What is the definition of severe acne scars?
Dermatologists’ viewpoint on severity is different from Medicare’s. Severe acne scars, as defined & accepted by procedural dermatologists are ‘grade 4 acne scarring using the Goodman-Baron scale.’
Grade 4 scars: non-distensible scars that cannot be covered up with make up or the normal growth of beard hair in male patients that are visible at conversational distances of at least 50 centimetres.

How to cost effectively treat acne scarring?
Here is how patients can receive effective scar revision with less costs. In most cases I only require one major session to improve acne scars. My mode of practice is to subcise in multi-levels, releasing as much tissue as possible. Laser resurfacing TCA-excisions-RFM are all conducted in this one session. You may require an additional session or two if
You have extensive scarring, example include scars on the temples, forehead & cheeks
You re-attach scars, most common cause is persistent inflammation due to acne
Your have active acne, however mild, as this distracts from collagen production
You wish me to proceed with minimal downtime

What are other treatments we use for scar revision?
After one to two sessions of the above, I transition you to my senior nurses. They provide cost effective & effective treatments including-
Cannula & saline subcision: Low risk, minimal downtime procedures to break up mild scars.
Biostimulatory injections of ‘fillers’: by definition, hybrid hyaluronic acid molecules that stimulate your own collagen.
TCA Paint: for deep ice pick & boxcar acne scars.
CO2 lasers: using one of 3 devices: best for box car & rolling scars.
NA Hybrid 1570 or 1550 lasers: for mild scars. This treatment has minimal downtime.
Manual microneedling: for scar on the forehead & temples
RF microneedling: for deeper rolling acne scars that require more collagen remodeling; cheeks & jawline

Hi Dr. I am curious to know if most People who come for acne scars treatment are not having any active Acne? If they have then whats the fastest way to treat mild acne so that the treatment of scars can be started. Not all acne treatments stop acne fully so Antibiotics are the answer?

I suffer with terrible deep athrophic scarring that has also left some unwanted post inflammatory hyperpigmentation and almost lumps in my skin. I have had 2 sessions of microneedling and my current routine involves… 1 - cleanse 2- hylaronic acid 3- vitamin c 4- moisturise And at night I use azaelic acid. I have really sensitive and dry skin can anybody recommend anything for athrophic scars in terms of serums or retinols to help aid fade and heal them I have tried everything on the market however I have never used a retinol . Recently my scarring has made me feel more and more uncomfortable in my own body and I’m in desperate times for some help. Thank you .

I hope in the second part you cover how to treat "Hypertropic Scars on Nose and Chin" formed by overproduction of collagen and healing the acne before it settles to surface level. Also hope that you compare Alma Pixel Er:YAG 2940 vs MNRF. Some of the research papers I have read say that MNRF treatments are very "short term" effects but then when I looked at the publisher, they were published by laser companies. So don't know if it's reliable. I have done one Alma Pixel Er:YAG 2940 at setting 9. Don't know if I should do MNRF or continue with Er:YAG 2940 for next sessions. Thanks for all you info and help. :D

You mentioned that you don't recommend pens because the motor can become bad over time. So what? It may only reduce the speed at which needles go into the skin right? At worse the speed will be same as a stamp but it can be safely assumed it will still be much better speed than stamping. So what is really wrong with pens?

Cant use retinol together with the Stamp but one thing i can say is with consistency there is hope. The Banisher 2.0 changed my hubby's life and faded his boxcar scars

Dr Lim, can you talk to how diet helps acne scar healing/supports treatment? I've seen video about acne management, but not about how it impacts scarring, scar treatment, etc. I have a young adult relative who is going through acne scar treatments (3 months out) and his diet is entirely composed of fast food, sugar, sodas and monster drinks. Zero vegetables. All refined carbs, processed food. The less results he sees, the more junk he eats and refuses to believe healthy eating has any impact. Is he just counteracting the treatments? Sugar's impact on collagen production? Glycation? So much focus on acne management and diet, but not enough on diet and scar healing.

