➡️ Subcutaneous Emphysema
Subcutaneous emphysema, a medical condition characterized by the presence of air or gas in the subcutaneous layer of the skin, is often a sign of an underlying health issue. This condition is most commonly associated with trauma or injury to the lungs or airways, but it can also arise from infections or surgical procedures.
➡️ Pathophysiology
The pathophysiology of subcutaneous emphysema involves the introduction of air into the subcutaneous tissue. This can occur when there is a rupture in the respiratory system or infection by gas-producing bacteria.
➡️ Causes
Common causes include chest trauma, like a rib fracture, which can puncture the lung, allowing air to escape into the chest cavity and subsequently into the subcutaneous tissue. Lung diseases such as chronic obstructive pulmonary disease (COPD) or asthma may also lead to the development of subcutaneous emphysema, as can medical procedures like mechanical ventilation or surgery involving the chest or neck.
➡️ Signs
Patients with subcutaneous emphysema typically present with swelling and a distinct crackling sensation when the affected area is palpated, known as crepitus. This sensation results from the gas bubbles moving within the tissue. Other symptoms may include pain, discomfort, and, in severe cases, changes in the voice or difficulty breathing if the air compresses the throat or lungs.
➡️ Diagnosis
The diagnosis of subcutaneous emphysema is primarily clinical, based on the presentation of symptoms and physical examination. Imaging tests such as X-rays or CT scans can confirm the presence of air in the subcutaneous tissues and help identify the underlying cause. These imaging studies are crucial in assessing the extent of the condition and in planning the appropriate treatment.
➡️ Treatment
Treatment for subcutaneous emphysema depends on the severity and underlying cause. In mild cases, the condition may resolve on its own without intervention. However, in more severe cases, especially those associated with trauma or lung diseases, treatment is directed toward the underlying cause. This may involve repairing any lung or airway injuries, managing infections, or adjusting settings in mechanically ventilated patients to prevent further air leakage. Pain management and monitoring for potential complications are also important aspects of care.
In rare cases, subcutaneous emphysema can lead to complications such as tension pneumothorax, a life-threatening condition where air becomes trapped in the chest cavity, or mediastinal emphysema, where air accumulates around the heart and major blood vessels. These complications require immediate medical intervention. Subcutaneous emphysema is a condition characterized by the presence of air in the subcutaneous tissue, often indicative of an underlying medical issue.

I developed this when I was delivering my first baby- right in the middle of pushing- I did get a chest X-ray immediately fellowing the birth , but nothing was found. I crackled for a while, but it just went away on its own. I didn’t realize it could be serious 😵‍💫
