This video runs through all of the clothing options and accessories (not weapons) for the appearance/redesign area. Note that greyed out items are greyed out not because they're missing, but because they don't work with something else you've equipped.

Doesn't look like the response took so i'll write it again. Sorry, i don't know where i got any of them really. I know i got them from looting enemies I've killed though.

I'm sorry, i have absolutely no idea where I picked up any of them. Just make sure you loot all the clothes. Anything you loot will end up here even if you sell it, so find someone in the outfit you want and most likely you'll be able to loot it off them.
