In this video, Dr. Robert W. Bastian explains three explanations for coughing in the context of Covid-19 infection, with emphasis on the potential for sensory neuropathic cough (Phase 3). The possible phases are:
1) During the “hurricaine” of the infection itself, when your lungs may be fighting active and as-yet undefeated virus.
2) During the “cleanup phase,” when the virus has been vanquished, but the lung debris (think: “downed branches and detached roof shingles”) is being cleaned up, even though “the sun is shining.”
3) For weeks to months after phases 1 and 2 are long past, coughing caused by damaged nerve endings that go “ZING!” with a tickling, burning, dry, or dripping sensation that initiates coughing, throat clearing, or laryngospasm.
This often-overlooked sensory neuropathic type of cough is explained in detail, including how to treat it.

In response to many good comments and questions: The KEY issue is to decide if your cough is still a Covid cough: EITHER due to active infection, or the “aftermath” of clearing away debris from active infection (That kind of cough might last some weeks, but should gradually improve over time). OR, is your cough instead sensory neuropathic in nature, and Covid-19 was simply the cause of the sensory nerve damage? If the latter, and if you now have sensory neuropathic cough (SNC), the cough will be ongoing, often without much sense of improvement over time. - - - How do you tell if you have SNC, initiated by Covid-19? For a start, see if you match to one of the syndromes below: - - - 1. Each episode of coughing, whether lasting seconds or minutes, begins with a sudden ZING sensation. (Examples: a sudden tickle, or abrupt “dry patch” that demands that you cough even though you aren’t sick or producing mucus commensurate with the magnitude (violence or duration) of your cough. Other sensations might be a sudden dripping sensation, pinprick, feeling like a sudden popcorn kernel, etc.) 2. Coughing is often spontaneous and unexplained, but if you do certain things, you are more likely to cough. Talking, a loud laugh, taking a deep breath, trying to sing in upper voice, posture change (especially getting into bed at night or rolling to a certain position), a very strong odor, etc. makes it more likely that you will cough. 3. Cough "frequency" and "severity" are out of proportion to any illness or any mucus produced. What mucus is produced may be the RESULT rather than the CAUSE of coughing. In other words, you begin to produce mucus only after hard coughing for 10 or more seconds (If I force myself to cough hard just as an experiment, I produce mucus too!). - - - What would a person do if he or she suspects they may now have SNC, with Covid-19 as the cause of the nerve damage? - - - • Go to the internet and enter the search term PeerJ Bastian to find a peer-reviewed but open source journal article that will describe SNC and how it is treated. Print and take that article to your doctor. • Go to www.laryngopedia.com and enter the search term “cough” to bring up abundant teaching material about SNC. • Ask your doctor: “Would it be a problem for me to just try amitriptyline or gabapentin (etc.)?” What do I hope you can avoid by doing these things? Being told “Oh, this is just how it goes with Covid-19,” when you actually now have SNC, which is treatable … • And if you strike out with your doctors, call nearby large ENT groups and ask “Does any of your doctors treat sensory neuropathic cough?” • Or engage a Laryngopedia teleconversation (laryngopedia@bastianvoice.com) Keep pushing the medical system to help you, because imprecisely treated SNC can be debilitating!

How come I never get a doctor like this? He seems knowledgeable, compassionate, and explains things in great detail. Every doctor I've ever seen spends only moments with me, has a somewhat sarcastic tone when answering questions, and seems to want to DEFEND his diagnoses rather than explain them. I've tried a dozen approaches, from stoic to extremely submissive to dominant, but nothing changes.

I feel your pain. I went to see a very expensive ENT specialist and the first thing he said to me was "Why are you so nervous?". I wasn't. He prescribed antidepressants for my persistent cough. Antidepressants. Yep.

​@@melinaesposito3434 Yes, the antidepressant may be what you need, just as described by the doctor in this video. Its a "neuropathic" cough, and so then the idea is to treat with neuralgia (nerve pain) medication. That is why one of the first medications the doctor mentioned in this video is Amytriptyline, which is known as an antidepressant, but is also used to treat nerve pain. The nerve "pain" may not really be painful, but itchy, tickly, or whatever the irritation is that causes the cough reflex. So, the ENT may have prescribed something along these lines for your cough. You should call and ask for more information because the ENT probably did not communicate clearly why he prescribed that drug, and you may have incorrectly assumed that he has prescribed it for anxiety. I got sick back in early January. I haven’t stopped coughing since. I’m constantly either clearing my throat or coughing. I was also diagnosed with Acid reflux. I finally went to see a pulmonologist. So far I’ve just been prescribed inhalers. So far nothing. It’s changed me so much both physically and mentally. I understand many of you. Hang on we can do this!

