Spondylosis is a broader term referring to degenerative age-related conditions affecting the spine such as osteoarthritis or degenerative disc disease. It is often caused by general wear and tear of the spine, such as vertebrae compressing over time and deteriorating the discs found between the vertebrae. Spondylosis can lead to compression of nerves in the spinal column, causing severe and chronic pain in the back and extremities.
Ankylosing spondylitis (AS) is a type of arthritis in which there is long term inflammation of the joints of the spine. Typically the joints where the spine joins the pelvis are also affected. Although the cause of ankylosing spondylitis is unknown, it is believed to involve a combination of genetic and environmental factors. More than 90% of those affected have a specific human leukocyte antigen known as the HLA-B27 antigen. The underlying mechanism is believed to be autoimmune or auto inflammatory.

I appreciate you discussing this. Unfortunately it is typically not diagnosed until years and years of pain and going from doctor to doctor. Please note IMPORTANT that those who have Ankylosing Spondylitis will have it for years and years before seeing actual proof on an X-ray and even a MRI. It’s best to be treated at this time so to prevent your spine from forming bone growth. This is where the problem lies. Doctors don’t see any proof on the MRI and certainly not on an X-ray, therefore do not give the diagnosis of Ankylosing Spondylitis. But meanwhile your spine is stiff and in pain. If you don’t have proper treatments, you will get the degeneration and bone growth. It’s too late once you have the actual proof on the X-ray. Treat it so that you prevent getting the proof on the X-ray, that’s the whole point. Your primary care doctor will not run proper tests to determine this. In fact the tests they will run will say nothing is wrong with you. Don’t accept this answer. If you have constant back pain and morning stiffness in your spine for more than 10 minutes when you get up and it persists for more than 3 months, you very likely have Ankylosing Spondylitis. Educate yourself and then seek out a doctor with knowledge and experience in this disease. I recommend a rheumatologist. With knowledge of the disease and the name for what they have, they can then maybe find a doctor to treat them. For example a rheumatologist, functional, or homeopathic doctor. I personally suggest staying away from the orthopedic surgeons and neurosurgeons. Having surgery will not decrease your pain. Unfortunately though people with this condition are desperate to find something to lessen the pain and so when surgeons boast that it can decrease your pain, those who are suffering, are willing to gamble with having surgery. It just makes things worse. So thank you for explaining and showing this information and exposing the name of a quite common disease.

All doctors need to be on the lookout for this disease more . And earlier diagnosis would be so much better than finding out at 50 and going all these years being told it's in your head or looked at as if I'm talking crap about the amount of pain I'm in . More training for doctors please . It's not just a male disease . My mother has it , I have it , now my son's suffer with it and most likely my daughter and my grandchildren will have this disease to . Let's get it diagnosed early to stop damage .

Thank you for this. I was diagnosed in 2004 through a blood test. I can't use oral anti inflammatories because I have reactions. I've been on 5-6 biologic drugs that started to work then would stop. I decided to do the elimination diet for anti inflammatory foods. I cut out gluten, dairy, eggs, and sugar. I'm in my 4th month now and I have less pain, more energy, and sleep better. I'm still eliminating foods that bother me. I'm feeling so much better. My body isn't swollen either. Mine is hereditary. Ou of 5 kids, I got it from my dad's mother. Diet plays a big part!!! My AS started in my neck. It's common in females hence why they are diagnosed later/less. They are looking in the lower back (and until recently wouldn't even do that if you were female - it was said to be a male only disease). My AS started showing up in my x-rays at 40. It really shows now that I'm almost 50. When I was diagnosed at 30, it wasn't even showing in an MRI but they found it in a bone scan.
@jacklynnroskey1981
@jacklynnroskey1981 Жыл бұрын
Did it show up as arthritis in the neck first? Because my rheumatologist said I have a lot of arthritis in my neck. I am positive for the AS gene and psoriasis. My rheumatologist thinks it's osteoarthritis, not psoriatic arthritis or AS. She says that I can be positive for the gene, but not have it. My psoriasis is out of control and have been on otezla for 3 months and things are not getting better. I actually feel worse.
@smokeyangelav
@smokeyangelav Жыл бұрын
@@jacklynnroskey1981 yes though I have had two rheumatologists that didn't really treat my neck as part of it and they wanted to focus on my Sacroiliitis. That was easier to manage than my neck issues. I wouldn't wish that pain on anyone. My rheumatologist retired so I had to go to a new one and redo the diagnostic process. When she sent me for my x-ray, I was shocked to see that she didn't request one for my neck. That's always been my worst area. Not much AS was found of course. I was later sent and they saw my neck had quite a bit affected by AS. I do not know why they immediately look at SI joints and the lower back. I guess it's more commonly found there but they need to look at the whole spine. Biologics can take a while to work. I have been on Taltz and Humira for about 5 months now and I feel the same as you. One of them are causing little bumps on my skin but they wanted me to continue to see if there's improvement. I may have had a little this month but the weather always affects it and it changed. I also have increased spinal pain with hormonal changes during my cycle. I would say as long as the treatment is the same, I wouldn't worry too much about it, but if it denies you certain medication that insurance won't cover due to diagnosis, then that would make a difference. I wish there wasn't such a shortage of rheumatologists. I waited over a year to get into my new one after mine retired. It would be nice to get a second opinion. Sadly when you see one in a practice, the rest won't see you if you don't like one so they don't steal patients when they cover for each other during vacations. Seems counter productive since it forces you to another practice. I'm starting to rant so I'll stop there. I hope you find relief soon!

