Hello. I am very much a beginner at the age of 66 and I experience pain and discomfort on my left (fretting) hand, particularly in my forearm and wrist. I suspect it may be due to insufficient flexibility in the wrist. I stretch before practicing and keep practice sessions short. This is stopping me from progressing and is very frustrating. Have you any suggestions please? Regards Andrew

Been professional for 35 years, never had a proper job, just played guitar with a lot of bands, and some big tours, albums, TV sessions, all kinds. The tendonitis tarted about 15 years ago and I tried to ignore it in various ways including having a drink to relax! However, it chose not to ignore me. Eventually I stopped playing just over a year ago, I couldn't even hold a pic. Just started practising again after 12 months having left the UK for France. I need your help Bernth, thanks for this video, it will support me. The thing is, when it was getting worse, I just strained harder to do my gigs, which made me more anxious, which made me drink more, which made me strain harder etc. Nuts. You give good advice Bernth and you're obviously a great player. I agree, it is about relaxing, totally. Thanks Keith Airey.

In 2011 I got tendonitis. It just ruined me, I didn't know what to do. Had been used to doing 4-6 hours of guitar a day. I worked up to it very gradually but pushed it too hard before jazz fest for school. I was doing classical for school, jazz for school, and played in a cover band and original band. It was just too much and when it happened I didn't know how to stop playing, it was like an addiction. The tendonitis forced me to eventually and then I got depressed and didn't practice as much. Did physical therapy, took time off, still hurts to this day but it's much better than when I was younger. Had to stop focusing on certain things which is a bummer but am human and have limitations. Great topic

Thanks very much for the information and advice Bernth! I’m 62, and have been playing for 8 months (after a couple 10-year layoffs). About 3 months ago, I developed severe pain in my left wrist: specifically, the thumb tendon, where the thumb meets the wrist joint. I completely rested for 3 weeks (no picking up the guitar at all). When I resumed, it was for only 15 minutes per day--just to remember scale patterns, etc. (prior, I was practicing 3.5 hrs per day….trying to make up for lost time, I guess!) I also started with physical therapy with a hand spcialist. Right now, I’m in a cast that keeps my thumb and wrist immobile. It has Velcro straps, so I can remove it to practice. Even when practicing, though, I keep the wrist braced, and warm up for at least 20 minutes (both with wrist manipulation and guitar light exercises). I also warm down after each session. I came to realize that I brought on the condition by my improper technique. I was using WAY too much force with my fretting fingers, and I was letting my left thumb get parallel with then neck (rather than keeping it behind the index or middle fretting fingers), and bending my wrist way too much. Now, that is my focus during practices, and I’m back up to 45 minutes per day (three 15-min sessions), with NO pain throughout. I have about a month of therapy left, and I’m getting through most days with no thumb/wrist pain when not practicing now. My therapist has often said that tendons are VERY slow to heal. My advice (of course, this is coming from a novice!!) is to be patient, apply Bernth’s principles (they’ve really helped me so far) and don’t give up! Best of luck!

I'm sure many of you can relate, this is very devastating for someone who's passionate about their instrument, I began playing guitar when I was 13, I would play for hours a day with no problem, then when I was 17 I took the antibiotic called Cipro, which caused drug induced tendonitis and it's completely ruined my ability to form chords and bar chords for a extended period of time, I'm 28 now and it's never been the same, I started getting serious about bass guitar after 17 and now's it's my main instrument, I love it but there is a soreness I feel about thinking of my dream to play guitar, for some reason the linier individual note fingerings used by bass players doesn't hurt my hand like the way guitar chords do and I can play bass for hours, but guitar is too painful, sort of feels like my fate was to become a bassist instead..

