The trach is out and the patient is in respiratory distress!! What do I do now?
The best treatment in a tracheostomy emergency is not mouth to trach resuscitation, although it is tempting! The trach box has many useful tools to aid in ventilation until an RT or physician can replace the tracheostomy.
If the patient has respiratory arrested or is in impending respiratory arrest, cover the stoma with gauze and ventilate through the oropharynx until help arrives.

Decannulated with functional upper airway and is in respiratory distress. Unsuccessful in attempts to insert same size and step down trachs. Based on the ventilation cycle call 911 then perform. GREAT JOB!

if Trach comes out first thing to do is attempt to put back in , before BVM.

Me too but for the untrained person it could be easily misplaced outside of the trach

Why wouldn't you do BVM to stoma when the trach dislodges? If they have upper airway complications then delivering oxygen via nose/mouth would be ineffective. I'm loving your videos and they make me think more critically so I'm just wondering!

With dislodgment of the trach, a bvm can force air into the subcutaneous space. I've seen it happen before.. it was insanely bad.

Subcutaneous emphysema overtakes the patient's head and neck within 3 or 4 breaths
