I've learned as a new GN and new NICU nurse that the NICU world is like none other in the hospital! So very much to learn and understand.

I've been a consistent viewer of yours for years. Im a NICU nurse and I watch your videos in the morning when Im getting ready for work to get my brain going. Im currently training for the resus team which is really scary to me but I find if I understand why we are doing things I feel a lot better. Seriously I can't thank you enough, you have helped me build my confidence and knowledge!!

I’ve been a labor and delivery nurse for over 10 years and have never had anyone explain it as simple and understandable as you did! Now I truly understand how to interpret blood gases instead of trying to just remember numbers. Thank you so much! Total game changer!

I’ve been a midwife for 8 years and every now and then I try to refresh my knowledge on this and it’s always just as confusing every time lol. This presentation absolutely made it so much clearer! I loved the end explaining the association of fetal blood. Ive watched a few videos tonight and I’ve truly learned things in a whole new way.

❤ I’m looking for help! I was an er nurse for 6 years. I left and went to nicu level 3. But for level one and two I didn’t get a good preceptor. I don’t know to manage time. I don’t feel like I know what I’m doing. She didn’t show me a lot and she didn’t give me any tips cause I had experience as a nurse. :( If anyone cause help!? I want to leave the unit because I’m struggling so much with time and … everything. Last shift I didn’t get to eat. ❤ please help. I don’t want to be negative and I’m not get much help on the unit. So I’m trying to go on my own and become better. Thank you for ANY HELP ❤❤❤

Hi Rose! I'm SO sorry you're feeling like this. First- the fact that you care and that you want to be more helpful is an excellent sign! Second- having worked for 6 years in the ER, you are used to the routines and all the different procedures- so it's really hard starting at the bottom again. You're used to feeling lie you now what you're doing. Keep going, and asking questions, and offering help, and reading up, and being a positive force in the unit. Don't give up! They will be Lucy to have you :)

Hello! Obviously we've made decisions for infants to be given a tracheostomy (whether from CLD or airway issues) and we've been involved in the care- but neonatologists are not really skilled at trach. care! Would you still be interested in a more "this is when you need one" type video?

My 25 weeker now 40 weeks still on cpap . Her blood gas recently was 66 . She doesn’t look she’s struggling with her breathing…. She have horrible reflux . I am so confused

Hi Dr Tala. How do you manage babies who have metabolic acidosis but with hypernatremia >150meqs serum Na? Do you still give sodium bicarbonate? Thank you.

Hello! You bring up a really interesting topic- usage of bicarbonate. During fellowship, if we had a metabolic acidosis with high Na, we'd use THAM, a non-sodium base to help correct the acidosis. Then in late 2000s, more data started coming out that bicarbonate probably doesn't do anything except make us feel better about the numbers (and maybe make the cells more acidotic). So I pretty much stopped using bicarbonate, and instead concentrated more on correcting the underlying cause. The contractility of a NORMAL heart isn't really affected until the pH reaches approx. 6.8, so maybe at that point, you'd consider it. (i.e., give it if infant coding). Cardiologists dealing with an abnormal heart (e.g. a HLHS etc) ask that we use bicarb in these infants because it's such a different population. So my answer would be don't use it, or use anything very sparingly. I'm sure others would tell you differently- so listen and read as much as you can- as you seem to be doing :)

We're so so happy you feel inspired to read- and thank you for the lovely compliment, but I'm sure, as you're probably realizing, the physician is just a small part of the puzzle. It's the entire team- from whoever trains you, to all the people you work with-other doctors, NNPS, nurses, RTs, the nutritionists, ST/OT etc. etc. that determines if a baby is getting the best care or not. Your enthusiasm is infectious though!!! Thank you!
