A young man had fractional CO2 laser treatment on his cheeks and forehead, presumably to treat acne scarring. He says that there are redness and hyperpigmentation issues still visible 4 months after the treatment. He asks what he should do, and adds he is a smoker.

Hello dr ... i was going to start my acne scar co2 lazer treatment due to acne scars.... but is it safe my dr suggested me to get this or microneedling(4-5) settings with one lazer treatment zzz and at last they prefer mnrf so can you suggest which i should prefer as my scars are not that deep but still i got so many on my cheeks... n they are redishbrown in colour.....just need your suggestion

Hello sir Thank you very much for the detailed information. I would like to ask you something I have a pigmentation(dark colour ) on my face exactly on the beard area due to the usage of beard colour product. It's been 2 years I am using the same product but I noticed 2 weeks ago.I have consulted a dermatologist and he prescribed a lotion (Desonide Lotion) & vitamin c serum for two weeks. However I heard that laser treatment is the best option to get my skin colour back Is it true? If I am taking the laser treatment will it affect my hair growth? Because it's my beard area? My problem is the same 2 weeks ago I have a CO2 laser treatment but I'm quite disappointed because it seems that the area base on my cheeks is the important place that I expect to be better as it is the only pace that I did paid but the outcomes after a week is like I felt the scar becomes more and I got pigmentation and dark spots which is very annoying..is it normal or laser isn't reliable for a back to normal skin?pleae3 answer my question.thanks

Hi Sheryl mae palmaira, CO2 laser may not have been the right treatment for you, especially if you have dark skin, as it can cause hyperpigmentation. Remember that it's not the laser that determines the outcome, but the experience of the doctor or practitioner using it. I hope this clarifies things for you.

Hi Sheryl mae palmaira, It is possible, but I don't know the details or extent of your procedure. Laser treatment often requires more than on treatment session. I often alternate laser treatment with PRP to continue to stimulate collagen, but reduce exposure to heat. It is best to follow up with your doctor as they know your skin and details of your procedure, and I don't know those details. I hope this helps you.

Hi I had a fully ablative co2 on my arm 5 weeks ago to correct scarring left from a tattoo that i had removed. The co2 made my scars worse and gave me new ones. This has been extremely stressful and I don't know what to do. Can you please give me advice?

Hi Rebecca, Sorry to hear about your problem. You have to be very careful with laser treatment, especially with fully ablative CO2, as it is very aggressive, and not well regulated in many states so it can be done by a non-physician. I would have to perform a physical exam to give you the best advice for skin and tissue rehabilitation. I hope this helps you.

Hi jenifer grande, Depending on your individual skin conditions, I recommend you space your treatments 4-6 weeks apart to allow for healing and collagen formation, and not make them too aggressive. Alternating between microneedling and laser is also a good idea, and you can consider PRP as well for collagen induction and to improve skin quality. Hello Dr. Great video and insights! May I please ask if Smartxide laser with low setting is safe for skin of colour and will not increase the chance of PIH? I have ice pick scars, and the place I live in, I dont get much of an option - its all mostly Co2 lasers.

Hi Dheepa, The safety and results of the laser treatment depend on the skill and expertise of the doctor performing the treatment more than the brand of laser. If your doctor has used this laser on others with dark skin, then they are better able to tell you about safety and effectiveness since they can examine your skin, and the acne scars you want treated. Please i need your help, 7 days ago I put lutronic CO2 laser, was very high power and several times in the same place, that was on the nose to treat scars that I have in the area of The nose, the scabs have fallen and now I have many more holes than before, and I have certain white blisters, my skin looks worse, much more battered than before, please tell me what will happens and if I have the risk to have more scars than before? I realize it is very stressful for you, but I do need to examine your face in a physical exam before I can determine recommendations for skin and tissue rehabilitation. I had fractional co2 laser about 3 years ago . I am still experiencing noticeable redness and I’ve tried absolutely everything. Some dermatologist say I could have rosacea but that doesn’t run in my family nor do I experience all the symptoms. Is my face still healing or is 3 years out considered prolonged erythema and should/Can I do anything about this?

Hi bigboybear2323, Sorry to hear about your problem. It isn't likely due to laser treatment done 3 years ago. Rosacea is not a genetic condition, and you don't need to experience all the symptoms to have it. Since your dermatologists have examined your skin, and I have not, I would have to defer to their judgment. I hope this helps you.

Hello..doc...i've done fractional co2 laser in my stretch marks at tigh and part of my legs it was my first session but i worried because after 4 weeks the wounded stretchmarks seems not a good result because its get dark..and its like discoloration of the affected areas

Hi Alexis Fernandez, Sorry to hear about your problem. I suggest you see a doctor with a lot of experience with lasers and heating devices, and a lot of different types of these tools. There are other lasers or radiofrequency devices that can treat stretchmarks that are not as aggressive as fractional CO2, and may be better suited for your skin type and color.

