Thank you, Dr. David. You are the only one and first one to tell a patient how to recover using the basics. I have made all the mistakes by continuing to exercise even the leg and arm hurt. Thanks a million for telling me to give the body time to heal first before engaging in exercises .

Hi @vzcorner I am suffering from the same with grade 4 , all doctors suggesting surgery, I have very little baby to look after bit hesitant for surgery , may I know these exercises provide some relief or helped restore cartilage in grade 4 .

@@HarpreetKaur-ti2xp These exercises are basically postural correction. Yes it reduced my knee pain . Also try ice packs on knees. Wish you health.

My doctor and physiotherapist don’t know why I suddenly have knee pain . Doctor gave me pregabalin and dicoflenac sodium and physiotherapist gave me 4 exercises to do that would be okay to do whether I have arthritis or Runners knee. It’s been over a month and I’m basically very limited and it’s starting to affect my mental health. Im so glad I discovered your channel today . I watch a lot of you tube videos lately to find a solution to at least get me to get I my car and drive again. 👩🏻🇨🇦

Dr.David. been following you now for a month and actually feel better. all that you have said regarding the medical community has been suggested to me such as lose weight, increase advil use, strenghthen quads. other than watching my diet which i feel better doing, i ignored the rest. and NOT jumping to surgery after two visits to ortho. thank you so much.

I appreciate your explanation of how central strengthening the glutes is to treating CP, PFS, and knee arthritis. I am doing this because targeting the quads and VMO in the past hurt.

Helloo.. I am a 27 yr old woman recently diagnosed with chondromalacia.. My Dr advised me to do an exercise like sitting on a chair and raising legs while counting 1 to 10...I am doing this every day... Isn't it a good one to correct chondromalacia?😢

Thanks for these videos. When you talk about cartilage healing in this situation (since chondromalacia patella is supposedly a permanent condition as far as the rough cartilage unless surgically changed), what do you mean by healing? I assume you mean a reduction or removal of symptoms (aside from noise perhaps). From my experience (grade 3), having strong legs made me have no inflammation ever in my life in my knees despite the condition, no matter what activities I did or how long, until I lost strength and muscle after an injury and then it became very inflamed for a while, probably due to all the extra pressure on the joints without that support. Gaining strength back has helped tremendously at getting back to having none. My knees always have been noisy, but without limitations or inflammation, that doesn't bother me.

@@HarpreetKaur-ti2xp I wonder if he means just a reduction or elimination of inflammation/pain from it. My legs were always strong, so despite my noisy knees (grade 3 here btw), I never had inflammation from it a day in my life no matter how active I was or what I did or for how long. Until I lost strength/muscle after an injury, then without the support on the joints it was terrible! Gaining strength back in time has helped so much. Cortisone for me did not really (and I had a crappy reaction with it for days). I figure once I get it all back, hopefully it'll be eliminated once again and just go back to nothing but noise. Best of luck with your situation! Dedication and consistency hopefully can help you with muscle gain to support the joints.

Mr David, wow thank you! You have really opened my eyes! I'm in my late 40s and this is my first injury - pretty severe CP. I'm feeling frustrated though - as you're saying, no quad strengthening and this is the majority of what my PT has me work on at home. I don't know how to tell them they're wrong, etc... I'm just feeling a little bit lost. Sometimes I think I should just work on this on my own. But it doesn't feel good to just quit PT.

I'm really really struggling. I'm 30 years old and my whole life has been turned upside down by knee pain. It's been 6 months now. Loads of MRIs, Xrays and bloods and they can't find anything other than wear and tear behind knee cap and fluid in my right knee and hip. I can't drive, if I do even a 5 minute journey it sets me into a flare. My other knee is having the same problem now too. I can't walk my dogs more than the end of my road. Some days I am bed bound by pain. I did start by resting for 2 weeks then exercising my quads as my physio recommended and now I think that might be part of why it's gone on so long. I was only aggravating it more. I can't go on like this it's too much. Right now everything hurts. But I will focus on my glutes. Thanks so much for your videos

Start with an anti inflammatory diet. Sadly that also means coffee, tea and alcohol. Will let the inflammation settle so you can heal.

@@TheJollyBot I drastically changed my diet to anti-inflammatory and I'm working with a physiotherapist, unfortunately I'm coming up to a year of being in constant pain in my right knee / leg / hip - however although I have a limit (which a 30 year old shouldn't have) I am walking further than I was able to before, and it is not at the point where I'm bed bound as often as I used to be, so I'm hoping that even though it's slow it might be improving very slightly over time. I am nervous about things going downhill again over the winter. The doctors still don't know what's going on.

Hey man really helpful video I’m 16.7 I got PFPS Oct 2021 till Current day by running and playing too much basketball with weak muscles and neglected the pain for 10 months until Aug 2022 I had enough and entered my 3rd physical therapy place. I did physical therapy 2 times but I just kept playing basketball and running with the same intensity so I never improved well recently I did stop everything and just focused on the pain and icing ect I say massive improvements with PT from Dec-Feb when i stopped and my knee pain was 90% gone. My only issue is I never went back to playing basketball or running not even cycling and now it’s end of March and my PFPS Or chrondomalcia idk if they are different I just assume I have it since it’s pain in knee cap, it hurts only to the touch when I press down on it. Now I worry that I never did anything of exercise so I may have set myself back i ice 3X a day both knees have this issue and it only hurts when I press down with pressure. Any tips and help? I’m desperate I need answers

You should get a MRI to check the condition of the cartilage behind your kneecaps.

Thank you for the information but recently I met with an accident and tore my ACL completely and dislocated my knee cap which caused little instability in knee cap (i don't have any knee instability caused by an ACL tear, so we chose rehab without surgery) and 1 or 2nd-grade chondromalacia... So, how should I work on my ACL and chondromalacia without injuring anything more? Please help

Hey doc, I've been doing PT for a couple of months now and even went and got hyaluronic acid + corticosteroid injections. I've seen a certain degree of improvement but nothing drastic. I'd really like to keep up with doing hypertrophy focused leg work in the gym and, obviously, the quads are a big part of that. If I can do a partial range leg extension, do you recommend against that? Same with squats, ex. squatting down at only a slight bend.

Thank you for this very helpful information! I’m two weeks into my first very painful flare up of Chondromalacia Patella. (I was diagnosed with it when I had an ACL repair but it never caused me any symptoms.) I’ve been trying to avoid ibuprofen so instead I’m using the Donjoy Iceman machine that they gave me for my ACL surgery. The cold water therapy works great for getting rid of the pain and swelling. At first I had to chill my kneecap pretty continuously and now I’m using it on and off throughout the day. I’ve also been taping my knee with KT tape which also seems to help. Using these techniques I’ve been able to start doing my glute and foot exercises several times a day. I’m making steady progress but I still can’t be up on my feet for any great length of time without the pain and some swelling returning. My question is, are these things okay to do for pain relief and how long does it usually take to recover?

Hey @riariaria, sorry to hear that you're dealing with chondromalacia patella :-( Yes, what you're doing is the best place to start getting the pain under control. Resting more at this point would be good to speed up your pain relief and decrease your swelling. You need to get off your feet as much as you can to allow the knee cap cartilage to rest and heal. You might even want to get on crutches or a cane for a time. You will be delayed in healing if you keep aggravating it when you stand or walk enough to make it hurt and swell again. It can take anywhere from 1 to 3+ months to heal from this properly. Once you've been able to tolerate walking and standing for more than 20 minutes without getting worse, then you're ready to start exercising to fix the root problem.
