Athletic Pubalgia or sports hernia is a strain or tear of any of the soft tissues (muscles, ligaments or tendons) attached to the lower abdomen or groin.
The most common symptom for athletic pubalgia is groin pain. The groin includes anything that attaches to the pubic symphysis (the point where the two pelvic bones meet in the front) or the lower abdomen. This includes where the leg attaches in the hip crease. The most vulnerable muscles are the oblique muscle tendons that attach into the pubic bones and the adductor tendons.
Athletic Pubalgia Symptoms
Symptoms can include one or more of the following:
*Sudden groin pain at the time of the injury
*Groin pain subsides with rest, but comes back during sports activity
*Pain that occurs during twisting movements
*Pain associated with movements that involve the deep abdominal muscles,
*Tenderness or bruising in the upper thigh and/or lower abdomen
*Groin pain that gradually increases from intermittent to constant, and/or pain that develops to the point playing sports becomes impossible
Athletic Pubalgia Causes
Sports hernia typically occurs in athletes, however at Rebalance we have seen cases where athletic pubalgia was caused by pregnancy/delivery.
Typically, sports hernia is brought on by high-impact twisting activities. This can include any actions where there is a sudden change in direction, such as kick boxing, gymnastics, or a planting of the feet and twisting, such as in hockey or tennis.
Athletic Pubalgia is not usually caused by a specific injury. It usually starts gradually and gets worse and worse over time.
It’s often seen in athletes, but it can also occur in individuals who do not do high impact activities. It can be brought on by cumulative misalignment and poor body mechanics over time.
Athletic Pubalgia and the Pelvic Floor
If the athletic pubalgia comes on gradually, it could be caused by misalignments which are often associated with pelvic floor dysfunction. Tensioning in the pelvic floor is also associated with the over recruitment of the abdominal muscles. If the abdominal muscles are over working, this can strain the obliques and the adductors and lead them susceptible to injury over time from twisting actions.
Tensioning in the pelvic floor can be caused by an asymmetry in the pelvis. You might be over stretching the back side and, as a result, tightening up the front. This uneven pull can lead to tension in the pelvic floor, which can cause misalignments that make you more susceptible to developing athletic pubalgia.
Athletic Pubalgia Treatment
It’s important to release your pelvic floor, release your obliques and learn to use the transverse abdominus.

We want to release any of the tissues creating bad alignment and increase awareness of alignment or do hand on work to improve alignment. A great explanation - thank you for taking the time to post. Because we get basic health care for free in the UK, we are conditioned to feel guilty about going to the Doctor unless we are nearly dead. If I eventually have to visit the Doctor, it will take him less time to explain these issues to me. Thanks again.

This was so incredibly helpful! I had my 2nd child almost a year ago and I’m pretty sure I have a sports hernia. I’ve slowly been trying to get back into working out and the pain has slowly been getting worse until this week when it was debilitating. I kept putting it off as just weak muscles and normal hurting from working out but knew something was wrong. I’m making an appt with my PCP and resting until then.

I have surgery for my sports hernia in 2 weeks. I’m super super anxious as I want this to be what fixes my issue so I can be back to my sport. Lots of pelvic discomfort and lower abdominal pain for the last year. Thanks for the info!

This explains what I’ve been thinking all along. I used to think of this as “other muscles having to pick up the slack for the other muscles not doing the work”. I’m in HR, so the example I was using was: what happens when some employees end up doing the work of those not doing the work, so that the organization can continue to function. With time, the employees doing the extra work end up burning out and quit or go on medical leave. I also think of it as “muscles having to realign after an injury so that you don’t use the hurt muscles”, which could mean that you move differently to avoid using the hurt muscles. This video just puts everything into perspective and validates what I (as a non-professional in the medical field) have been thinking.

Thank you very much. I understand a lot about, but I learned the most from Dr. David Denton. DC. He told me with the L6 (my biomechanics) makes things change. And I have learned a lot with this body. If I had not learned it, I don't know where I would be. But your talking about the mechanics brought it all back. And so, when you have that, you are born with this instability in that area. Dr. Denton, felt that surgery would (at that time-- in the early '80s. Now, I look younger than I am. How it all started bothering me a the gym. I came home and had these two golf balls (hernia). It is better, but I was not happy with the hernia. And I exercise ( but not too heavy with weight) And most of my workouts are with machines. Anyway, thank you, because I feel it is stabilizing (and that area was locked up) That L6 was a pain (literally. So, I need a hernia belt. One that gives me compression in that groin area. I have a small belt, which helps it, but it is not a hernia belt. I don't want surgery -- and will not. But I will continue the way I am treating it, as those golf balls are down to almonds (at times). Overall, I think if I get the proper belt (last one sent back twice- first size and the second it was not snug enough. Again, thank you very much. More practitioners should have your knowledge. Dr. Denton worked on Burt Reynolds when he was hurt by a stuntman who picked up a real chair and clubbed him in the head with it. Burt Reynolds was a wonderful guy who was wonderful to all. Okay, I will keep you posted. Big strides ahead.

I'm 43 and I play softball. I had a nice triple the other day and I ran the bases ( pretty hard for me) I didn't notice anything till a few days. I do landscaping and gardening for a living. The pain is in my inner leg. I did notice when I take a hot shower I don't feel that pain I'll give it a week and if it doesn't go away time to see a Doctor.