ive read the study and it was not correlation equals causation. people actually need to read the article!!! It was because the esthetician microneedled over ACTIVE ACNE, in which Dr. Lim says NOT to do. vitamin C is super effective for fading post acne marks as it inhibits the tyronaise as dr. lim mentioned.

it just 'happened' that the esthetician used vit c ontop, and guess what??? that particular vitamin c serum caused allergic reactions in the client... so now everyone thinks vitamin c is terrible to use after microneedling, but no one read the actual research paper????

Haven't seen anyone talk about what I have, and that is cortisone damaged skin. Derms injected 'cysts' I used to get decades ago, the skin was forever 'thinned' and thus 'depressed'. Redness talked about, goes away on its own in time, however my thinned skin a bit shadowed. Think of skin like an onion, and one layer or two removed on your face. How on earth to fix this type of scarring?? Case of skin acne rosacea 'treatment' worse that the disease! One derm tried to say it would even out, never ever did!

A big YES to the Banisher 2.0! It is indeed a very simple, cost effective and SAFE way to treat acne scars at home. Thank you for reiterating that going deep with bigger needles does not mean it is effective but it causes more trauma. Hi there! We don't really recommend using the Banisher on raised, keloid or hypertrophic scars as using it may cause further aggravation on the skin. It's best to consult about this with a dermatologist first just to make sure.

Isotretnoin aka accutane is about the only thing that cleared my skin. Started getting acne at 20 and tried many many things. Then my new doctor put me on 10 mg isotret and eventually upped it to 20 mg for 6 months. I stayed acne free till I reached 35.

I also have scar marks from acne.. I was recommended the Vitamin C Creme from banish as I also have oily skin. They recommend using the Banish Serum if you have more of a dry to normal skin. I haven't tried their retinol serum yet but I heard that it is also good to use :) hope this helps

I'm a year post accutane and I was told not to use anything whilst on accutane, especially anything with vitamin a. I think I was told to wait at least 6 months afterwards to use something like retinols

I have been using banish for over 3 months and seen no improvement whatsoever which I was very disappointed and frustrated about. The only thing I can say it actually did was keep my acne away, that's about it

My son has acne and is currently on Epiduo Forte. His skin became red and irritated bec of it. Banish sunscreen, vit c creme and gel saved his skin!

you can use glycolic acid and microneedling is safe on pregnant women (as long as u dont put a non pregnancy safe serum ontop of it)

you can use BANISH as it's helped for my scars (like smoothed out the edges) but if you have really deep scarring it wont cure it. My scars look 70% better but in some anggles u can still see the scars

I've asked about this before. Unfortunately, they don't recommend doing microneedling if you are prone to keloid scars since it will aggravate the situation more.

Tried 6 treatments of microneedling, fully ablative Co2 laser and retin A. I look the same. Pictures from the surgeon's office prove it. I feel like a failure when I seem to be the only person who doesn't respond to treatments that are supposed to stimulate collagen. Even a 30-50% improvement would have been something to be excited about. I'm thousands of dollars dumber for nothing.

@robert hart I don't know what that is. Honestly my skin has lost so much elasticity that I'm just considering a mini facelift. I'm scared to spend anymore on scar treatments. I will look it up though.

@@theHappyTrucker yeh I totally get where you’re coming from. I had the laser which cost me almost $2k, and don’t feel I had as much change as they promised. I then did 3 treatments of Venus viva at about $250 per treatment and have seen much better results, and it’s significantly less downtime, literally one weekend

Usually if you have deep scars like ice pick, microneedling and co2 laser won’t respond because they’re too deep. You should do Tca cross at like 80-100% which will fill in the scar and make it more leveled

Based on what I have read and researched before, there is no age limit to using the Banisher but it's best to be used by someone who is 15 years old and up and with adult supervision.
