I think a scissor lift mechanism replacing the truck's chassis, sort of like those food/beverage trucks at the airports use to extend up to the plane's loading height, is the solution. This way you can board at ground level, and then elevate the truck to the desired height.

Rope ladders are a common way to get in tall boys. You just need to attach the rope to the lower seat bracket so your weight creates handles in the rope from the seat to the door jam. You can add jump rope handles there to make it more comfortable. Then you can roll it up and tuck it between the base of the seat and the door.

Moon boots for sure with throw down rope ladder to help you get up, then just throw those moon boots in the back and put on some regular shoes you have tucked away in the truck