I have read stuff about that, and some of the side-effects after taking certain medications could make things worse for us... I apologize to hear you’re a fellow guitarist and music enthusiast struggling with the same thing as many of us here. (Personally) It must be improper posture and playing form, for me, as a self taught player and using too much force and attack, as he mentioned in the video. I know I never had all the correct knowledge, I was too stubborn to pay for lessons when I was between jobs a lot when I was younger and I should have been there for myself better. My fretting hand is the main one tho… it kinda feels sharp and limited in range of motion to hold a cup, or to open a door with a round knob, especially doors considered “heavy” by their intense hinged mechanisms, Anyways, it isn’t my picking hand. It seems to be common from what I’m seeing. As a righty that makes it the left hand. Some medication can also cause tendons and potentially nerves to not heal sufficiently as before, only restoring much of the potential muscle and strength up to about 40-60% of previous strength as previously. I would recommend also checking out what certain antibiotics might do in relativity to this condition, but some of it can be treated and healed with the right gentle massages and exercises that aren’t painful for getting the “shorter” under-used tendons to regain some of their strength and balance. Eventually he is right I keep seeing the wiser advice say that it will go away… potentially not always permanently though. That makes me really regret not paying attention to the “little” but most biggest things of all, our posture, and not resting enough because I was too excited to land the right technical melody of a song I like or learn a solo in “one sitting” playing it suddenly for an entire 10-12 hours on a weekend… that’s not good at all.. I feel so unintelligent for this. And some pain medications out there will also will do the adverse effects. (Look up the specific ones). I think some that are the worst are corticosteroid injections, please try to not accept that treatment, I’m not liking the things I have heard about them, or any heavy pain relievers that give lethargic and sickly symptoms. You will feel ok again temporarily from most any pain meds, but then the symptoms grow from permanent tissue scarring and loss of function over repetitive wear and tear if you continue to play a bunch. I’m not trying to say this is the proper advice, situations are unique and all very specific. My chiropractor is trying to tell me to check into what type of arthritis it is from a whole other neurologist based doctor, and I just now finally only attained a primary care doc. I skipped out on health insurance too many times and let this build up. Don’t delay it like I did! I’ll just leave on this note that I hope you take time to be patient and kind to yourself! You are important too! All caring Musicians should know. And musicians (together) united we will grow. Unlike the rest of the web, I don’t believe in “advertising” or “selling” or withholding information about the discussions that are forgotten and not talked about. I disagree with my chiropractor that it is arthritis (osteoarthritis or chronic generalized, or even rheumatoid arthritis). Because I’m still 28, I know I’ve had enough time to abuse my body but it’s wearing down rapidly and unexpectedly, I “appear” healthy but maybe I have a whole other issue, I won’t delve into it all. Autoimmune disorders or disease can also really destroy a body, even chronic stress triggers inflammatory responses. (Sifting through it all really sucks, even reading this might suck and I’m truly sorry). I just want to help if I expect that anybody will read this. But as before, take it with a grain of salt because I accept that I don’t have all the answers myself. Take it easy!

I’ve unfortunately experienced this issue in my wrist. Once you feel pain due to tendonitis it’s often too late to just stop and rest. It’s usually due to tightness in the forearm not bad technique. Make sure to stretch and massage your forearms to keep them relaxed. If massage doesn’t give you some level of discomfort then you aren’t doing it hard enough,dig in!

That’s where a lot of my cramps immediately stem from, but hurts to hold cups for example, extended finger gripping postures, or bending my hand “up” when it’s vertical compared to the floor Please try it to ease back from being too excessive you might rupture something or cause it to bleed internally. Those little bumps he mentioned aren’t a joke, I had those appear on my fretting elbows. But stretching and strengthening are also really good points! I’m not trying to tell you what to do, you know your body better than anybody. Just listen to your gut feeling. Just be mindful if you go a little rough

Just realised the extra recent comments on here. Seriously guys, try collagen - it's about the best thing for tendonitis. It works in two ways: 1. pain-relief, almost instantaneous, and 2. healing; strengthening the collagen-matrix in your tendons. Collagen makes up most of the tendons and is hard to get in any decent amount from diet without supplementing it. Combine that with massage which signals anti-inflammatory cytokines and is also great for pain-relief. Up to 60, 70, possibly 90g a day during the healing process, and in 15g doses. 15 is optimal as a big recent study found it gives a bigger boost to the amino acids that help you heal and become stronger. The same study also showed a chart where those levels quickly rise and then reach the peak 1 hour after ingestion from normal levels, and gradually reduce after a further hour. So that 1 hour peak is when you should massage, and hit the stretching especially (if you're at that stage). Then when you're at a stage where stretching doesn't aggravate it, stretch for 5-10 mins a day, full-range of motion stretches. This will strengthen the tendons significantly beyond what they would normally otherwise be. And it's cumulative - after one year of stretching and combining with collagen (which gives a synergistic double-effect), you'll be MUCH stronger for it. I can do a lot more now than I ever could even before my 2-year long severe tendon injury that I thought I'd end up having surgery on.