9308pink... I have severe acid reflux. When it comes up I feel like a dragon blowing smoke from my mouth. It also makes me cough.

Over a year of constant violent coughing, 6 specialist, multitude of scans scopes and X-rays and in 4 days your gabapentin dose recipe stopped the cough completely! You have no idea how bad it had gotten and how weak the body and mind got I am SO SO grateful for your video. It was so spot on. Thank you so much!

@@magentaorchids just like the dr ordered 1/300mg x 3 days (if still coughing) 2/300mg x 3 days (if still coughing) 3/300mg x 3 days (if still coughing) 4/300mg x3 days etc Cough stopped after 3/300mg day Than decreased dose after three days to 2 per day (for three days) and still no cough and then 1 per day etc Adjust the dose up/down as needed or even stop dose if cough doesn’t return Good luck!

@@vicviengales4037 requires a doctor’s order. Gabapentin is for nerve pain, like restless leg, shingles (neuralgia), or feet neuropathy. Good luck!

I'm a doctor and I consulted my professors for my chronic cough, I tried bringing up this diagnosis but they're adamant that I try therapy for asthma, allergy, and acid reflux and they never work. Thank you for your work Dr. Bastian, I'll return for a follow up using the articles that you provided. I'm lucky I can "self-medicate" with proper guidance.

@@yogprashantsingh1604with the treatments I previously attempted? No. With the treatment for SNC? It's too early to tell as it's only been a week. Also, I tweaked the dosage a little bit, gradually increasing the dose that's probably why I haven't seen a significant change.

@@yogprashantsingh1604 I wouldn't particularly describe it as having a "dry" cough, but for my case, due to the chronicity and severity of cough, it's like I always have mucus present in my nasopharynx. It's also an urge for me to cough in an attempt to clear my throat especially when the air is dry and I feel a tickle at the back of my throat that not coughing would just prolong the irritating sensation. It's pretty complicated at this point.

@@colettejacinthia9405 damn girl. I’m only 2 months out from my infection and the cough is already getting on my damn nerves. Breathing, eating, drinking, talking, laughing, all makes me cough pretty much non-stop. And my cough sounds like a dying animal, not normal at all. I also feel like my trachea squeezes tight after coughing, which scares me. I will try to ask my Dr. to refer me much sooner.

Same here except mine came a month after covid and every cough syrup or pills prescribed does not work! Oh god yes same here with morning and night. Soon as i get out the office, it becomes killer.

This so describes me! I’ve always been prone to coughs that linger for weeks . Now post COVID for 4 weeks, I’m still coughing and it’s so embarrassing. Everything you mentioned triggers coughing from mild to raging. I want it to stop. My doctor almost belittles me when I say I have been coughing so long. Thank you everyone for sharing your personal stories. I greatly appreciated cause I had covid 3 weeks ago on January 15th, 2022. Last week I took 3 tests on 3 different it all shows negative but I still coughing, shortness of breath & feel so weak. Before I could run a few miles & strong and ambition in everything in life but after covid my life change. I feel so related to each of you. Some day or a few hours you're ok & next few hours you feel weird. I pray for everyone including for myself.

@@jackelynem3303 I went to emergency because I feel my lungs were drowning, can't breath even I was sleeping with a sit-up position, non-stop coughing. I decided to go to emergency room & they did lots of blood tests & X-rays. Thank god/universe. All my blood tests came back good, no blood clogs, liver, kidneys, white blood cells & red blood cells are good. X-rays show clear (there are the best news I could get at this moments) but it's inflamed from covid & I got asthma from it. They prescribed steroid for me & discharged me but I feel extremely tired & weak after steroid wear down. 🙏🙏🙏 life & death is a very thin line. I really don't know what's my health future. I feel it effects my heart too.