I hope you understand that I would have to examine your skin before making treatment recommendations, otherwise suggestions without an examination can make it worse. I suggest you bring up your concerns with the doctor performing your current treatment. If this doesn't help, you need to have your skin examined by another doctor for a proper second opinion. Thanks for this video, Dr Prasad. I have had two laser work done with a Korean doctor. After finding out about hyperpigmentation and fat loss on my own, I decided to stop laser for the time being. I am Asian and do not have lots facial fats. I am glad to learn all these because doctors don't tell us things and we do not know how to ask the right questions until we are educated. And such a good idea about using Retinol products after laser work.

Hi Asma Shaikh, Laser treatment is not always the best treatment for acne scars, and the brands of lasers are not as important as the skill and experience of the doctor using them. Your doctor can give better judgment as I have not examined your acne scars or your skin. If you are not sure, have a consultation with another doctor who can examine your skin, and decide from there. I hope this helps you.

However, your skin does need to be examined before a recommendation can be made. Hi Sir, About 1 months ago I had co2 fractional laser treatment done to my face for a few mild acne scars only, now it appears my skin has hyperpigmentation more scarring, So far I see no results, only my skin looking worse. First off, you're a great bloke for answering all the queries. I had my second session few days ago, high intensity and now I have pigmentation on my cheeks that also looks like blisters. Should I be worried or do these go away eventually. I am from Nepal so I have medium skin complexion. Side question, do I need to put on suncreen at night to protect my skin from rays coming from light, electronic devices? Thanks.

Hi Bip me, I suggest you wait while your facial healing and appearance and improves, but if it hasn't over the next week or so, you can follow up with your doctor for a proper examination. I only advise sunscreen for sun exposure, and not for electric lights indoors. I hope this helps you.

Hi@@bipme4495, I wouldn't classify it as "normal" without seeing it, but you will have to wait as it heals. You don't need to protect your skin from electric light, it is more intense solar radiation like UV exposure that you need to address. I hope this helps you.

Dear doc, my lower eyelids have been resurfaced by fractional co2 Laser, 18 weeks ago. I am wondering, why there is still redness by 4,5 months post laser - but far worse: it seems like I got some scarring on my lids! Indentations making my lids looking much older and pretty bad. My surgeon is gaslighting me. Any suggestions? I am thinking about trying tretionin after 6months to see if those indentations can fill in…

I had Erbium/CO2 Fractional laser two weeks ago. I’ve been following instruction and strictly avoid sun. I was aware of PIH due to my olive skin tone and I’m experiencing it on my entire house right now, it’s very dark, I started working out just a few days ago and I sweat a bit especially when I use the rowing machine. Does working out and swearing worsens PIH? Can I begin applying my bleaching cream with 6% hydroquinone? Thank you.

Hi Eira Rivera, Working out does not worsen PIH, but you should consult the doctor who performed your treatment before using any bleaching cream. I haven't seen your skin, or results, so your doctor would be better in recommending specific treatments for possible PIH. I hope this helps you.

Hi, I had a very aggressive erbrium laser treatment on my face 1.5years ago and now I have so much hyperpigmentation from that laser. How do I get rid of this deep dermal hyperpigmentation please? I’ve tried hydroquinone and Tretinoin and it hasn’t worked :( Are there any other lasers that would work? Like picosure? Based in the UK, derm is no help.

Hi blue green, It does take a physical exam by an experienced doctor to determine what's best for you. Lasers are just tools, and it's the experience and expertise of the doctor using them that determine the best course of treatment. Hi vishakha kale, It’s important to remember that the laser itself does not cause complications like facial volume loss, nor does it determine successful results, but rather the experience and judgment of the doctor using the laser or other device. The device is just a tool, and it’s the doctor’s judgment using the tool that separates great results from ineffectiveness, or potential complications. Hallo doctor,thank you so much for answering question- I had pigmentation on my chin for first time of my life and I am 33yrs old and Asian .I used sunscreen 50 and always protect myself from sun.I don’t smoke and take healthy diet. I did laser on my pigmentation hoping it wil go away but I had opposite reaction-it first appear burn ofcourse,than skin start peeling off and I didn’t touch anything. It appear red and now it’s kind of 80% more darker Dan before.I start using retinol now.What should I do more?? I am getting so self conscious now and it is putting my self confidence down.

Hi Doc, thank you for taking YOUR time to help many! The oath you took is appreciated even if it is rarely seen or heard these days...Some docs do care so thank you!! I too had CO2 laser and am of Italian decent. I do not smoke and take great care of myself. My twin sister and I had the Active FX C0 2 done on the same day one month ago and have grid lines all over our faces. They are still very apparent, even with make up....Our skin turned dark.....not red. We have had NO SUN! The doc that performed it, shrugs it off and says it could take months to go away.....Help! I have SO much on the calendar for this summer. I am sick at heart...money NOT well spent...scared! I really look freaky!

I often do not perform fractional CO2 on Mediterranean or olive skin tones due to the problem of hyperpigmentation, which sounds like what happened to you. I would need to examine your skin to see if anything can be done to improve it. For now, you can wait it out, and make sure you are applying sunscreen regularly to prevent the problem from getting worse. In the future, you can consider fractional Erbium or ND:Yag laser as they can work on some darker skin tones. I hope this helps you.

