It really should be in there. At the time it was omitted because in many places in Africa (where we practice) the bougie was not available. Maybe it is time for version 2!

These procedures need to be standardised. Every university has their own method and procedure. it is honestly very annoying.

How far to you advance the tube? Should we just continuously auscultate to ensure adequate air entry bilaterally, or is there a specific landmark that can be used? Secondly, what would be best to secure the tubing?