My life changed when I quit eating inflammatory foods and began exercising. It's hard at first, but absolutely essential. I'm 35 now and feel better than in my 20s. In my 20s I thought I was dying I hurt so bad and head severe gi issues with it. I was sure I had cancer and had every test done known to man. I was preparing for my death, I could barely do anything with my kids and of course they kept trying to diagnose me with depression anxiety etc, but my pain is what made me depressed. I was a very active athletic person throughout my teens and 20s.

@@Beamlight7 I'm not a doctor of course and this is just opinion, but I would avoid humira at all costs. That is the only medication that helps it aside form diet and exercise. If you have excess weight you'll want to lose it. High levels of stress flare it up too. Another serious thing is if you are active make sure you stretch good too. You have to be careful with excessive strenuous activity too. For example, I used to love weight training heavy. Cant do that at all. Also loved basketball too tough on my joints now. I do a lot of body weight exercises, lighter weights, and I run which seems to loosen the joints up.

I keep exercised, do Crossfit, very very slow steps for ward with that,getting strong helps. As well, I do well when I remember to do yoga. If i don’t keep up with fitness, I get very stiff and sore. I didn’t know I has A.S, till after chemo treatment for breast cancer and I reacted badly with chemo, it enhanced inflammation I already had in my body, when I told my oncologist, he treated me like I was over reacting, because a person without A.S would not have had that bad pain reaction I did with chemo.So, maybe oncologists could be a bit more sympathetic to looking into why one would suffer so badly. Thanks for the educational video.

HAve searched help 37 years, got diagnosis only after 24 years, along with Aspergers, which may be a reason why nobody helped me. PSychic problems often give worse treatment or even like in me, neglect. As insult to injury, I got sleepless by neighbour's barking dogs for 12 years, which was the final breakdown after abuse trauma in psychiatry earlier, that turned my Bekhterew's aggressive, degenerative, and I got eating addiction from stress, as well a s allergy, fear of doctórs , lost hair, etc. Fighting for personal assistence, still waiting for success, and having to get back my home that they took, as well as regaining my savings, small sick pension and rights, reputation etc. They have mad my tragedy into hell, and I have been burnt out for 20 years, worsened to stage 3. i have lost motivation and courage to go on excercising, never getting support, therapy, stress relief. No energy or nutrition balance either, like he says but worse. Have been freezing fr 20 years too, while skin often feels hot as a stove: I am losing warmth and can't produce it well. HAve to dress warmly always. Am so afraid, although praying and working at my thought-feelings.This video made me doubt that I can be helped or even live much longer, if all the horrible prejudiced, false assumptions-mal"treatment" is not turning. I have lost so much time going for the wrong things/people and having to fight for my rights to help and writing, waiting, recovering from abuse from many, many people/institutions/authorities.I don't get how they can torture an innocent handicapped who has done more than all to help herself, help others and get help to get well or better functioning. I'd be happy and grateful for good support , friendship, healing comrades. I want to heal or stop the progress.

I'm 71, and was diagnosed by my orthopedist with ankylosing spondylitis a month ago after several weeks of persistent, severe back and hip pain. I'm very active, play golf nearly every day (3 handicap), and until recently, have had only short, sporadic periods of pain lasting less than two weeks. I'm now told my neck and lower back are essentially fused now, he said, and the only course forward is seeing a rheumatologist. Online research has proven helpful, but I still have a ton of questions. For example, can AS impact your heart? I've read it can cause arterial blocks. At 67, I had a widow maker heart attack, with a completely blocked and calcified LAD. Yet, my cholesterol has never been above 140, and my family has no history of heart disease. I've also had severe pain and extensive swelling for a period of several weeks in each knee, yet MRI's showed a generally healthy joint. Thanks to anyone who has suggestions, and thanks to Dr. Mandell for his very clear description of the disease.