It was like that for me, I feel ya. I thought I wasn't going to make it through that period and have my hand come out intact but now it's better than even before. Remember to massage as well and stretch lightly for a short time every day until they get stronger and you can do fuller stretches (could take a few weeks for the initial strengthening). I wrote a few replies in more detail to some reddit posts people made if you want to check them out: www.reddit.com/user/Lightning_Strat/ I think there's a lot of hope and prospect for recovery, just keep at it and doing the right things, you should get there. Good luck and it's always interesting to see how people are progressing in recovery so I can pass it on to others so I'd be happy to hear how you're getting on

And just a quick side note, it's collagen type I that is more for the tendons than the other types you can get (sometimes they are mixed, but type II and type III themselves are also common on the market just to put that out there)

How are you doing now? I felt the same and couldn't go half an hour of playing before the pain came back, but now a year later I can do 7/8 hours in a day easy, no problems, with plenty of speed work in there. Two things really worked for me: collagen (take in 15g doses and with vitamin C foods), and stretching which should be done 1 hour after you take the dose of collagen as that helps build the tendons even stronger. Go easy on the stretches if there's still pain. After about a year of doing this all pain subsided and I built the strength needed through stretching to go as long as I need. Be patient - it paid off in the end, and this is coming from a severe case of it where I thought it was all over. Good luck bro.

Quodge: I´ve heard that many (many) famous guitar players had/have tendinitis. Kiko Loureiro (Megadeth) told in his channel that he´s with that problem. As we love guitar, we must do everything to get rid of tendinitis: stretches, nerve (radial, ulnar, median, carpathian) glide, accupressure, accupunture, fisio-therapy, fisio-taping, REST, ICE, HEAT, praying (it works for me!), being happy, being possitive, meditation, work with elastic bands, thera-bar, Graston technique, Cyriax technique, PATIENCE, MASSAGES, WARMING UP, etc.... Consider that when elbow hurts (example) the cause is frecuently in other part: wrist, shoulder, back, etc... One thing. Don´t do it all at once: slow, continous recovering (time heals). Cheers!
@LightningStrat
@LightningStrat 5 жыл бұрын
That's interesting... didn't know about Kiko. I know John Petrucci had it in his early days and let it rest and didn't have a problem since after he started stretching. It's the same for me - rest, collagen and stretching will be all most people need. I think the ice and heat thing has been a bit overstated generally, and if I recall that part of the RICE (rest, ice, compression, elevation) mnemonic has been withdrawn, at least for after the initial 24hrs. Do you know of any other names who had this issue? To also add to the list: Larry Carlton, Adrian Vandenberg, Troy Stetina (who swears by glucosamine - I tried it but didn't get much passed the loading phase and collagen was giving the relief I needed) had it. There was also a suggestion that Yngwie and Ritchie Blackmore dealt with it from a guy who allegedly asked Yngwie at a clinic, but I haven't heard anything else on that. I'm still doing good myself but I start to notice it creeping in at low levels when I miss days of stretching and collagen. Pretty crazy what we put our hands through when you think about it - and I don't think a professional athlete training for hours a day would go without stretching, so neither should we!

@@LightningStrat : I know Jimmy Page had a severe tendonitis problem that affected his playing around 1977, and it was because he used his Les Paul so low. Demonaz from Immortal also had the issue for 8 years aprox. Gil Moore, drummer and singer of canadian band Thriumph, had a one year calendar tennis elbow problem. That year the band hired a temporary drummer, while Gil sang with a bandage in his arm. After that, Moore named his drums solo in their live album "Stages" as: "Druh Mer Selbo".

