N233R mavigates through dangerous icing conditions… diverts and lands safely
Story 2
On December 20, 2011, about 1005 eastern standard time, a Socata TBM 700, N731CA, collided with terrain following an in-flight loss of aircraft control near Morristown, New Jersey. The airplane was registered to Cool Stream LLC and was operated by the pilot. Visual meteorological conditions prevailed and an instrument flight rules (IFR) flight plan was filed for the flight from Teterboro, New Jersey (TEB) to Atlanta, Georgia (PDK). The personal flight was conducted under the provisions of 14 Code of Federal Regulations Part 91. The certificated private pilot and four passengers were fatally injured, and the airplane was destroyed. The flight originated from TEB about 0950.

In the first scenario, ATC literally saved that pilot’s life. His questions- his advice- all the way through to handoff at Ontario Airport was outstanding. That tape recording should be saved for the FAA training classes on how to assist single pilot aircraft in IFR conditions. That guy deserves an award.

That first controller was outstanding. Great situational awareness. He clearly saw the danger and moved aggressively. He didn’t even need to wait for the pilot to ask, but instead kept inching the aircraft down as much as terrain would permit. He didn’t task-saturate the pilot, and he repeated his instructions distinctly and with a proper cadence. Great job! Truly first class.

N731CA forgot to turn on the inertial separator that is what tue NTSB suspected to be the reason he said getting a little rattle up here. When they found the wreckage if you look at the docket the inertial separator is the only switch on the panel off. So he allowed the engine to ingest ice while trying to climb. Nose up with no power and covered in ice turned into a stall and a unrecoverable dive. All 5 people including the pet dog got ejected during breakup. The descent rate was crazy all the way from 17,000 feet.

"yeah, we are okay to go straight through it..." The TBM is a very capable aircraft, but a PIREP of moderate to severe ice? Shit went wrong really quick. RIP.

When you have an experienced pilot, looking at it from as close as a helicopter would be circling it, and he can't even identify if it was a rotary or fixed wing aircraft, you know it was a horrific impact. 😞

The first pilot might have faced disciplinary action from the local FSDO. All in flight emergencies get reported to the FAA Flight Standard District Office, and this pilot stating he wanted to continue his flight and return to his home airport despite having no ASI or VSI was exceptionally bad judgement.

I'm not a pilot. But I was questioning his sanity when he tried to go to Hawthorne instead of landing at nearest airport. Controller was looking out for him that day.

Not sure I agree. At one point he said IAS came alive. He was in VFR. Better course of action was to land, but I’m not sure it rose to the level of disciplinary action to continue back to Hawthorne. He also corrected himself pretty quickly when IAS died (again) and he entered IMC (again) as he climbed back up to 5,000. It all comes down to judgment; and judgment does not have to be perfect to still be within acceptable limits, particularly in such a fluid situation. Also, the aviation authorities do not want to discourage declaring an emergency, so they are not quick to jump down anyone’s throat. (You said FAA, but I think this was in Canada, so it wouldn’t be the FAA). Just my two cents worth here.

Yo if you’re going to fly into cold clouds make sure you buy a FIKI equipped plane 🤦 this isn’t a rocket appliance.

Thats a good thing to have but it usually gets overwhelmed quick. A lot of the icing accidents are FIKI equipped airplanes. The pilots stay in the icing conditions instead trying to pass through it and it gets overwhelmed or they forget to turn on things like the inertial separator and allow the engine to ingest ice.

Have you ever noticed there are never women that make these terrible decisions in the air? Macho kills. even the first guy tried twice to go back to Hawthorne. Why? Get a damn hotel. Arrive alive tomorrow. The critical question is “how was the weather the next day?”
