A Beauty, Sclerosing Rhabdomyosarcoma, a very rare subtype of Sarcoma Malignancy. MD Gardner, I like the approach, always inquisitive and enterpreneurial, having an eye for the morphology and the actual chemistry of the pathology. Modus Operandi: Desmin, SMA Actin, S100 and SOX10 for neural/melanic proteins and pancytokeratin (Broad spectrum cytokeratin markers for Carcinoma investigation). This is a Translocation Aetiology and highly defiant variant. I reckon the pediatric prognosis, just as for the adult version, is notwithstanding dismal and poor. Those spindly cell proliferations are just plain egregious. Es immer gesund und friedliche zu Gesundheit machen. Viel Dank. Prost!

is it possible to have diffuse strong cytokeratin stain in spindle cell RMS?

I’ve never seen that but I have only rarely encountered this entity. I suppose it is possible but I would want to make sure the clinical scenario, morphology, and IHC findings otherwise fit well for RMS. Spindle cell carcinoma would be the other thing to think of.

@@JMGardnerMD Agree. I'm part of a RMS clinical trial and have seen a few of these now and CK is usually negative or weak.

I wanna be an pathologist but this one doubt running on my mind always will AI will replace pathologist?

I think AI will eventually be able to help pathologists do our jobs faster safer and better. It will be a tool to assist us just like the other tools we already use. It’s hard for me to imagine AI replacing (or even mostly replacing) the work we do any time in the foreseeable future. Just because an AI algorithm can look at an image and decide that it’s cancer does it mean that it can sort out all of the diagnoses we make, use common sense to recognize artifacts or false positives/negatives, or that it can be integrated as a complete smoothly functioning package into a hospital laboratory. Just look at EMR programs. They are relatively simple from a computer standpoint…Databases and user interfaces with data entry and search functions. This type of computer program has been around for many years. Yet EMR programs still have significant flaws even in 2022. Usually this has to do with some combo of poor design that doesn’t take users into account, inadequate finding or tech support for implementing a system correctly, and inability to interface with EMR in other hospitals systems. If we struggle to make EMR work as well as it should even after all these years, it boggles my mind to imagine how we can plug-in an AI system that’s going to read all of the pathology report out the diagnoses in a way that safe effective and efficient. Even decades from now even if AI algorithms are very good at making diagnoses I still have a hard time seeing an automated system that can do all of it without human pathologist help. Maybe I’ll be proven wrong but having worked a lot as a pathologist and done a bit of research and publishing related to AI, that’s my current view.
