Big My (Sonata) Secret - Analysis and Performance

What is the secret alluded to by this wonderful piece of music by Michael Nyman? "The Piano" character Ada plays this piece in the movie - it shows some classical influence to start with, but then develops into a passionate improvisation with lovely piano textures over lush chords. With pianist Henrik Kilhamn.
Unfortunately, my sound device didn't cooperate for the analysis part, so I had to use backup sound from a camera, which is not ideal.

Dear Henrik, I love this piece, and your website, and am learning Big my Secret now. Are you able to tell me -- on the second bar, RH, there is a double "D" note. This also appear elsewhere, for example in Bar 3 it is a double "B". I can find no info online as to how to play this? Many thanks, David.