@@jennydkwong you go Covid d January 4,2022 and tested negative January 11 and on January 12 started the same exact symptoms. I did a virtual visit and was prescribed méselo pack and z pack along with albuterol inhaler by day 3 I was feeling somewhat fine or normal and not getting as tired and could speak normal again. Day 5 the last day of steroid started off perfect and in the evening regressed back to day 1 with avénganse. I went to the hospital and was admitted in outpatient observation under steroids , nebulizar treatments , Levi ox and now even insulin ( mind u I’m not a diabetic and it affected me now). In the evening a nurse saw that I was still breathing heavy and struggling and started oxygen therapy 2ml ( o what an amazing difference) I was able to sleep, the tightness was gone and when the pulmonologist came in in the morning saw me so well he discharged me saying he thought I just needed more steroids ( sorry buddy but it was the extra oxygen) I was discharged home and advised not to do oxygen anymore because it could cause me more harm than good. Started next round of steroids nebulizar and symbacort inhaler and we are on almost a month later with symptoms still the same. Just saw an asthma specialist and treated differently but the treatment has its ups and down . Day of did great , day 2 was bad with shakes, weak and tired ( mind you this also affects they way I speak) and today day 3 still really tired and weak. My daily life and functions are almost non existing at this point

@@julissagalarza4052 im so sorry when I read about your experience. I can feel so related to you. They (from emergency room) gave me steroids too high that my lung specialist told me not to take it because it's really bad for me. Instead of taking 50 mg , he prescribed me 10 mg. I used to be so vibrant & full of energy that people thinks I had too much caffeine from my coffee, but it's just me. Now since the day I test positive until today, most days I struggled doing basic daily care, I'm lucky that I got help from my husband, he works from home but he's my care 24/7. Food & drinks to bed, I have to hold on the wall walking to the bathroom from my bed. I really hate this feeling but I know there's a light at the end of the tunnel, please be strong & be hopeful. One day at the times will restore our energy, strength & health 🙏🙏💝💝

After watching this video two weeks ago and following the advice, I got my doctor to prescribe Amitriptyline. After just 2 days symptoms reduced and now I'm almost completely healed after an additional week. Thank you so so much for sharing this information - God bless you

Finally an answer with possible solutions to my post Covid cough. More doctors should make themselves aware of SNC considering how many people I know who had Covid (workplace) and suffer from a cough they never had prior to catching Covid-19.

@thayalansuntharalingam I think it just went away but I did order cough drops from Amazon. They're called , Fisherman's friend. They're pretty strong mint flavor but they'll yet rid of a cough.

Definitely I’m going to share this my primary physician. I suffer from Long Covid. Many symptoms have hung on after being infected and thought it was over. Shortness of breath, palpitations, excessive fatigue, as well as what seems to be a never ending cough. 🤪

Thank You! I have suffered with SNC for well over a year now and was given fluticasone and Albuterol. The Doctor, who is a pulmonologist, did not tell me that it was related to my acid reflux and seasonal allergies. This is so refreshing! Thank YOU so Very Much!!!

Excellent resource. You are on to something. My cough continues at night and keeps me up until spasms stop. I then fall asleep from exhaustion.

@@TASIAawful1 It went away. I ended up with a leg clot and once I started the meds, 2 days later, it went away. I do not believe that I had clots in my lungs but weird how that worked out.

Thank you doctor🙏 from Nepal I have been trying to find out why my mom has been coughing from past 40 yrs. And now I believe she is suffering from SNC, as you mentioned one a remedy of using capcaisin spray, we cannot but it there so instead, she boiled chilli in water. And strained it and gargle as often as needed, she is finally feeling better. Not to mention, no medicine works for her cough and no doctor in my country knows what she is dealing with. I'm still coughing and it's annoying. Heading to month 2 in a few days and I am coughing like once an hour on average. Month 3 and I got my vaccine today. Still coughing and God is good. Stay safe my brothers and sisters. I noticed that I coughed 4 minutes after I ate. 30th September I only coughed twice , it's a new record

I had Covid variant in Dec, 2020. I coughed so hard, I tore my diaphragm. I got my first injection to reset my voice box. I’ll get a 2nd on the other side. For the first time in 3 years, I could talk about 15 min. Without a cough. 12 days and it’s beginning to wear off. I cough so hard, I’d throw up, feel faint from not able to get oxygen in. People run from me. Sleep? I also had to give up speaking and singing. Sweets and stress are my triggers. COPD from prior chemo. Gabapentin is my drug , it slows the frequency. The 300 mile drive is worth 10 days of reduced cough. If I didn’t get this cough gone, this IS NOT QUALITY OF LIFE. I did not want to live like this.

