Video handwritten lecture discussing Erythema Multiforme, Steven Johnson Syndrome (SJS) and Toxic Epidermolysis Necrosis (TEN) for the USMLE Step 1 and Step 2. We will be discussing the pathology, pathophysiology, causes and etiology, signs and symptoms, lab findings and management of this skin disorder.
ERYTHEMA MULTIFORME
Erythema multiforme is the mildest form of the epidermal necrolysis condition. Erythema Multiforme has a very characteristic target lesion that is very similar to urticarial. It is differentiated from erythema multiforme by the fact that urticarial disappears after 24 hours whereas erythema multiforme doesn’t.
Causes and etiology - Herpes, mycoplasma, drugs allergies may imitate erythema multiforme
History - Generally it is not preceded by Prodrome such as cough or sore throat, but they may have fever with a history of prior attacks.
Examination - normally found on face and extremities. Also exhibits the Koebner phenomenon. Sometimes the central area can blister. Mucocutaneous lesion may also be seen.
Course - usually last 2 weeka nd then there is resolution without sequelae, however recurrence is common.
Management - hospitalize if there is impaired feeding, may develop to SJS, and if there are constitutional symptoms.
EPIDERMAL NECROLYSIS
STEVEN JOHNSON SYNDROME (SJS)
Defined as involvement of less than 10percent of body surface area.
TOXIC EPIDERMYLOSIS NECROLYSIS (TEN)
Defined as involvement of more than 30 percent of body surface area.
CAUSES - High risk drugs - Sulfonamide, anti-convulsants, allopurinol, NSAIDs, NSAIDs usually taken 8 weeks prior. Also viral infections, immunizations, mycoplasma, GVHD.
Clinical - skin peels off for 5-7 days and plateaus for days to week and this is when there is sepsis and failure. Starts with prodromal symptoms such as fever, headaches, rhinitis. Cutaneous begins with face to upper trunk and then to upper limbs. Lesions starts as erythematous macules that then become confluent. These patients are Nikolsky’s sign positive meaning skin will rub off with minor pressure. Mucocuteneous lesions consists of pain on swallowing, eye pain. Extracutaneous symptoms related to pulmonary system (ARDS, Bronchiectasis), Esophageal to colon necrosis causing diarrhea, melena, malabsorption. Kidneys, nails, vulva, vagina involvement may also be present.
Prognosis scoring system is known as the SCORTEN. Age greater than 40, heart rate greater than 120, presence of cancer, BSA greater than 10 percent, urea greater than 20mmol, HCO3 greater than 20mmol and glucose greater than 14mmol. If more than 5 than 90 percent mortality.
Management - Oxygen and ABG. Check glucose, urea, and HCO3. First begin by stopping the drug which is causing the allergy. Admit to ICU or Burns wards and give fluids and electrolytes. If infection than give anti-biotics. Mouth rinse with anti-septic wash every day. Give emollients, anti-biotics and vitamin A for 24 hours. Medications such as corticosteroids, IVIG, cyclosporine A, plasmaphoresis and hemodialsis

Lamictal caused SJS 8 months ago, I was having trouble swallowing, my eyes burnt and vision was blurry. The rash was there, and it hadn’t blistered yet. My husband talked me into going to the ER. I did, and had a fever of 104. They said it was acute bronchitis, pumped me full of antibiotics and sent me home. In less than 12 hours, we were on our way back, the skin was sticking to my clothes. My sheets had to be changed every round due to the skin peeling. My urethra swole shut, and I didn’t eat for 7 days, so, the other end didn’t do anything for even longer. I was told: you got here just in time, a few more hours and you wouldn’t be here.” I still deal with it. Peeling and dry skin, the sun hurts after too long and showers can hurt, too. So please, don’t mess with this. Get help immediately.

was diagnosed with TEN at 6 mths old i am 41 now and its amazing to me that 1. i am allergic to naproxin, 2. atrophic kidney stage 3 kd, type 1 diabetic 3/7/87 and 10/10 was diagnosed with ulcerative colitis but there were no granulomas so was changed to Crohn's. now in remission. its smazing watching this because i figured and my doctors that all the things i have been through were from my diabetes. Thank you

glad that your're ok Jennifer! just for knowledge purposes .. Crohn disease is the one associated with Non-caseating granulomas and lymphoid aggregates (mediated by Th 1 cells) take care!

I am glad they know this now. When I went through my struggle with TEN/SJS, God was my only savior. I was the show and tell patient at the hospital. The staff was great but they had to find a doctor that had witnessed this. I am a 14 yr survivor of this crap all due to an antibiotic. I only had it one day.

Nice write up. I was put on bactrim due to cellulitis. Anyway I had to go to emergency surgery to clean out wound and they put me on vanco. They upped my dosage on Bactrim from 2 a day to 4 due to me being a bit bigger 210. Anyway last Thursday Friday developed a fever and felt a bit weak. My testing heart rate jumped from 55 to about 80..as a result difficulty breathing. Saturday noticed a severe rash, stopped taking the bactrim. Went to the urgent care and dr. diagnosed me with SJS. He gave me a shot of a steroid..it’s helped with the discomfort and no longer have the or elevated heart rate. My question is does this always require hospitalization? If a appointment with another dr tomorrow. The rash covers a significant amount of body, especially upper torso. I’m a pretty healthy fit guy hopefully this helps in my recovery but I’m also worried to all hell.

TEN occurs in all age groups including neonates, SJS predominantly in children and adolescents.

Also in TEN prodromal symptoms are worse with painful skin amd severe mucocutaneous involvement . Another difference would be higher mortality.

My current symptoms are overgrowth nails and extreme sweating. My eyes also get very dry fast so I have eye drops.

I heard in the video you mentioned syphilis a lot. Does it matter if you born with syphilis from inheritance and I also was treated for it at young age so I have the antibodies not sure if I should tell my doctors about it.

Wow! I had TEN when I was 2 yr. old, in the year 1964. (Yep, I'm old, lol). All my hair fell out, and blood was oozing from my scalp (plus numerous other very unusual reactions). I have had cranio/facial hyperhydrosis (my head sweats like a faucet in the summer, or if I do ANYTHING that increases my heart rate). I wonder if it is because I had TEN?