It bothers me that everyone talks about the sugar and "processed foods"... Why is it that such a small percent of the people that eat McDonalds and DingDongs have AS? Sure they are over weight and might have other issues, but so few are affected by this specifically. I can't believe diet has much to do at all with this. I guess I'd like to see proof, rather than promises. I eat mostly salads because my wife makes me, organic dairy and meats, and occasional pasta... Maybe McDonalds breakfast every couple months. To think that is whats causing such intense pain and for so many years, is sort of ridiculous. Also, that same intense pain doesn't exactly allow for "getting up and exercising". Everyone that doesn't have one of these conditions is always an expert. Always promises "just be more healthy" will somehow make it all better. Like we can just wish it away through hard work and determination. I can tell you after more than a decade of being misdiagnosed by people like this guy, that know everything and promise the world, NO amount of diet or hard work will ever stop these diseases. You can't just motivate away degenerative diseases. Its almost shameful that these people are more concerned about "thumbs up" and subscribing than producing anything helpful. Not trying to be jerk here, just saying think about it. Don't eat crap, don't smoke, don't drink too much, and keep your weight down. Sounds like 8th grade health class doesn't it? Not sure we need more guys in their bedrooms that must have their mugs in the frame the whole time... Count me as one of the thumbs down.

Mike S, IKR.... I'm No Pollyanna and I don't see a positive damn thing about having ankylosing spondylitis! I have posted several times on A,S KZfaq comment sites so read no further if you have previously stumbled across my RANTS + bad attitude comments. I have suffered from ankylosing spondylitis for years but due to being a female that is HLA B27 negative, I was misdiagnosed many years with Dish disease. I did not receive an ankylosing spondylitis diagnosis until I was already severely handicapped . I was severely fused in some areas of the spine before I found a rheumatologist that treated me with tnf meds. I don't believe doctor approved diets and exercises will always help but probably won't hurt. Before my diagnosis, I was trying to exercise and lose weight .I continued to work trying to stop my loss of Mobility but I still became handicapped due to the Bone Spurs& Fusion, & spinal stenosis. The pain ,stiffness , the loss of range of motion was hell,. I just kept on getting sicker! A.S. effects some worse & faster than others. Diet & exercise aren't enough for some of us. GETTING treatment with tnf meds early on before bone spurring & spinal fusion begin might be the only help cuz the damage from bone spurring & fusion cannot be reversed. My son inherited my HLA B27 Negative ankylosing spondylitis but fortunately for him, my rheumatologist saw him & began early treatment with Enbrel. His stiffness and inflammation improved and he doesn't have significant bone spurring. He still has bad days. Even with good range of motion, he still has some soreness & IBS symptoms from the A.S. Good luck ,Mike. I'm hoping you find a treatment plan that works for you. I might be happier with a good attitude but I'll still be sick LOL...... A,S, has seriously kicked my ass. Learning to accept my situation has helped. I have learned ways to become more comfortable and medical equipment that helps with my adl's. My life as I knew it 20 years ago has completely disintegrated so I've made a lot of changes to accommodate my disease. I pretty much know what will work and what won't. Unfortunately for the chronically ill often their friends, family, and sometimes complete strangers will say stand up straighter and eat your veggies so you will get well. BTW, it's okay for you to tell those people to f*** off. 😄

I changed my diet cut out sugar, dairy, and wheat and it helped my general health and gut problems but did not do much for pain....I've been in agony for many years...it has destroyed my life...I just lay in bed 24-7. The pain is unbearable much of the time. Pain meds only work for about an hour or two every six hours.... but at least it's something.

@@robinbradley2229 Thank you, Robin for responding. I am very sorry you have had to go through so much suffering. I too have a child...a daughter...who has symptoms of the disease....we are both HLAB27 positive...and her daughter, my grand daughter, has symptoms as well and she is only 12. She was is HLAB27 negative and was diagnosed with juvenile arthritis. I do not have fusing, only inflammation. I've had symptoms since I was in high school but it has only gotten severe in the last ten years. Chronic fatigue and GI problems too. I will definitely keep the ablation idea in mind ... sounds very interesting and I will definitely discuss it with a doctor. I am currently battling cancer ... medication for that is working so far... so once this is in total remission I will tackle my pain issues. It is all so overwhelming that I could just scream. I do take narcotics for pain but my doctor has a limit on how much he prescribes. It just isn't enough but to be honest....I would rather have my pain treated in another way because of the side effects like constipation and drowsiness. I am so happy you found pain relief with the ablation. God bless you and your son, I will keep you both in my prayers.

This was very helpful to me to understand Ankylosing Spondylitis and how my x-rays and blood test related to discovery of this. Unfortunately I was misdiagnosed for several years, at 27 the proper diagnoses was given but I was told that I would have a long time before I had to deal with the fusing and severe pain. Newest diagnoses determined that I have rapid progressive Ankylosing Spondylitis at 43 years old and fusing in my neck has made it impossible for me to turn my neck, look up or down. Thank you for explaining the difference between the 2, now I am taking TNF Humira but have been on it for 5yrs now. I still have severe pain and I'm not sure how to over come the pain. I have also changed my diet as recommended in the video but it's only been a few weeks.

