I’ve gone through all 1000 UWORLD, NBME’s, Anking, and AMBOSS, yet I scored 70 on shelf. To be fair, it was my very first shelf but I really needed how to navigate such long question stems… I wish I had seen these videos before I started clerkship 😮😢

5:57 Myeloperoxidase is NOT present in bacterial cells . The antimicrobial effect of dapsone is well known in the treatment of ( Mycobacterium leprae ) . It inhibits the synthesis of " dihydrofolic acid " in the mycobacteria by competing with " para-aminobenzoic acid " for the active site of ( dihydropteroate synthetase ) enzyme ! The same mechanism is employed by sulfonamides in bacteria . While the anti inflammatory effect of dapsone is well known in the treatment of ( dermatitis herpetiformis ) . It reversibly inhibits the neutrophil myeloperoxidase (MPO) ===> impaired oxidative burst of neurophils & release of inflammatory mediators extracellularly !

G6pd is x linked recessive. Rather rare for a female to get it unless her other X chromosome is inactivated or she has 2 of the recessive alleles on her X chromosome. I get your reasoning on how to answer the question but this pathology does not match the answer you seem correct well

This G6PD deficiency he is talking about in the context of the question is not the genetic condition. It is as a result of whatever drug she is taking. He did mention dapsone which among others can lead to this.

this question is meant to be missed. The minute it mentions female, I am not thinking about G6PD def, especially with only 80 secs

I picked D. I thought the secondary diagnosis causing the icterus could be a primary biliary cholangitis which can be associated with celiac disease.
