Sims 2
In The Sims 2, Sims that have level 6 of the Show Business Career, can get Dr. Vu's Automated Cosmetic Surgeon. This machine allows Sims to change their facial structure. If it's used successfully, it will open a CAS, where you can change your Sim's face however you want. Once the changes are made, they cannot be undone. But there is no limit on how many times you use the machine. But beware...
Because things can go wrong.. so wrong
every time a Sim uses the machine, there is a chance that it will result in changing the face in the most random way. You Sim will scream when they see the result. here are some "procedure gone wrong" results: Using the Automated Cosmetic Surgeon. But don't worry, it does not affect the Sims' genetics.
Sims 3
Cosmetic Procedures were added with The Sims 3: Pets. But now, Sims can get them in the hospital. And in The Sims 3, Sims can now modify not only their face, but their body too. And you can get it done expensive or cheap. If you decide to go for an expensive beautification, the chances that the procedure will go successful will be higher. Like in The Sims 2, a CAS will open up, and you can modify the Sim's face however you want. If you decide to go for the cheap beautification, there is a 40% chance that the surgery could go wrong. Your Sim will get a randomized face. When things go wrong, and they will be embarrassed.
The body modification works the same. If you choose the expensive procedure, The chance for failure is lowered to 10%. Quite exquisite
Again, if the procedure fails, CAS won't open, you'll get a randomized body.
Sims comparison - Sims 4 and Sims 1 don't have this option.

@@jacksont9455Which is accurate because Michael Jackson's nose only ended up as small as it did because after getting multiple surgeries on it he experienced tissue death. So they just covered up the bone of his nose with spare skin.

Love this! Also, IIRC, in sims 3, if you send a sim to the hospital for surgery but cancel when you get to CAS, you get a cute lil message about how the sim has changed their mind and likes themselves as they are or something along those lines

this actually happened in a russian ts3 let’s play that I watched back in 2012. the couple playing it (gf on controls, bf commenting and joking with her) spent an entire episode changing the face of the character that they purposefully made really funny looking (they basically made her to be a house maid, all of her traits were about cooking, cleaning and repairing), the surgery failed, they went back to the last save and just let the sim be as she was. the “ugly” sim then proceeded to a marry their pretty boy main character and live happily ever after. it was truly an experience

About the Sims 3 aliens why they can't get plastic surgery due to cultural issues, the real reason is why they can't is due to coding issues. But despite that, you can still change the alien's faces by using TestingCheatsEnabled first and Shift+Click on the alien Sim to Edit in CAS. It still works without issues.

Why they would have a coding issue ? They are just like regular sims but green with black eyes. If they wrote that phrase it's because they programed that way.

@@augusto7681 they have a separate coding in order to use alien abilities - same thing with mermaids to correctly apply their scales and plantsims, mummies and servobots (from ambitions) to correctly disable their motives. but yeah you can create them with nraas easy enough, you just can't save them in the library

So I had a sim who was from a legacy, he came from the most prominent family in town. The family had "normal" faces but I wanted to make them look like a prominent celebrity-ish goodlooking family so I had my sim undergo plastic surgery, the expensive kind but he ended up with the ugliest face I've ever seen in a Sims game. I made him marry a very beautiful girl and luckily the child looks good lol

for me even though i love the sims 3 the winner is the sims 2 because there is no effect on genetics in the sims 3 kids can inherit fixed facial features. And the operation to correct the failed operation does not return the original appearance, but gives a pudding face so they lost their genetics forever. I prefer to fix that random face with a repeat paid procedure

Both Sims 2 and Sims 3 plastic surgery doesn't affect the genetics. I also don't like to pay for plastic surgery and trust me even the expensive ones went wrong ❤️

@@tintaverse oh, I had that in one save file when cheap operations were always successful, and two expensive ones both failed. I corrected the shape of the lips and one of the children of the next generation inherited these lips, but maybe i need to check it again

@@tintaverseActually, unless you use mods, plastic surgery will alter the genetics in Sims 3, same thing with changing hair color (mother or father), even if done so during pregnancy. Also, if the partner's life state (i.e. became a vampire), while their significant other is pregnant, even that change could be passed on to the unborn sim. Sims 2 has the most realistic genetics of all the mainline Sims games.

It’s hard to choose a winner here because the ”procedures” are so different from each other, you can’t really compare them imo. Sims 2 definitely wins in drama, weird technology and randomness, and the gone-wrong faces are straight up gold xD But Sims 3 wins in realism, for sure as the sim actually does it in a hospital. It depends on what you’re looking for imo.

I had a case with the Sims 2's plastic surgery where I forced random sims into my current playable sims, and forced them to run through the machine. Most of the time I get weird big eyed ones with hawk noses, but the one I think that was the best in my books, was how one of them ended up looking like they had a giant, baby head-like shape, with a massive forehead. XD