@@DanielHarveygamers69 surely you will, please join the group on FB ( akylosing spondylitis natural treatment ) I had severe sacroiliitis, edema, sclerosis and erosion, walking was extremely painful, actually impossible is the word. The allopathy drugs didn't really help. I did lots of research and understood that the gut is connected to inflammation that occurs in our body. I highly recommend you joined the FB group, loads of people have found their AS gone in remission. Its been 3 weeks I gave up sugar, gluten, processed foods, non veg, starch, and diary. And I can walk now, trust me... AS is not a genetic factor, its our environment such as the chosen foods, that creates autoimmune diseases. And autoimmune is related to our gut. I used to have loads of gluten, sugar and processed foods. I suggest u also start taking better amounts vitamin D3, L glutathione, but first start diet, detoxify and then move towards healing gut..... check with a naturopath, they can help u better

I apparently have this going on but my posture is fine. I'm always conscious of my posture, but i'm experiencing so much pain and weakness and muscles loss. something has triggered a sudden onset of symptoms from joint to muscle and nerve pain throughout my entire body. it seems like my body is suddenly shutting down and i'm getting smaller and weaker every month. in just 6-7 months i'm down 30lbs of muscle and barely able to function. going from active and fit to having trouble lifting a glass of water. this is crazy and scary how sudden and fast this is moving and affecting me. it's messing with my digestive issues and i'm not absorbing nutrients and exercise seems to make me worse flaring up my muscles and joints!

@Tina Carmony Yeah I started that since early 2021 4-5 months after everything started. did a few 3 day fasts and then for months did OMAD. didn't seem to help much. doing traction on my neck seemed to help the most. something's causing pressure in my neck and cervical discs and messing with my nerve function which has messed with EVERYTHING in my body especially digestion. which i think led to leaky gut issues. I may have had it for years but very mildly and I just aggravated it and it set off all this craziness

This may be a silly question but can you have both? I have DDD, bone spurs, straightening of the cervical lordosis, chronic back pain, chronic pelvic pain, and have been searching for years to figure out why I walked in a forward leaning position that required me to use a walker for posture and balance to now not being able to walk due to loss of balance, falling, and extreme radiating pain. I now have to use a wheelchair. I suspect I may have ankylosing spondylitis since I have many of the symptoms including sensitivity to light, hip pain, back pain, hunched back, degenerative spine, and pain that has not been treatable. What do you suggest as far as these symptoms? Could I likely have AS?

I was on Humira but I was having so many infections. My new rheumatologist told me that he thinks because I have so much new bone growth that I won't have any more damage. He doesn't think the Humira is anywhere near close enough good for the risk. The only thing Humira will do will prevent further kyphosis. Can you think of something natural to help relieve the dope sick feelings i deal with multiple times everyday that wont add weight or inflammation? I dont care if it tastes like crap if it works. I sometimes get pain, especially if I turn my head to the left for an extended period of time. I also feel a little numbness in my right hand and face periodically, and some hearing issues in my right ear. The other bone issue is a recurring subluxation in my spine at or near the top rib. I have been doing the spine and neck exercises you prescribe, and they've been helping me a lot, also to correct pelvic tilt. I'm wearing better shoes now too, and it's alleviating the foot pronation. What can I do about my twisted bone in my neck? It seems to be stuck that way. Can this cause bigger problems down the road? My chiropractor didn't know what to say. Any tips or advice you can offer are greatly appreciated.

AS is a chronic autoimmune disease that results in fusion of the spine. Eventually the fusion will involve the whole spine, from the neck to the tail bone, stooped forward , making it impossible to even look a person in the eyes. It also fuses the joints between the ribs and the sternum with makes the rib cage rigid so that the lungs can not move properly making it hard to breathe. All of this is caused by inflammation, meaning severe pain throughout the body. AS also affect other organs and joints in the body. Often the heart and lungs are affected. Your fusion was made to make your life better. That's the difference

Hi I've got spondylolisthesis at the L5/S1 with a bilateral pars defect. I've been following a few trainers and therapists that are focusing on core strengthening rather than spinal fusion. Do you have any tips on continuing modified gym regimen or cancelling it entirely? Would love for you to do a video on spondylolisthesis and exercises to help with it. Thanks!

You can not cure AS. AS is a chronic disease. Don't let people fool you with remedies. You need to exercise and take care of yourself but you also need to see a rheumatologist and have a medical treatment. I'm sorry, I wish there was an easy way to get rid of it and I also wish people would stop trying to take advantage sick people just to earn money by selling herbs or books or whatever.

Enbrel saved me from being worse off. Without it nobody cared. Especially not the Federal Government or MD Healthcare. Then I started to go blind.