Hi Shenoy Pereira, We do use lasers for dark spots, but lasers can cause other issues like hyperpigmentation of hypopigmentation on darker skin. I would have to perform a physical exam to determine your skin type and color, and also determine the appropriate setting for the laser, or see if another treatment may be better for your case and skin. I would really like to know your opinion regarding the effects of laser CO2 on skin thickness. I am 20 years old, with thick and light skin, and I have some acne scars. I am interested in getting an ablative CO2 laser, as I have tried 2 non-ablative fraxel and also 1 discovery pico, and they didn't do anything in my case. In some articles, I have read that CO2 laser can cause thinning of the skin and in some other articles I have read that CO2 is good for skin thickness and quality. So, the main question is if, besides hyperpigmentation, can an ablative CO2 laser also "harm" the skin thickness that is actually beneficial for the skin?

Hi Νέλλη Κ, If you didn't get the results you wanted from previous treatment for acne scars, I suggest you stay with same doctor, instead of changing doctors and devices if you don't get the results you want. Acne scar treatment usually takes several treatment sessions, and several different modes of treatment before you see improvement. If you keep changing doctors, and trying different devices with another doctor, then the likelihood of damaging and thinning your skin increases. It's not the device itself that causes the skin to thin, but the experience and expertise of the doctor using it, as well as not being informed about details of your previous treatments. Hi Dagher Haneen, There are many ways to treat acne scars. Fractional CO2 laser will take several treatments, so you will have to be patient with it and stick to your doctor's plan. Generally speaking, fractional CO2 laser is good for acne scars that are not too deep, and have similar depth. It is also better for lighter skinned people. It is important to discuss your skin color with your doctor as fractional CO2 laser shouldn't be used on darker skin. It is also important to look at your doctor's before and after pictures of people he treated with similar problems and skin type as you. I hope this helps you.

Hi shankey kabtiyal, Laser is not always the best treatment for acne scars as the right treatment depends on type of acne scar, type of skin, skin color, and other individual factors. I could not recommend specific treatments without a physical exam, but these include subcision, laser resurfacing, chemical peels, PRP injections, cosmetic fillers, and other treatments. The best thing to do is have a treatment plan outlined by one doctor, and follow it patiently as acne scar improvement does take more than one session and more than one type of treatment to improve. Hi dhivya lakshmi, I don't know the details of your treatment or skin, so I really would need to perform a physical exam on you. Laser treatments have different energy settings, and exposure time by number of passes. Also, you don't indicate the time between sessions because healing is important, and exposure should be limited. Also, I need to see your skin type. Have you consulted your doctor or practitioner? They know your skin better than I would because I've never examined it.

Hello Dr. My name is Mary.. I am an dark skinny with burn scars.. I've seen a doctor and he has advised me to have a laser treatment.. I'm still not sure if I should proceed with this treatment..?! Is it safe?

Hi Mary, Laser treatment can help with burn scars, but it is mostly dependent on the doctor's experience with laser treatment, as well as laser treatment on dark skin. I would have to defer to your doctor's assessment as they have the advantage of physically examining your skin and your scars before recommending a treatment. I hope this helps you.

Hi, Doctor. I am Asian, and I have done Fractional CO2 Laser 3 weeks ago. I still have some mild redness on my upper cheeks near my under eyes, especially on my right side because it was were I felt the stinging sensation the most for almost the whole week of my healing time. I developed 2 tiny brown dots on both cheeks. I am anxious that my redness will turn to brownish pigmentation. I went back to my doctor, and she prescribed me melanin blocker cream, Hydrocortisone cream and Hydroquinone cream. Do you think this is just something temporary, and is normal for this redness to take time to go away? Thank you.

Hi @fainelynsakandal7125, Sorry to hear about your problem. I usually use a radiofrequency device instead of fractional CO2 laser on Asian skin types due to the risk of hyperpigmentation. I hope you understand that I would have to examine these dots like your doctor did to give you a proper second opinion. Without an examination, I have to defer to your doctor's judgment. Hi navjot virk, Sunscreen is for prevention of melanin production, not for the treatment of pigmentation. You don't indicate whether your skin became darker from laser treatment, for from sun exposure. I hope this helps you.

However, laser treatment may not be the best treatment for your acne scars. Hi sandhu kp, Heat treatments like lasers and radiofrequency can cause hyperpigmentation (too much pigment) when treatment is too aggressive, but treatments like lasers and platelet-rich plasma (PRP) can also be used to reduce pigment. 2.5months before I went to the clinic for mole removal she removed my moles via Derma pen moles are gone second session she gave me ov Co2 resurfacing and fractional after Co2 my skin is healed fully but the area which she did Co2 is darker then my origninal tone now brown spots are visible on my fair skin don't know what to do only dark spots are left and they are much prominent plz tell me sth should I go for pico laser???)