Thank you so much for posting this Dr. Bastian. I've been struggling with this cough, just as you described, for more than a month after my course of Covid illness. Because I'm fully vaccinated I had hoped I wouldn't have lingering symptoms. I have a primary care appointment in 2 days and thanks to this video I'm better prepared to discuss this problem with my doctor. I also understand I may need to seek help elsewhere, so I won't give up!

Took 200mg of gabapentin for 3 weeks, it did stop the coughing and gagging and the zing. But the coughing returned. So, I'm now following this Dr's advice.

Why isn't there doctors like you when I go to the hospital. Majority are young who are always joking/goofy or mid age who seem mad or frustrated and in a rush. They don't offer any solutions and 90% of the time pushes to a "specialist"... Maybe I need to go to Chicago.

I cough when something tickles my throat I cough when I swallow my saliva and it goes down the wrong way I cough when my heartburn/acid reflux/ indigestion comes back up And I cough when something gets stuck in my throat. None of these things I've mentioned mean I have covid or ever did.

There are many causes of cough. This video attempts to help people not overlook SNC. If there is not explanation for a tickle in the throat; if swallow evaluation proves that swallowing is normal (even though swallowing seems to sometimes trigger cough); if treatment for acid reflux makes no difference; etc....then SNC may need to be considered...just as a possibility.

So far one of the best discussions and addressing the post Covid bioweapon cough. I have had this now 3 times and the post cough takes longer and longer to get rid of. I have had none of the symptoms they take about when being infected and never had a fever… finally someone is talking real science and medicine!

It been a year since I got Covid. I had laryngitis for about a month. I still have a cough. Sometimes it feels like my larynx is contracting. Sometimes I get a small scratch in my throat or dryness and I go to coughing to the point that I'm gagging. It's a violet cough.... sometimes I have mucus, sometime times I don't. This SNC explains me to the T!

I have MS and my neurologist dx me with neurogenic cough. I don't know if SNC is the same or different, but one of your videos helped me tremendously. The video with the straw method to inhale actually helps relieve even the spasm feeling I get in my throat. I have had what you described as laryngospasm while drinking coffee. I didn't aspirate the coffee thankfully, because I couldn't even breathe in or out. When I could inhale it was tight and constricted. Gabapentin helped relieve it too, but I don't want to keep talking that. So tired of pharma. Thank you for your video on laryngospasm!

Big thanks for this Doctor. I had a really bad cough for a few days while infected with Covid that left me feeling like I'd been mugged. Worried when a week later ,even though it's no longer painful,I can't shake the cough. Got something to take to my own Doctor now,as soon as you pointed to "the spot", yep that's exactly it.

Wow that’s great information. My coughing attacks can last for 10-15 minutes. I get to the point of having difficulty finding a chance to take a breath of air in between coughs. My eyes water, my nose runs, I gag and mucus comes out eventually. The gagging is the most embarrassing because it sounds like I’m violently throwing up. I can’t go to work and I can’t go out to public places. My doctor just sent me for a chest X-ray and the results are still a day or two away. Nothing is fixing this problem and I’m exhausted. I always cough through the night and the cough suppressant isn’t fixing the coughing episodes. Even though I don’t yet have a cure, it brought me comfort just watching this video because your description of the violent type of coughing and how it feels is 100% accurate. Btw I did have Covid and since I recuperated, the coughing has been extreme. I will mention this to my doctor when he calls me in a day or two.

Same here. Ive been forced to end work from home so now im coughing all day in the office and on the train etc. Nothing the docs have prescribed me has worked. These include syrup, cough suppressants etc. I cant even sleep properly cos of this . I wish something would work 😭😭

@@nawabKhan-tv7vh I used to have dry cough like every day..every episode was a horrible experience ..no cough only itchiness and dry cough and howling..also there was always that little sticky white mucus like substance that always caused me cough real hard..!!! well it took me like 4 months to recover from dry cough ... Emptied 3 big honey and 4 small bottles... Btw if you looking to recover from that dry cough .. I have been taking "levocet M" and "mucolite" tablets...I used to take each one of it everyday but now I take those pills after every 5 days plus I get absolute no dry cough or ichy throat (I recommend u should try dry cough syrup that u prefer too) !! But tablets I mentioned help me for real big :) ! Also consult these pills to your doctor before any intake ! Plus don't eat or drink cold stuff and don't stay in cold weather or air ! Take hot steam every 2 -3 hours (as per u prefer) u can also try black pepper (little bit) + ginger + hit water tea every morning and evening ! Happy recovery 😊 may you recover soon and fast !

