Really good informative video. I lost some of my hearing and my GP told me to wait a few days, by the time i got to see the ENT department it was too late to take steriods

Several months ago I was hit with SNHL to my right ear. I was tested at 90db loss and 4% speech recognition. I was given two shots of Prednisone over a two week period. My hearing came back to normal. I give most of the credit for improvement to using CIST (constrained induced sound therapy). UM was not aware of CIST. I think there is a lot to be learned from CIST.

@AMWF I'm highly confident the cause was sound exposure.

My definition of normal is: being able to hear the same as I have my whole life. I'm a musician with excellent tone and volume sensitivity.

Hello Dr. Hofmann. Thank you for this very informative review of hearing problems and SHL. Nice to see an IU alum. I grew up in Bloomington, IN and graduated from IU as well! I am a healthy 49 year old man with no known health problems, surgeries etc.... Anyway, last November 2012 I came back from a driving trip from San Diego to San Francisco with the family. We went through mountains driving there and of course SF is very hilly. Shortly after arriving back I got this really clogged feeling in my left ear like I was wearing ear plugs or I was underwater sensation. If I remember correctly I think I might have had sinus/allergies or an upper respiratory infection of some sort but I an remember that for sure. I know for certain that it sounded like water in my ears for when I would tilt my head in various different directions I could clearly hear fluid sloshing around in there. I had no pain ever and no problem with my right ear. I experienced some tinnitus during this episode from time to time. I went to my primary care doctor and they did not do much for me. I insisted that they give me amoxicillian augmentin which they did but that did not help. My ears finally cleared up about 31/2 to exactly one month later. I finally got my referral to an ENT. He said I did not have fluid in my ears at that time of seeing him. That seemed to make sense as I know no longer felt any clogged feeling in my ear at all. Hearing was much improved and seemed back to normal. I was thinking that perhaps I might have had ottis media with effusion or ETD or both. Anyway, the audiologist at the ENT office did an audiogram and it showed mild to moderate slope of hearing loss in the 1500 to 3000 Hz range returning to normal at 4000Hz. I had a little bit of tinnitus but it came and went. Over the past year I did not notice any tinnitus but within the last month and a half or so I hear high pitched tinnitus 24/7. I have been going through a lot of stress and anxiety and had a panic attack in which I was rushed to the ER. This all happened about 3 months before the return of the tinnitus. The big question I have is that do you think that I might have had sudden hearing loss and that if I got to an ER room or ENT within a week or less and was put on steroids that I might have saved the hearing in my left ear? This is really upsetting me right now so much given that I am thinking that I could have done something to help myself more. I wish my primary care doctor would have been more alert to helping me. Please, please let me know if you think that I might have had SHL with the condition and symptoms I described above. I really appreciate your presentation done is a caring, relaxed manner with humor and all. It was a pleasure to watch and was the only thing I watched that helped me to try and understand what is going on and not be upset or nervous about it all. Thank you and All the best. Go Hoosiers!

Last week I woke up with a hearing loss (right side) of -60dB. It scared me like hell. No ear wax. I have a history of middle ear infection. My otolaryngologist suspects neuritis vestibularis (nerve infection), for which he prescribes Medrol (prednisolone), a cortisone. Six days later on there is no improvement, this Medrol has no effect. I suspect there is something else going on. I think it could be a disruption of the ossicular chain. The sound simply does not make it to the inner ear. Very loud sounds I can hear but they are heavily distorted, like rattling and rumbling. I also experience tinnitus, this is a constant background noise along with short funny "plinking" sounds like from a tone generator at different frequencies. Unfortunately, I have to wait for 6 weeks to get a MRI scan that may bring some answers. I hope my hearing can be restored, as it is driving me crazy and it makes me depressed.

@@FishingWithYon About six weeks later, the symptoms have not improved. Still a serious hearing loss and a nasty resonance on certain frequencies. Tinnitus is softer now, like the hissing of an air leak in the background. Meanwhile, I have had two cortisone infiltrations in the middle ear, through the ear drum. (Kenacort-A 10mg/ml.) The procedure is done under local anaesthesia. It is uncomfortable, but not really painful. I have my doubts about the diagnosis of neuritis vestibularis. I suspect it may be something different, maybe one of the ossicles in the middle ear is broken or obstructed by bone growth (sclerosis), hence the resonance, like the sound does not make it from the ear drum to the cochlea. I hope the MRI scan will bring the answer.

I am going through the same thing. Had a hearing test, MRI, will have my second steroid ear injection tomorrow. This is affecting both of my ears. Praying I get my hearing back. I think this is a side effect from the Moderna covid shot. I had a bad reaction after the second shot. Wound up in the emergency room having trouble breathing.

This happened to me. Unfortunately I saw an Urgent Care Nurse Practitioner who thought that I was unable to hear due to a swollen ear canal. Had she performed the tuning fork tests maybe I could've recovered fully. I had partial recovery and eventually got corticosteroid and HBOT treatment but it was too late and they didn't really help. If this happens to you I would definitely go out of your way to see an ENT. Call them and explain the situation sometimes they'll be willing to see you same day as this is an emergency.

Thank you Dr. HOFFMANN. It happened to me 2 days ago. Sudden loss in left ear. No hearing. Rt ear normal for 82 years old man. Drums are ok. All done in ENT urgent care facility. On steroid treatment for 2 days. No improvement. Your explanation was very helpful. Will see the ENT DOC in 7 days. He is too busy. If steroids don't help, then it may be inner ear. Again thanks.

I have a question. I had sudden sensorineural hearing loss in both ears following preventative antibiotics for Lyme disease (Doxycycline). I took my meds, got sick from them, and then woke up with hearing loss severe enough that I was unable to tell where my alarm was. I still have severe tinnitus loud enough that it is almost physically painful, and hearing loss to the point where I can't even listen to music anymore which, as an aspiring musician, is career ending. I saw a PCP about it the day after it happened, but rather than treating the condition, they sent me to an audiologist to get a hearing test. They didn't treat me for anything before the test, saying they wanted to get the test done first. After I had finally had the test done, I still wasn't given treatment. I did my own research, and after 30 days of the initial hearing loss, I decided that I was going to go to urgent care and have them give me steroid treatment since my primary care office has refused to do so, and now due to their inaction, the damage is likely permanent. Treatment has not gone well, and very little of my hearing has returned. My dream of being a musician is over, after spending the past few years devoting myself to learning and creating music. My only source of happiness is gone, and every day feels like torture. My question is: Is my primary care office responsible for the permanent damage to my hearing because of their refusal to give me treatment for something so time sensitive, especially knowing that I was an aspiring musician?

Im so sorry that happened to you and you didn't get proper treatment 😔I've lost hearing in my left year and receive steroids after only 15 hours but they didn't do anything unfortunately. I had 23 treatments in hyperbaric chamber because it's another step you can do to treat hearing loss ( and it helps to a lot of people ) it restore a little bit of hearing in my ear , but it's so little that I hear only tinnitus...it's depressing to hear all that noise all the time 😢😢now I'm waiting to chochlear transplant ...I wish you best of luck and please do not lose your hope, I say that but I know how hard it is to find some joy..

Dr. Hoffman...after the 30 db question in this video at 30:44 I am hearing a large volume drop off in your voice, sounds echoey, and I hear a large fan noise competing with your voice. This continues until the end of the video. Prior to 30:44, the audio is "normal" sounding to me but muffled. What is happening?

Ahmed - you'll need an assessment from a trained audiologist or otolaryngologist to determine the cause and advise you on your options. A good place to start is with your primary care provider. They can provide an initial assessment and suggest a best course of action. We wish you the best possible outcome.
