In The Sims 2, Sims that have level 6 of the Show Business Career, can get Dr. Vu's Automated Cosmetic Surgeon. This machine allows Sims to change their facial structure. If it's used successfully, it will open a CAS, where you can change your Sim's face however you want. Once the changes are made, they cannot be undone. But there is no limit on how many times you use the machine. But beware...
Because things can go wrong.. so wrong
every time a Sim uses the machine, there is a chance that it will result in changing the face in the most random way. You Sim will scream when they see the result. here are some "procedure gone wrong" results: Using the Automated Cosmetic Surgeon. But don't worry, it does not affect the Sims' genetics.
Sims 3
Cosmetic Procedures were added with The Sims 3: Pets. But now, Sims can get them in the hospital. And in The Sims 3, Sims can now modify not only their face, but their body too. And you can get it done expensive or cheap. If you decide to go for an expensive beautification, the chances that the procedure will go successful will be higher. Like in The Sims 2, a CAS will open up, and you can modify the Sim's face however you want. If you decide to go for the cheap beautification, there is a 40% chance that the surgery could go wrong. Your Sim will get a randomized face. When things go wrong, and they will be embarrassed.
The body modification works the same. If you choose the expensive procedure, The chance for failure is lowered to 10%. Quite exquisite
Again, if the procedure fails, CAS won't open, you'll get a randomized body.
Sims comparison - Sims 4 and Sims 1 don't have this option.