Yep, had all this for couple of years after whooping cough five years ago. Medical people, hospital staff were incredibly dismissive as were family when I spent MONTHS violently coughing. For at least two months i was coughing violently until blue in the face and vomiting and pissing my pants for up to two hours at a time, then half an hour of collapsing into complete exhaustion until the next fit of 2 hour coughing episodes, this was my life. I could not function. I now have permanent damage in my lungs and had major sinus surgery last year from disease that resulted from pertussis. Prior, I had no respiratory disturbances. I then spent the next year with terrifying paroxisms where my airway almost closed up, I was going through an inhaler every few days. It took 'them' years to find the source of my long term coughing fits because they looked everywhere BUT my lungs. I finally got onto the right inhaler. I still have a bit of trouble laughing really hard. Because of these airway disturbances, I simply CANNOT tolerate wearing a mask.

Oh my god where have you been man!!? I had COVID March last year and coughed ever since.. no help no explanation!! I was telling the doctors it feels like my larynx!! This is the only one I have seen so far that describes exactly what this lingering cough is like 😩 Having been vaccinated and my booster, I ended up testing positive for Covid the first week of this month. Out of nowhere I developed a cough followed with a horrendous sore throat, fever, and my sinuses were jammed. Felt like a bad cold really. This cough is driving me nuts. It may not sound like a lot, but I am going on a hair over three weeks. I have cancelled scheduled appointments with different doctors because I don’t want to put myself in a waiting room with other people with a mask on, and hack my brains out. I am respectful of others and even the receptionists all appreciated my actions. Anyway I have contacted my primary doctor to see if there is anything I can take for this cough. She told me they don’t typically prescribe anything because coughing was good as it will prevent pneumonia from setting in 😫 It’s disruptive period. This was very informative and will suggest it with my doctor. My patience is zero right now…….Thank you very much!

Never had cough from COVID was taking Ivermectin, but had stomach issues. All of family got it 71, 70, 55, 53, 25, 23, 21, 9 years old. All took Ivermectin and none to hospital. Confirmed tests.

I used IVM prophylactically in July 2021 and had no stomach issues. My wife and I got COVID this December and we took 15mg for 5 days, vitamin C D zinc NAC melatonin and no stomach issues. Our siblings used IVM as well. No stomach issues and best of all NO HOSPITALIZATION.

I was tested positive, i had symptoms like overfatigue, slight cough, muscle pain but no fever.. 15days of isolation and thank God the virus did not progress.. but after isolation i have this like a feather itching my throat, i cough but not a bad cough. It triggers then stop, then again.. Thanks for this video now i know what is happening to me, i was a little concerned that i still have the virus.

I am facing the sam issue right now. Tested covid positive 20 days back. Got all the symptoms except fever. But still getting Cough. None of the medicines seems to be working.

Thank you so much for this video. Now I know what this long standing cough is called. I had COVID twice and I am left with this cough. It is very aggravating and at night worse. Thank you again for addressing this subject.

This is what I have been experiencing, 100%. I've had Covid twice, both times was horribly sick with a tortuous cough/bronchitis/pneumonia. This was on top of already existing asthma cough. Thanks for giving me a direction.

I never had covid but I have this cough exactly as you explained.. my doctors were trying to treat me for ocpd.. lots of chest x-rays and pulmonary testing.. then I saw your video and I took it to the doctor's office.. where you explain the symptoms.. and I told them that's me that's exactly what I have.. so they prescribe me your first medication on your list.. I'm happy to say five days later I am about 80% better.. such a great relief thank you doctor..

I’m curious to know why DM wasn’t mentioned as a med option. Is it not effective for this type of cough? Anti depressants and anti convulsants are scary!

As an Update: I actually took minimal doses of a cough suppressant (DM) for about 7-10 days and I experienced enough time not coughing, that the coughing pattern finally stopped.

@@sparth1989 Yes, the DM worked to stop the cycles of spasming and after 1.5-2 weeks it was completely gone.

You are right about Gabapentin! Mom takes it for myelopathy symptoms occasionally but when she does, she does not have the lingering post covid cough nor the mucus!