Currently affected, but I fear it's probably a carpal tunnel syndrome in my case. Had to visit a hospital on Friday night because the pain became really unbearable. Felt like having my hand ripped off or being crucified. I've made an appointment for coming Tuesday with a specialist last week before it got so much worse already, and I put down the guitar as soon as I noticed the uncomfortable sensations in my hand and wrist. But I've made the mistake not to call in sick for work, still spent 8 hours at my desk as a software dev, and still spent a lot of my free time in front of the computer as well. I should have taken this much more serious when it first occured, and stop all this activity. Now I'm sitting here typing with just one hand, adapting my whole life to just being able to use one hand for a while, and coping with nasty opioid painkillers that get the pain and discomfort to a bearable level at least, but also make me feel heated up and nauseous a lot. Don't be like me, take it serious even at the slightest level of discomfort or pain, your body is possibly begging you for a break. We only get to be born with one set of hands, and they're such complicated and fragile tools, so treat them like the gift they are.

Had my appointment at the orthopaedist on Tuesday last week and got an orthosis prescribed for my hand/wrist which I have worn almost 24/7 since then. I'm off sick from work until next Monday since then as well. Also had an appointment at the neurologist two days later (thank god I didn't have to wait weeks for it) for a nerve conduction velocity measurement, I will know the results on my next appointment on Monday. All hints point towards a temporary inflammation / entrapment of my ulnar nerve (aka "funny bone") in my elbow, which I bumped fairly heavy some time before the issues began. Things are finally getting better day by day, and I started doing some (yet very careful) stretch and gripping exercises yesterday to hinder any kind of atrophy in my hand. I'm taking 400mg of magnesium and 3x100mg vitamin B6 every day for over a week now, too, as it's supposed to help with neurogenesis. I was also able to stop the opioids since I have the orthosis; was already worried about building up a dependency. Today is the first day I could dare to handle a guitar again, and I'm really happy about it. From now on I will be extra careful, and always do sufficient stretches and warm ups, and take injuries serious from the beginning. I also want to start exercising at least a little bit soon, maybe some yoga and pilates to begin with. Will probably help my body and mind a lot. So yeah, what do I say... be careful with your elbows as well! The human body is such a weird complex apparatus.

Word! Great content. Words of wisdom about prevention and listening to your body. I suffered through this over 10 years ago. After a lesson from a classical player about correcting my posture/mechanics, dropping a string gauge, I recovered in a few months. I recently got back into fingerpicking and turned a corner with my playing - only to develop Tendonitis in the same spot in the knuckle of my index finger fretting hand. I don't think I'm doomed but feeling foolish for letting this happen again. Listen to your body and be careful about long repetitious practicing!

I'm currently struggling with tendonitis which is leaving me with a pins and needles sensation in my hand and a very sore forearm. This has resulted in a huge reduction in my practice of scales. Bernth's description of how this occurs is exactly what happened to me. I began to practice a lot more during the first lock down in 2020 and I still struggle now! I have seen a physio who gave me stretching exercises and this helped considerably, however I foolishly began playing too soon and too often resulting in another flare up. Listen to your body, practice technique not just speed. I have found that I can play chord progressions without too much trouble so at least I can get my guitar fix, I've rediscovered the acoustic guitar and that has been a comfort. If you are struggling with tendonitis and have not seen a doctor or gotten advice I would recommended it. This was a great upload. 👍👍

I had it badly in my fretting hand's wrist several years ago and it was crippling. I wore wrist wraps which helped, but it took around 15 minutes to warm up and sounded embarrassingly horrible during that. After that initial warming up for that time, it felt like my wrist bone would snap back into place and with the wraps, I was ready to play, but not to my best ability. I eventually saw a specialist, who actually played guitar also, and had a cortisone shot. It has lasted for around 4 years or so. However, I've been feeling some pain in my thumb again, nowhere near what my wrist felt like, but am getting worried again. I've gotten a pretty big gig, and I have to be in fighting shape before the end of July to record the album, then going on tour with Exodus. I may have to see the specialist again, but my dream is finally starting to come to fruition, and now this. Depressing doesn't begin to explain. I refuse to lose my spot though, waited way to long for this. Scared!

@Sleeping Ronald good actually. Every once in a while I get some pain in my wrist, or base of my thumb, but with certain stretches and exercises, including staying hydrated, I'm mostly pain free. Was totally pain free for a few years after the shot. I might get another one before touring next year to be safe, but I can't complain. Those new grip wrist wraps are irreplaceable. I wear them still. It helps so much with finger independence and strength, and will allow you to play mostly pain free, depending on each different person's situation.

