The cathedral was built between 1151 and 1227 in place of a pre-existing building; inside, in the apse, there is the valuable cycle of frescoes by Filippo Lippi Stories of the Virgin, painted in the last years of the artist's life, between May 1467 and September 1469. In 1491 it was added, by Antonio Barocci and his workshop, the portico of the facade, in Renaissance style, which had the task of giving greater magnificence to the cathedral. Between the 17th and 18th centuries, the interior of the cathedral underwent heavy renovations in the Baroque style. In 1608, Maffeo Barberini became archbishop of Spoleto and immediately showed his intention of wanting to change the appearance of the cathedral. Later became pope with the name of Urban VIII, he was able to complete his work thanks also to the help of his nephew Francesco Barberini, who in turn became archbishop of Spoleto. The basilica layout was so upset, the central nave and the two side aisles were completely rebuilt.
The internal reconstruction works were only completed towards the end of the 18th century, when Giuseppe Valadier was given the task of designing the aedicules in the aisles, the head altars of the transepts and the high altar. In the central niche of the facade there is the mosaic Christ enthroned between the Madonna and San Giovanni Evangelista (1207).
Inside there are numerous chapels; the altars in polychrome marble and semiprecious stones (agate, carnelian, jasper, lapis lazuli) of the aisles and transept are placed in a symmetrical and specular way. As well as the main altar are the work of Giuseppe Valadier, a neoclassical architect, commissioned by Pope Pius VI. In the fifteenth-century apse there are the valuable frescoes by Filippo Lippi (1467-1469) depicting Stories of the Virgin. In the Chapel of Bishop Eroli, an area used as a baptistery until 1845, there are valuable frescoes by Pinturicchio, works created in 1497. The Chapel of the Most Holy Crucifix houses a Crucifix, the work of Alberto Sotio (1187), painted on parchment inserted on a wooden support , a beautiful example of Christus triumphans. In the Chapel of the Most Holy Icons is kept the Byzantine Icons of the 11th-12th century donated by Frederick Barbarossa in 1185 to the city as a sign of reconciliation, after having sacked it thirty years earlier. The chapel is adorned with stuccoes, frescoes and a tabernacle by various artists.