For mine, like a knucklehead I did abdominal exercises every day after I started seeing a 6pack for the first time and I got obsessed. Little did I know that I was weakening the abs and then pop! Then the pain searing into my testicles was unbearable at times. The horrible part was I tore on both sides of my lower abs so about 2 weeks after the first pop came the 2nd pop! Now, I had pain radiating from the tear into both testicles and the inner thighs. Back in 2006 more surgeons would scoff at a SH out of ignorance but thankfully I found DR. Micah Sinanin at UW Medical in Seattle who had done many SH surgeries. It’s been 24 years and for the big majority of the time the surgery did what it was supposed to do. If I get a little to rambunctious then I can aggravate the scar tissue but otherwise it’s all good.

I had a partial tear of the long adductor muscle two years ago, and all treatments have failed. The doctor told me we are converting a partial cut into a complete cut. Does hitting the ball weaken the sole of the foot? Thank you so much.

So, the little spasms that I've been mistaking for heart attacks for the past 3 years might be the same spasms as on the other side of my chest, and in my stomach? My lower abdomen doesn't hurt, it itches and has for 12 years especially in the sun or heat. Ive had lots of lab test done in the last year chasing an std scare. All negative. Where's your clinic?

Did you ever find anyone? I too live in the SF Bay Area and I felt like she was talking directly to me! I have had cortisone shots to help with physical therapy, physical therapy, rest, etc. But I have never fully healed. Better but never 100% back. I have Kaiser so I'm stuck going to them and I don't think they have any specialists like that.

@@opivyattack Hi. No. I never found anyone. I have Kaiser, too. Couldn't stand the idea of using them for therapy, so didn't. After a year of extreme pain, etc., I figured some things out on my own. First, stretch, especially the butterfly. Second, rest. Third, don't take too much pain killer! This was a big problem. I took Ibuprofen. It dehydrates and causes fatigue. Icing helped a lot. Finally, after getting off the Ibuprofen and stretching 2-3 times a day, I began light weights. Then 2 months ago I started jogging lightly up/down our short road, very very slowly. Now I am able to run 2 miles, and can walk 4. And that is where I am at. I did have, just four weeks ago, a sudden snap adjustment to my left hip out of the blue, which caused my body to realign, and now my strength has really increased. Something in the groin snapped back into alignment, probably from the stretching and light exercise. Helpful? or No? Too much info? Incidentally I'm 52 years old, and a former soccer player. And diver/gymnast.

I have a problem with my lower abdominal muscle and groin. It started about year ago. Physical therapist has no idea. Rest doesnt really help it. I practice jujitsu and have had all kinds of injuries. I need someone to help me. My doctor doesnt even know what athletic pubalgia is

I started swimming freestyle using silicone webbed gloves that act as small hand paddles, after swimming for years 90 minutes almost daily. One time while swimming I briefly felt a short shooting/burning pain very close to the inguinal opening for the spermatic chord. I had a inguinal hernia repaired there 17 years ago when I was 22. I thought nothing of the pain as it went past quickly. Then I started using medium hand paddles. One night while laying down on my back I felt something odd in that area, a discomfort. When I palpate the inguinal area and around the hip bone crest anterior, there is a discomfort but not serious pain I guess it could be described as tender. Also as I rub my fingers over the abdominal muscles (six pack lower area) on the right side where the "hot radiating discomfort" is there is a bulge which seems to me to merely be a more developed muscle as I am right handed, I do seem to pull harder and faster on the right side when swimming freestyle with the paddles. Also when I swim freestyle, I squirm like how a snake moves and the muscles in my hip area grew as a result. So I think all these factors have give me a "sports hernia". I fear that I cannot swim at all for weeks now. Sitting upright for long causes a burning throbbing pain and it feels like something is pooling up, a liquid that drains away after I lay on my back and I can push the pooled liquid away.

Hello! I found this video extremely helpful! I was just "diagnosed" with athletic pubalgia yesterday after a soccer incident this past Sunday. I live in Phoenixville and just called to schedule an appointment at your Narberth location...However, it seems like business is going well since you are booked until October...LOL. I am having a great deal of difficulty finding orthopedists/PT's that understand and/or specialize in this type of sports hernia. I was wondering if you could point me in the right direction to someone that might be able to help me since I need treatment within the next few weeks or so. I am in a great deal of pain at the moment, so my thought is that I might have a more serious tear. Thanks again for this wonderful information! Made me feel much better! Jordan

Hopper830 I’m going to a general surgeon in two days and I’m assuming I’m gonna need surgery. Been having that sharp unbearable pain for 3 and a half months now. Tried PT and it helped but I hit a plateau and stopped getting better.

strengthen almost all deep muscles in my legs specially glute minimus and medius and deep abdominals. still working now. im on my 9th month but not 100% pain free.

@@paulmaldo8261 did you use core x? And what are your deep abdominal muscles? I notice you have core x in your playlists. I used the physical therapist exercises of the gluteus minimus to help my recovery so far. It's strange though because if I jog my pain starts to come back immediately

I've had this kind of pain for 20 years,I've been checked 6-8 different times and they kept saying it's your prostate,it's prostatitis,it's diverticulitis and then I went to physical therapy for 10 months and she said it was my psoas. I went back to the doctor and he said the hell it is?? I did some exercises for sports herniation and it start feeling better. 20 years of putting up with a bunch of morons with white coats!!!!

Topical comfrey and oral calendula and plantain supplements are supposedly good for increasing cell growth in such tissue. One natural hernia treatment website really advocated for comfrey for hernias.