I have been exercising my fingers for more than 3 hours for 03 years. Now when I use the second and third fingers of my left hand to play notes, it feels like an electric shock. Because of this, it is difficult for me to play the guitar now, and it also creates mental pressure. Saw this video at the right time. Thank you🙂

That sounds more like carpal tunnel. The affected nerve on the wrist is the reason for the electric shock feeling.

Well, I am late to the party on this, but I have been struck with tendonitis in both my index finger and right thumb--a combination of too much touch screen usuage (phone, KIndle) and guitar. In the United States, tendonitis can be treated by an Occupational Therapist (who often can be found at a Physical Therapist's office). You may need a prescription to see one (such is the case in Michigan), so that is a bother. My son (non guitar player) hurt his wrist at the gym and saw an OT for that. Turns out his finger strength was acutally quite low for someone his age. Working on strengthening the fingers (the muscles that control them anyway), helped a lot with the pain, so I am starting to do the exercises he did.

I am a very beginner at 61 and now have the awesome pain in my thumb. Playing guitar is the main driving force behind the cause, but also maneuvering my smart phone all day with left hand. Grabbing the guitar with left hand is another no-no. So, what I should do is dip my left hand in concrete and never use it again for 6 months, just kidding of course, but the general idea is pay more attention to everything you do with the left hand.

I got a tennis elbow and a small rupture in the tendon 13 months ago to the fretting hand. For the 9 first months I couldn't play at all. One doctor suggested me to try NSD powerball it has helped me to start playing againg. Now I have been able to play quite a lot for the last 3 months. Other things that I have to do in order to be able to play is to use a tennis elbow support (not one of those large over the elbow ones but one that does not stretch) and to use a whammy bar for ALL vibratos and also to help bends with the whammy bar as often as possible. Vibratos will quickly destroy my elobw and I don't think I will be ever able to do them with out the whammy bar but other than that, the elbow is not a major problem anymore. I just can't play acoustic guitars anymore and I'm forced to buy somekind of a shredding machine that will stay in tune with extensive whammy use. One more thing is to use as light strings as possible (based on my experience NYXL 008-038 is the only set worth using).

I suddenly started with pain in my index finger on the fretting hand. Could not curl the finger properly and struggled to use that hand for carrying or fastening safety belts, let alone play guitar. Had this for a few weeks now. I only practiced 30 mins a day, but it could of been caused by some other activity. I don't know what the problem is yet but I'm going to see my doctor. Be careful not to assume it'a tendonitis because it could be something else, even Arthritis. As the video says, see your doctor. Look after your hands!

Man, just saw this video! I've been playing guitar for over 15 years, and I had shoulder surgery (on my strumming arm) about 2 years ago, which put me out of commission for a number of months, and during that time, also being an avid weight trainer, I lost a lot of strength and muscle mass in my arms and I atrophied quite a bit. I couldn't even pick up my guitar let alone play it, because it was just too heavy. But once I recovered I went right back to playing like I normally would. But something just felt very off, since it was my strumming/picking arm, I had to compensate for how I sat or stood with the guitar just to play comfortably without getting sore or hurting myself. So for the past few months to a year I've noticed that my stamina is like half of what it was, and i get these really bad throbbing pains in my right forearm and up into my bicep. I can realistically play for maybe 15-20 minutes before stuff starts hurting. It's pretty devastating to be honest, to be hurt from one thing that prevents you from doing something you love only to have a different pain take over after. But, at least I can still play! Just have to do some things a little differently and adapt! I appreciate you making this video, and I appreciate you all sharing your stories! Everyone be well and keep playing 🤘💯👌

Forgot to mention, but one things that helps for me is a TENS machine, kind of like electro-shock massage. Playing on and off over the years and really got back into practicing last year. I wanted to master downpicking so bad, and could never get the speed up. I spent most nights practicing the usual downpicked riffs, with the aim really just to play along to songs for fun. Anyoo, I have developed a tennis elbow sensation in the picking arm. Perhaps its from holding the pic too tightly as I play. But I never had this issue before, until last year. In all the time I played at home some years back, I got the usual stingy fingertips, but that was it. In the mornings, the flaming around the elbow is bad. Then it wears away. I dont feel the pain when I play, but it happens overnight when I sleep. I worry that it will be long term, and I dont enjoy playing because of the aftermath of inflammation.
