the subject's blood. This would have probably made direct comments on why Rich actually died. Sadly though, we would never know now... Rich was an inspiration to many and he'll always be sorely missed.
