We had Isuzu npr and Mitsubishi fuso trucks at my Dads shop back in the 90s and early 2000s, prior to those they ran E350 econolines. The Isuzu’s were tough. Half were gas. All were rode hard. Never any major issues. Also those trucks have the GM 4L80E trans.

I think your starting issue is down to fuel draining back into the tank after it's been stopped for a while. There's a lot of height difference between the fuel line at the tank and the carb, so a single fuel pump can struggle to prime and supply fuel after it's been idle. Fitting a second fuel pump near the tank along with one up near the engine will help cure the problem, this is what is done often in fuel injection and diesel vehicles, you have a fuel lift pump at the tank and then the injector pump at the engine. The best option would be to find a pump with a non-return valve to maintain the prime or to fit a non-return valve between the tank and the first, lift pump and that'll help cure the problem. It'll also mean the fuel pump up by the engine lasts a lot longer as it's not trying to pull the fuel up from the tank and pump it into the float chamber as that'll be what's killing the fuel pumps, they're having to work too hard which means they're running too much and, inevitably, burning out...

My truck used to hard start like that after a couple of years the fuel pump went afterward when replaced it started right up!

check the return line check valve on or near the fuel tank it may be losing prime because the valve is open
